Calcium and magnesium in drinking water and the risk of death from breast cancer.

The relationship between mortality from breast cancer and the levels of calcium and magnesium in drinking water was examined using an ecological design. The study area consisted of 2.52 municipalities in Taiwan. Data on the levels of calcium and magnesium in drinking water were collected from the Taiwan Water Supply Corporation (TWSC). The age-standardized mortality rate (ASR) for breast cancer ( 1982-1991) was compared among municipalities with different levels of magnesium and calcium in drinking water. Weighted multivariate regression analysis was used, and after adjusting for fertility rates and urbanization, there was a significant inverse relationship between the levels of calcium and magnesium in drinking water and risk of death from breast cancer.

Sleep problems in Chinese elderly in Hong Kong.

BACKGROUND: To examine the sleep habits and one-year prevalence of sleep disturbance (difficulty in falling asleep, broken sleep and early morning wakening) as well as insomnia (subjectively inadequate or poor sleep) in an elderly Chinese population in Hong Kong. METHOD: In Phase 1, a representative sample of elderly aged 70 years or above were interviewed with a sleep questionnaire, and Cantonese versions of the Mini-Mental State Examination (CMMSE) and Geriatric Depression Scale(CGDS). In Phase 2, those with scores suggestive of cognitive impairment on CMMSE or depression on CGDS were interviewed by psychiatrists for making clinical diagnoses according to DSM IV. RESULTS: 1,034 elderly were interviewed in Phase 1. Occasional or persistent sleep disturbance were reported by 75% and insomnia in 38.2% of elderly. Slightly less than half of elderly with sleep disturbance complained of insomnia. Advancing age was associated with a higher rate of sleep disturbance while females had a higher rate of insomnia. Factors associated with sleep disturbance and insomnia included poor perceived health, past history of smoking, current depressive disorders, more chronic physical illness, more life events and more somatic complaints. Only 2.8% of the sample had taken sleeping pills within a one-year period. CONCLUSIONS: Sleep disturbance and insomnia are two separate but overlapping constructs and should be differentiated. Sleep disturbance is very common in the elderly and may be due to physiological changes with ageing. In contrast, those with a concommitant complaint of insomnia have impaired physical and mental health and may merit more medical attention.

Apolipoprotein E promoter and alpha2-macroglobulin polymorphisms are not genetically associated with Chinese late onset Alzheimer's disease.

In this study, we investigated two newly reported polymorphisms in association with late onset Alzheimer's disease (AD) in Chinese. They were a -491 A/T polymorphism in the Apolipoprotein E (APOE) promoter region and a five base pair deletion at exon 18 of alpha2-Macroglobin (A2M). There were 196 AD and 180 normal controls (N), which were age- and sex-matched. APOE epsilon4 alleles were significantly increased in AD vs. N (chi2 = 33.3, P < 0.000001). However, neither the -491 A/T (chi2 = 1.13, P = 0.29) nor A2M (chi2 = 0.18, P = 0.67) polymorphism was associated with AD risk, suggesting that these polymorphisms do not represent risk factors for AD in the Chinese population.

Pancreatic cancer mortality and total hardness levels in Taiwan's drinking water.

The possible association between the risk of pancreatic cancer mortality and hardness levels in drinking water from municipal supplies was investigated in a matched case-control study in Taiwan. All eligible pancreatic cancer deaths (883 cases) of Taiwan residents from 1990 through 1994 were compared with deaths from other causes (883 controls), and the hardness levels of the drinking water used by these residents were determined. Data on water hardness throughout Taiwan was collected from Taiwan Water Supply Corporation (TWSC). The control group consisted of people who died from other causes and were pair matched to the cancer cases by sex, year of birth, and year of death. The results show that there is a 39 % excess risk of mortality from pancreatic cancer in relation to the use of soft water. Trend analyses showed an increasing odds ratio for pancreatic cancer with decreasing levels of hardness in drinking water. This is an important finding for the Taiwan water industry and human health.

Rectal cancer mortality and total hardness levels in Taiwan's drinking water.

The possible association between the risk of rectal cancer and hardness levels in drinking water from municipal supplies was investigated in a matched case-control study in Taiwan. All eligible rectal cancer deaths (986 cases) of Taiwan residents from 1990 through 1994 were compared with deaths from other causes (986 controls), and the hardness levels of the drinking water used by these residents were determined. Data on water hardness throughout Taiwan were collected from Taiwan Water Supply Corporation (TWSC). The control group consisted of people who died from other causes and the controls were pair matched to the cases by sex, year of birth, and year of death. The results show a significant negative relationship between drinking water hardness and rectal cancer mortality. Odds ratio and 95% confidence intervals were 1.24 (1.01-1. 55) and 1.38 (1.10-1.73), respectively, for exposure to moderately hard water and soft water compared with the use of hard water. Trend analyses showed an increasing odds ratio for rectal cancer with decreasing levels of hardness in drinking water. This is an important finding for the Taiwan water industry and human health.

