[Aneurysm of the infrarenal abdominal aorta].

Preoperative and postoperative treatment as well as standardisation of surgical techniques over the past 20 years have helped to bring about considerable reduction of operative mortality in cases of asymptomatic aortic aneurysm. Yet, with all improvement, rupture of aneurysm has continued to be associated with high rates of mortality. At the Department of Surgery of Cologne University, between 1963 and 1985, operations were performed on 681 patients for abdominal aortic aneurysm. Asymptomatic aneurysm were surgically removed from 41.7 per cent of them, while 27.5 per cent underwent surgery in symptomatic stages. Aneurysm had ruptured in 210 patients. Operative mortality accounted for 5.3 per cent of all asymptomatic patients. High mortality rates among patients with ruptured aortic aneurysms were attributable to preoperative shock. Only 16.3 per cent of patients survived in this group. The mortality rate among patients without shock amounted to 39.5 per cent.

[Asymptomatic carcinoid in a Meckel's diverticulum as the incidental finding in the operation of an abdominal aortic aneurysm].

A case report is given of a patient who had surgery because of an aneurysm of the abdominal aorta. During laparotomy a tumor in Meckel's diverticulum was found, which was removed by tangential incision without opening the intestine as to avoid an infection of the prosthesis. The tumor had a diameter of 1.5 cm and histology showed transmural spread of the carcinoid tissue; under these conditions further metastatic spread is possible. After the patient had recovered successfully from aneurysm surgery he was operated again and partial resection of the small intestine including the regional lymphatic system was performed. Carcinoid tissue was found during histological examination in the submucosal layer of the intestine in the area where surgery had been done before; no pathological changes however were found in the mesenterium. Thus the patient can be considered to be cured.

[Profundaplasty in older persons].

The profunda femoris artery has become the most important collateral vessel for compensating femoro-popliteal arterial occlusion. The circulation of the ischaemic limb can be obtained by profundaplasty. From 1972 to 1982, 70 patients with a mean age of 75,7 years underwent surgery. Good results were obtained in 75,9%.

Total parenteral nutrition as the sole therapy in Crohn's disease--a prospective study.

The effect of total parenteral nutrition (TPN) as sole therapy was studied in 30 consecutive cases of complicated Crohn's disease. After insertion of a Broviac-type central venous catheter patients were nourished parenterally for 3 weeks in the hospital and then for an additional 9 weeks at home. During this time no medication or oral intake was allowed. Surgery was avoided in 25 patients by TPN. These patients returned to work, ate normal meals and needed no medical support. In 5 cases it was not possible to control the acute disease and the patients were treated by resection. During TPN, catheter-related sepsis occurred in 3 patients (0.9 cases/1000 days TPN) and catheter embolism in 2. Four other patients developed intrahepatic cholestasis. A relapse of Crohn's disease was observed in 17 cases 3-48 months after the course of TPN. The cumulative recurrence rate is 60 per cent after 2 years and 85 per cent after 4 years. Compared with the results of resection, obtained from a 10-year period before TPN was instituted at our hospital, the cumulative recurrence rate after TPN is four times higher. It is concluded that TPN is not an alternative to resection in the treatment of Crohn's disease and should be reserved for patients with multifocal lesions, when surgery is not advisable because of the risk of a short bowel syndrome.

[Vascular replacement of small caliber arteries--a new bovine collagen prosthesis].

A new bovine collagenic prosthesis (Solcograft P) was tested in animal experiments. In 45 dogs the infrarenal abdominal aorta was substituted. During a follow-up period of up to 18 months the functional rate amounted to 88%. Joint-crossing iliaco-femoral bypasses were implanted in 31 dogs, 5 times in both sides. For the new bovine collagenic graft the functional rate ran up to 83%, during a follow-up period of 9 months. A wound infection was observed twice in the inguinal region thus exacting the removal of the prosthesis. An aneurysm was not observed. In our opinion the satisfying results are due to the compliance of the new prosthesis which is similar to natural artery.

[Surgery of diseases of the veins (author's transl)].

The article describes the present state of surgery of the v. cava inferior, of the veins of the pelvis and legs in acute phlebothrombosis, post-thrombosis syndrome, traumatic lesions, congenital malformations and in tumour surgery. Every surgical performance concerned with the venous system must be diagnostically clarified by phlebography. The veins of the thigh and pelvis are well visualized by means of ascending phlebography. Closely meshed functional testing by means of phlebography control, as well as venous pressure measurements and peripheral phlebodynamometry are required for timely recognition of an early relapse and for assessing the results of surgery.

[Surgical treatment of lung cancer: therapy or palliative?].

In spite of the revolutionary innovations concerning diagnostic and intensive care techniques as well as better understanding of tumor biology during the last decade, rates of resection and long-term survival in bronchogenic carcinoma could not be improved. All efforts are to be done in investigation on practicable, suitable, and supportable screening programs for early recognition of tumors in the first stages.

[Venous thrombectomy].

The aim of surgical treatment of acute deep venous thrombosis is to prevent lethal pulmonary emboli, the venous desobstruction and the maintenance of the valvular mechanism. From 1972 to 1982, 161 patients have been operated at the surgical clinic of Cologne. In 41.4% of the cases the long term result was excellent. The clinical examination, the complaints of the patients, the phlebography were judgement criteria.
