An allergic agranulocytosis developed after promethazine administration to a 34-year-old man with a compulsion neurosis. Promethazine-specific leucocyte antibodies were demonstrated in the antiglobulin consumption test. Four similar cases, reported in the literature, are discussed in relation to this personal case. The findings are contrasted to the majority of cases of toxic "phenothiazine agranulocytosis".

As a casuistic contribution to differential diagnosis of thrombosis of cerebral veins and sinuses a case of occlusion of sinus sagittalis superior by infiltration of an intracerebral metastasis from a teratoma of testis is reported. The leading symptoms - epileptic seizures, progressive hemiplegia, choked papilla and clouding of consciousness - gave rise to the tentative diagnosis of a cerebral tumor; in carotis angiography however, the metastasis was not detected, merely the occlusion of the sinus was demonstrated. Therefore the progress of the syndrome was misinterpreted as caused by an advancement of a venous thrombosis. Only autopsy showed the symptomless primary tumor and its formation of metastases.

BACKGROUND: It is well known that the acceptance of the fetoplacental unit in human pregnancy requires maternal immune tolerance, which is thought to be regulated locally by the placenta. Therefore an anti-inflammatory cytokine such as IL-10 plays a critical role in different pregnancy disorders including preeclampsia. In the present study, we examined the expression of both proinflammatory (TNF-alpha, IL-1beta, IL-2) and immunoregulatory (IL-6, IL-10) cytokines from normal term and preeclamptic patients in human trophoblast cultures. METHODS: Eleven patients with preeclampsia and 11 patients with a normal pregnancy at term were included in the study. Trophoblast cells isolated from placentas were cultured up to 48 h under standard tissue culture conditions and cytokine release was determined by ELISA. IL-10 synthesis was significantly decreased in the third trimester in preeclamptic patients in comparison with the control group. RESULTS: There were no significant differences in IL-1beta, IL-2, IL-6 or TNF-alpha expression but a significant alteration in IL-10 release in trophoblast cultures in vitro in term placentas from preeclamptic patients compared with normal pregnancy. CONCLUSIONS: Because IL-10 is a potent regulator of anti-inflammatory immune response these abnormalities may be associated with the inadequate placental development in preeclampsia.
OBJECTIVE: To determine the immunologic characteristics of tumor infiltrating lymphocytes (TILs) in comparison with the corresponding peripheral blood lymphocytes (PBLs) of patients with ovarian cancer in order to detect specific antitumor-reactive-immunocompetent cells. METHODS: Tumor infiltrating lymphocytes and PBLs were phenotyped by their surface markers, cytokine pattern, proliferation rate, and cytotoxic ability. RESULTS: The phenotypes of both lymphocyte populations were very heterogeneous. Peripheral blood lymphocytes had a higher proliferative activity and cytotoxicity against natural killer-sensitive tumor cells than the corresponding TILs. Furthermore TILs showed increased interleukin-4 expression whereas in PBLs, interferon-gamma production predominated. CONCLUSION: Peripheral blood lymphocytes showed more potentially valuable behavioral characteristics than TILs. In the future we need to combine several methods in order to define immune cells with antitumor activity. These cells should be expanded ex vivo and stimulated specifically for use in the immunotherapy of ovarian cancer.
In a clinical study we found that patients with primary node negative breast cancer whose tumors showed high expression of H-ras had a significantly better prognosis than patients whose carcinomas did not. In the present study, these tissue specimens showed no H-ras gene mutation. The rate of gene amplification was 5% and did not correlate with the level of H-ras expression. These data indicate that increased expression and not amplification of the H-ras gene is valuable, if H-ras has a favorable role with respect to clinical outcome. This might be a general protective reaction as long as no mutation event has occurred in the H-ras gene.

Methods of processing ocular fluid specimens, mainly from vitreous surgery, are described. Cytopreparatory techniques using Millipore filtration and Papanicolaou staining for cytology, celloidin bags for cell blocks, air-dried and wet smears and electron microscopy are discussed regarding the details of the methods and their role in the pathologic diagnosis of eye disease. Methods of evaluating tissue scrapings from the cornea and conjunctiva are also reviewed.