Esophageal cancer mortality and total hardness levels in Taiwan's drinking water.

The possible association between the risk of esophageal cancer and hardness levels in drinking water from municipal supplies was investigated in a matched case-control study in Taiwan. All eligible esophageal cancer deaths (2084 cases) of Taiwan residents from 1987 through 1996 were compared with deaths from other causes (2084 controls), and the hardness levels of the drinking water used by these residents were determined. Data on water hardness throughout Taiwan have been collected from the Taiwan Water Supply Corporation (TWSC). The control group consisted of people who died from other causes and the controls were pair-matched to the cases by sex, year of birth, and year of death. The results show that there is a 42% excess risk of mortality from esophageal cancer in relation to the use of soft water (adjusted odds ratio and 95% confidence intervals was 1.42 (1.22-1.66). This is an important finding for the Taiwan water industry and human health.

Lorazepam for chronic catatonia: a randomized, double-blind, placebo-controlled cross-over study.

Acute catatonic syndromes occurring in the context of various medical and neuropsychiatric conditions, including schizophrenia, have been shown to respond well to benzodiazepines (BZD). However, there have been no studies specifically designed to address the BZD treatment response of persistent catatonic states. Eighteen patients with clinically stable chronic schizophrenia, who also displayed enduring catatonic features, underwent a 12-week long, random assignment, double-blind, placebo-controlled cross-over trial with lorazepam (6 mg/day). A comprehensive assessment, including the subjects' clinical and motor (catatonic as well as drug-induced movement disorders) condition, was performed at baseline and four weekly intervals thereafter. Pre-existing medication was kept constant throughout the study. Lorazepam had no effect on the subjects' catatonic signs and symptoms, suggesting that acute and chronic catatonic syndromes associated with schizophrenic illness might have a different neurobiological basis.

Aromin-A, an Annonaceous acetogenin from Annona cherimola.

Chromatographic fractionation of a MeOH extract from the stems of Annona cherimola led to the isolation of a new non-adjacent, bis-THF ring acetogenin (one THF ring being at C-4 and the other at C-16), aromin-A (1) along with the known cytotoxic acetogenin squamocin (2). The structures of these isolates were established by means of mass spectrometry and spectroscopic techniques.

Calcium and magnesium in drinking water and the risk of death from hypertension.

Many studies have demonstrated a negative association between blood pressure and calcium and magnesium levels. This report examines whether calcium and magnesium in drinking water are protective against hypertension. All eligible hypertension deaths (2336 cases) of Taiwan residents from 1990 through 1994 were compared with deaths from other causes (2336 controls), and the levels of calcium and magnesium in the drinking water of these residents were determined. Data on calcium and magnesium levels in drinking water throughout Taiwan have been obtained from the Taiwan Water Supply Corporation. The control group consisted of people who died from other causes and the controls were pair matched to the cases by sex, year of birth, and year of death. Magnesium levels in drinking water were inversely related to the risk of death from hypertension. The adjusted odds ratios (95% confidence interval) for the highest versus lowest quintiles of exposure was 0.63 (0.47 to 0.84). After adjustment for magnesium levels in drinking water, there was no difference between the groups with different levels of calcium. The results of the present study show that there is a significant protective effect of magnesium intake from drinking water on the risk of hypertension. This is an important finding for the Taiwan water industry and human health.

New annonaceous acetogenins from Rollinia mucosa.

Four new compounds, a mixture of 20,23-cis-2,4-trans-bullatalicinone (1) and 20,23-cis-2,4-cis-bullatalicinone (2), rollimusin (3), and rolliacocin (4), along with eight known acetogenins, were isolated from an ethyl acetate extract of the unripe fruits of Rollinia mucosa. The structures and stereochemistry of 1-4 were determined on the basis of spectral data and chemical evidence.

Gradual withdrawal of long-term anticholinergic antiparkinson medication in Chinese patients with chronic schizophrenia.

Previous antiparkinson drug withdrawal studies involving white subjects have yielded inconclusive findings, whereas there is a paucity of data concerning Asian patients. A double-blind, placebo-controlled, randomized trial using gradual withdrawal of antiparkinson medication was conducted to evaluate the need for maintenance antiparkinson therapy for clinically stable Chinese patients with chronic schizophrenia. Seventy-five schizophrenic subjects who had received a diagnosis according to DSM-IV who had been ill for at least 5 years and on antipsychotic and antiparkinson medication for a minimum of 2 years entered the study. After baseline assessment, 58 subjects were matched according to age, sex, age at onset, length of illness, dose and length of antipsychotic and antiparkinson medication, and the presence of various extrapyramidal side effects. Randomly assigned dose-reduction and control groups were formed consisting of 29 subjects each. Trihexyphenidyl (THP), the only oral antiparkinson drug used in the study, was reduced by 1 mg every 2 weeks, whereas other psychotropic medication remained unchanged. Monthly assessment was performed using the Brief Psychiatric Rating Scale, Hamilton Rating Scale for Depression, Abnormal Involuntary Movement Scale, Simpson-Angus Scale, Barnes Akathisia Rating Scale, and the Nursing Observation Scale for Inpatient Evaluation-30. Complete withdrawal of THP was possible in 25 (90%) of the 28 subjects who completed the study, whereas considerable dose reduction was achieved in the remaining 3 subjects. There were no significant differences between dose reduction and control groups on any of the rating scales at the completion of the study. Our results suggest that long-term prophylactic administration of antiparkinson medication is unnecessary in the treatment of the majority of Chinese patients with chronic schizophrenia because withdrawal was accomplished without adverse mental or motor effects.

