[Immunolymphoscintigraphy. A new procedure for noninvasive lymph node staging illustrated by the example of breast cancer].

In the course of a prospective study 43 women underwent axillary immunolymphoscintigraphy (ILS) shortly before a planned operation for suspected carcinoma of the breast. The aim was to test the feasibility of this procedure for the noninvasive staging of lymph-nodes. Three antibodies were used: HMFG-1 (3 patients), HMFG-2 (13) and 3C6F9 (27). The diagnosis was confirmed in 35 patients. They underwent radical mastectomy with removal of the lymph-nodes. There were marked differences in the usefulness of HMFG-2 and 3C6F9 for ILS in these circumstances. There were too few patients to evaluate HMFG-1. Sensitivity and specificity of the test were 0.57 and 0.83, respectively, for HMFG-1 and 0.83 and 0.93 for 3C6F9.

[Joint pain in pre- and postmenopausal arthropathia climacterica].

The menopausal syndrome consists of a variety of psychic and physical signs of different intensity. At our outpatient department for climacteric complaints and osteoporosis prevention (1. Dept. of Gynecol. and Obstetrics, Vienna) 820 women, who have been consulting this department for the first time because of prevailing complaints or prophylactic reasons, were registered in the year 1986. In the course of this work we paid particular attention to the symptom "pain in a joint" as a sign of the beginning climacteric period. Referring to our patient population we tried to analyse epidemiologically the frequency and the degree of severity of menopausal arthritic pain as well as the therapeutical response to a hormone substitution therapy. To what extent an endocrinologic explanation can be found and which facts let us assume such a statement, we tried to demonstrate in the discussion.

[Comparison of urodynamic findings before and after loop operations for recurrent incontinence].

30 patients with a recurrent stress incontinence were undergoing a dura-sling operation. The mean period of incontinence complaints up to the second surgery was 2.6 years. In the follow-up investigation the urodynamic parameters showed a significant improvement of the UCP, the urethral functional length and the DepQ. One year after surgery 86% of the women showed both a clinical and a urodynamic continence. A short term of urination disorders has to be considered in a part of the cases.

[Phospholipase A2 levels during labor].

Determinations of phospholipase A2 activity 2 hours after spontaneous start of uterine contractions in 14 women showed higher values than immediately post partum. The reason of this increase of enzyme activity inducing prostaglandin synthesis is not known yet.

The influence of nicotine abuse and diabetes mellitus on the results of primary irradiation in the treatment of carcinoma of the cervix.

References in the literature to the frequent occurrence of cervix carcinoma accompanied by nicotine abuse led us to investigate the effects of cigarette smoking on the results of treatment in primary irradiation of cervix carcinoma. As not only nicotine abuse but also diabetes mellitus can lead to angiopathy, we also investigated the influence of diabetes mellitus on the results of treatment. Of 410 nonsmokers with carcinoma of the cervix in Stages I and II, 260 (63.4%) reached the 5-year limit, but only 62 of 115 smokers survived (53.9%). In Stages III and IV there were significantly less favorable rates of cure in patients with nicotine abuse. Of 626 nonsmokers with cervix carcinoma in Stages III and IV, 212 survived (33.9%); but of 153 smokers, only 31 (20.1%) could be cured (P less than 0.01). The frequency of side effects of primary irradiation was distinctly higher in smokers than in nonsmokers. Reversible complications occurred in 17.5% of the smokers and 15.5% of the nonsmokers. Severe irreversible changes occurred in 28% of the smokers versus 15.2% of the comparative group of nonsmokers (P less than 0.01). The injuries caused by smoking not only reduce the biologic effectiveness of ionizing radiation but also increase the rate of side effects due to the deficient capacity for regeneration of the tissue surrounding the tumor. With diabetes as a complication, however, no significant changes in frequency of side effects were noted. Five-year survival in diabetic patients was affected in Stage I and II, but not in the advanced stages.

[Effect of a parenteral ozone-oxygen mixture on the concentration of immunoglobulins (IgA, IgG, IgM), of vitamin A and lysozyme activity in patients with cervical cancer].

In the literature you can find several therapeutic possibilities in the treatment of carcinoma by additive ozone therapy. We investigated the consequences of ozone therapy for the immunological status, lysozyme and vitamin A. 21 women with progressive cervical cancer (Stage III, IV) got besides the conventional irradiation therapy also an additive ozone therapy. After irradiation and ozone therapy a small decrease in IgG, IgA and IgM can be seen. A statistical significance could not be evaluated. There was no difference to the control group in lysozyme and vitamin A.

[Configuration frequency analyses of psychiatric symptoms in climacteric females].

Configurational frequency analysis according to Krauth and Lienert is a multivariant no-parametric statistical test method to identify types and syndromes. It was used in an attempt to disclose combinations of symptoms by which the various forms of climax of women could be typed. The point is made that outcome of this study should be evaluated in terms of heuristics, with further studies being necessary.

Radioimmunoscintigraphy using monoclonal antibodies before second-look surgery in patients suffering from ovarian cancer.

19 patients with a known history of ovarian cancer were investigated by radioimmunoscintigraphy (RIS) to look for recurrent disease a few days before second-look surgery. The tumor-associated monoclonal antibody HMFG-2 (400 micrograms/patient) was injected intravenously after labeling with radioactive 123I (0.5-2.2 mCi/patient). Scans were reviewed for activity accumulations due to uptake of the tumor-associated antibody by tumor sites. In 15 out of the 19 cases the scan results correlated with the intraoperative findings. There were 2 false-positive and 2 false-negative scans, the latter in patients with subclinical disease. The smallest lesion detected by radioimmunoscintigraphy had a diameter of 1.5 cm. In 3 patients, tumor sites were identified that had been missed by all other routinely performed methods of investigation including transmission computed tomography. These data indicate that RIS is of considerable clinical value in the early detection and localization of recurrent ovarian cancer and may, therefore, improve the management of these patients.

[Effect of nicotine abuse on treatment results in primary irradiated cervical cancer].

In view of the fact that frequent occurrence of cervical carcinoma in smokers is referred in literature, the authors examined the effects of cigarette smoking on primary irradiation therapy results in cervical carcinoma. Whereas of 410 patients with cervical carcinoma of stage I and II, 260 (63.4%) attained the 5-year limit, out of 115 smokers only 62 survived (53.9%). In the advanced cases of stage III and IV, on the other hand, the rates of cure achieved in patients who were habitual smokers were significantly poorer. Of 626 non-smokers with cervical carcinoma in stages III and IV, 212 survived (33.9%), whereas of 153 smokers only 31 (20.3%) were cured (p less than 0.01). The incidence of side effects of primary irradiation was also distinctly higher in smokers than in non-smokers. Reversible complications occurred in 17.5% of the smokers and in 15.5% of the non-smokers. Severe irreversible changes occurred in 28% of smokers but in only 15.2% of the comparative group of non-smokers (p less than 0.01). The noxious effects of smoking not only impaired the biological effectiveness of ionising radiation but also increased the incidence of side effects owing to deterioration of the regenerative capacity of the tissue surrounding the tumour.
