[Duodenal cancer. A clinical-pathological study].

Seven primary carcinomas of the duodenum were observed from 1973 to 1976 at the University Hospital Hamburg; four in females and three in males with an age between 32 and 69 years of age. The interval between the first symptoms (epigastric pain, jaundice, pruritus, diarrhea, and loss of weight) and surgical therapy (duodeno-pancreatectomy) averaged four months. All carcinomas were resected radically from the macroscopic (intraoperative) aspect as well as from the histological findings. Local tumour recurrences which proved fatal occurred in five patients within nine to twenty-one months. One patient died of peritonitis and another of pancreatitis. The diagnostic mode has been changed since the introduction of endoscopy and retrograde cholangio-pancreaticography (ERCP). The consistent inclusion of the duodenum in routine gastroscopy leads to the hope that more carcinomas of the duodenum can be detected early.

[Pathology of ulcerative colitis (author's transl)].

The macroscopic and histological appearance, and the local immune response in ulcerative colitis are discussed. The main criteria for the differentiation between ulcerative colitis and Crohn's disease of the large bowel are reviewed. The risk to develope carcinoma in the large bowel is greater in patients with total ulcerative colitis than in the general population. Precancerous changes in rectal and colonoscopical biopsies are a useful parameter in detecting early cancer in colitis. A description of the morphology of precancerous changes in ulcerative colitis is given.

[Surgical diseases of the veriform appendix].

Various diseases can symptomatically resemble appendicitis. It has been proved that luminal obstruction is the most frequent cause of inflammation of the appendix. There are various causes of obstruction, such as scars, foreign bodies, tumors, granulomas, and dysfunction of coecocolon. Every appendix should therefore be histologically examined. The operative indication for appendectomy should not only include the time factor, but also anamnesis, complaints, and clinical presentation. It is inconsiderate to perform appendectomy as a prophylaxis against appendicitis. Finally, it is surprising that there have been almost no experimental studies on pathogenesis of appendicitis.

Immunohistochemical and electronmicroscopic observations on the local immune response in ulcerative colitis.

Inflammatory cell infiltrates in ulcerative colitis have been investigated by means of the immunoperoxidase method and by electronmicroscopy. Considerable morphological and functional changes of the local plasma cell population have been found. The absolute number of plasma cells is raised with a marked increase of IgG-cells and a relative decrease of IgA-cells. In particular complement (C3) has been demonstrated at the basement membrane of the surface epithelium and between epithelial cells. The significance of these findings, as a local humoral immune response, is briefly considered, with regard to their possible pathogenetic importance in aggravating and perpetuating the disease.

[Immunoblastic sarcoma (reticulum-cell sarcoma) of the gastro-intestinal tract in a case of dermatitis herpetiformis Duhring (author's transl)].

In a 43-year-old man who for 20 years was known to have dermatitis herpetiformis there co-existed, since 1974, a "dermatogenic enteropathy" with gastro-intestional symptoms. Several small-intestine biopsies demonstrated severe partial to subtotal villous atrophy with crypt hyperplasia, as seen in gluten-sensitive enteropathy. At post-mortem there was an immunoblastic sarcoma - so-called reticulum-cell sarcoma - in the upper jejunum and abdominal lymph-nodes, which was presumably the immediate complication of the sprue-like lesions of the small intestines.

[Carcinoma of colostomies. Investigation on Wistar rats and clinical observations (author's transl)].

48 surviving male Wistar rats, which achieved a double loop colostomy at the descending colon, developed spontaneous adeno-cardinomas at the proximal orifice of the stoma within 120-200 days. To differentiate the importance of fecal passage and eversion of the colon, the descending colon was separated from the fecal stream by a colostomy at the ascending colon, splitted longitudinally and inserted in the abdominal wall. Adeno-carcinoma arose in both positions, mainly within 120 days. Colonic mucosa, thus exposed, predominates to maligne transformation. The preferred development of cancer at colostomies, as seen systemically induced colonic cancer, is therefore explained to be of syncarcinogenic type. Similar lesions were found at human colostomies. The rareness of original carcinoma at human colostomies is caused by a more-years time of induction in relation to the high average age of patients getting a colostomy. In fact we regard this type of cancer as a biological phenomena comparable to the cancer of the gastric stump.

[Intussusception of the vermiform appendix (author's transl)].