Magnesium in drinking water and the risk of death from diabetes mellitus.

This report examines whether magnesium in drinking water is protective against the probability of dying from diabetes mellitus. All eligible deaths from diabetes (6781 cases) of Taiwan residents from 1990 through 1994 were compared with deaths from other causes (6781 controls), and the levels of magnesium in the drinking water of these residents was determined. Data on magnesium levels in drinking water throughout Taiwan were obtained from the Taiwan Water Supply Corporation (TWSC). Controls were pair matched to the cases by sex, year of birth, and year of death. The results of the present study show that there seems to be a significant protective effect of magnesium intake from drinking water on the risk of dying from diabetes mellitus. This is an important finding for the Taiwan water industry and human health.

All eligible rectal-cancer deaths (986 cases) of Taiwan residents from 1990 through 1994 were compared with a sample of deaths from other causes (986 controls), and the levels of calcium and magnesium in the drinking water of these residents were determined. Data on calcium and magnesium levels in drinking water throughout Taiwan were obtained from the Taiwan Water Supply Corporation (TWSC). The control group consisted of people who died from other causes, and the controls were pair-matched to the cases by gender, year of birth and year of death. Compared with those with calcium levels below 22.0 mg/liter, the adjusted odd ratios (95% confidence interval) were 0.72 (0.53-0.98) for the group with water calcium levels between 22.0 and 40.8 mg/liter and 0.63 (0.45-0.87) for the group with calcium levels of 40.9 mg/liter or more. The adjusted odd ratios were not statistically significant for the relationship between magnesium levels in drinking water and rectal cancer. The results of the present study show that there may be a significant protective effect of calcium intake from drinking water on the risk of rectal cancer.

Low density lipoprotein receptor related protein gene exon 3 polymorphism association with Alzheimer's disease in Chinese.

Since apolipoprotein E4 (apoE4) is the major genetic risk for late onset Alzheimer's disease (AD), proteins that interact with apoE might be involved in AD pathogenesis. Low density lipoprotein receptor related protein (LRP) is an apoE receptor in the brain. In exon 3 of the LRP gene a polymorphism was found to be underrepresented in AD compared to normal Caucasian subjects (N). We examined this polymorphism in Chinese AD and N subjects. The polymorphism frequency in N was roughly half that reported for Caucasians. Compared to N, the frequency was significantly decreased in pathologically diagnosed, but not in clinically diagnosed AD patients. Thus, the role of the LRP exon 3 polymorphism in AD has now been demonstrated in two ethnic groups, suggesting the importance of LRP in AD pathogenesis.

Chlorination of drinking water and cancer mortality in Taiwan.

Chlorination has been the major strategy for disinfection of drinking water in Taiwan. An ecologic epidemiological study design was used to examine whether chlorination of drinking water was associated with cancer risks. A "chlorinating municipality" (CHM) was defined as one in which more than 90% of the municipality population was served by the chlorinated water while an "nonchlorinating municipality" (NCHM) was one in which less than 5% of the municipality population was served by chlorinated water. Age-adjusted mortality rates for cancer during 1982-1991 among the 14 CHMs were compared to rates among the 14 matched NCHMs with similar urbanization level and sociodemographic characteristics. The results of this study suggest a positive association between consumption of chlorinating drinking water and cancer of the rectum, lung, bladder, and kidney. Although these findings must be interpreted with caution because of limitations in the ecological study design, their public health significance should not be disregarded because chlorination of water is so widely practiced in Taiwan.

Acetogenins from seeds of Annona reticulata.

Chromatography of an ethyl acetate extract of seeds of Annona reticulata led to the isolation of a new cytotoxic gamma-lactone acetogenin, cis-/trans-isomurisolenin, along with six known cytotoxic acetogenins, annoreticuin, annoreticuin-9-one, bullatacin, squamocin, cis-/trans-bullatacinone and cis-/trans-murisolinone. Structures of these compounds were established by means of mass and related spectral experiments. Some of the compounds isolated, showed potent cytotoxicities against Hep. 2,2,15, Hep. G2, KB and CCM2, four cancer cell-lines.