Intussusception of the vermiform appendix is reported in a frequency of 0.01%. Different aetiologic factors are implicated, a directed hyperperistalsis against intraluminal obstruction is thought to be the pathogenetic mechanism. There is a lack of typical symptoms, very often appendicitis or caecal tumor is diagnosed. Concomitant inflammation renders recognition more difficult. The caecum must be resected if a malignant tumour cannot be ruled out. Appendectomy is performed in definite findings. Our case report may remind of this uncommon occurrence.

[Thymic carcinoid. Case report and review of the literature (author's transl)].

A case of a primary thymic carcinoid tumour of a 32 year old male is reported, and the previously published 25 cases are reviewed. The neoplasms occur in adults, predominantly in males: they are located in the anterior or antero-superior mediastinum. The only sufficient treatment is the surgical excision; through radiotherapy may be indicated in some cases, too. The tumour is characterized by the formation of rosettes, ribbons and garlands; sheet-like or medullary areas, and large clusters with central necrosis. The tumour is positive for the Grimelius argyrophil silver stain. Ultrastructurally numerous dense-core "neurosecretory" granules are observed. The formal pathogenesis is similar to that of carcinoid tumours of other locations; particularly of those arriving from derivates of the foregut, respectively from the APUD cell system. At present it cannot be decided whether this particular tumour of the thymus represents an autochthonic thymic carcinoid or simply a teratoma with a differentiation in one direction in the sence of a simplified teratoma.

[Nitrosamin induced neoplasms of the colon and rectum. Investigations on the morphogenesis in rats (author's transl)].

The morphogenesis of neoplasms (carcinomas and pracecancerous polypoid lesions) induced by N-methyl-N-nitroso-N'-nitroguanidine (MNNG) was studied on 154 male Wistar rats (including controls), from which a group received a colostomy. Operated and non-operated rats were treated by intrarectal instillations of 2 mg per kg body weight MNNG twice a week during a period of 210 days.

[Epithelial dysplasia in ulcerative colitis. Histological possibility for the early diagnosis of "colitis"-carcinoma (author's transl)].

From 249 histologically examined cases with ulcerative colitis (within the years 1967 to 1974) 20 cases were selected, which had an onset of the disease before the 20th year of age and/or a duration of more than 10 years. These cases were checked-up for "prae-cancerous" epithelial dysplasia. Four of these 20 cases showed medium to severe epithelial dysplasia together with carcinoma (= 1.61% of the entire collective). In 2 cases low grade to severe dysplasia occurred in different parts of the colon without demonstrable carcinoma. In 1 case epidermoid epithelial metaplasia were found in the splenic flexure and in the rectum. The biological significance of these metaplasia remains unclear. With reference to the formal pathogenesis these lesions are probably "indirect" metaplasia. The early diagnosis of "colitis"-carcinoma can be improved by systematic colonoscopic examinations resp. by colonoscopic biopsies. "Prae-cancerous" epithelial dysplasias, which occur in a high rate together with carcinoma, are found not only in the rectum but also in other parts of the colon. Multiple biopsies from different parts of the colon as well as the rectum would thus seem to be desirable if mucosal sampling is to be employed as a screening test.

[Generalised herpes simplex virus infection presenting as an "acute abdomen" (author's transl)].

A 23-year-old patient developed acute upper abdominal symptoms during in-patient treatment for vulvo-vaginitis. Clinical picture, scintigraphy and angiography suggested a space-occupying process of the liver. Laparotomy showed ascites and a congested liver covered with cell necroses. The patient died the day after the operation from circulatory failure. Virological investigations of liver and spleen cell suspensions as well as the ascites fluid led to the demonstration of herpes virus type 2. The complement binding reaction gave a titre of 1:20 to herpes simplex virus. Disseminated herpes simplex virus infection is rare and in this case spread from the genital area.

Plasma cell alterations in ulcerative colitis. An electron microscopic study.

Colon biopsies from 37 young patients with active ulcerative colitis (UC) were investigated by electron microscopy with special regard to the inflammatory infiltrate of the lamina propria mucosae, which is characterized by lymphocytes, macrophages and plasma cells. There are close topographical relations between plasma cells and other inflammatory cells. In particular, different forms of plasma cells occur, which mainly differ in shape and contents of their rough endoplasmic reticulum (RER). The different degrees of development and dilatation of the RER express different functional states. The frequent occurrence of Russell bodies, Mott cells and necrobiotic plasma cells is explained as a--at least partial--pathologic alteration of plasma cells in UC. In context with recently published immunohistochemical findings these observations permit the assumption of an imbalance of the local Ig-system in UC.
