[The study of transcatheter arterial chemo-lipiodol administration in liver metastasis of colorectal cancer].

Transcatheter hepatic arterial chemo-lipiodolization (TAC), using totally implantable reservoir, was performed for the treatment of liver metastasis from colorectal cancers in three cases of H1 (metastasis in one lobe only, n = 10), in 3 cases of H2 (a few scattered metastases in both lobes, n = 7), and in 2 cases of H3 (numerous metastases in both lobes, n = 9). We performed TAC in H1 cases after resection of liver tumor. We found a recurrence on lipiodol CT 2 months later in 1 case of H1. The metastatic tumor responded to TAC in two patients of H2 (1 case completely), but no response was observed in H3 cases. The present study suggested the effectiveness of TAC in cases H1 and H2, but further study is needed for cases of H3.

Effect of hepatic blood oxygenation on bile secretion in rats.

The effect of hepatic hemodynamics and hepatic tissue blood oxygenation on bile flow was studied in anesthetized rats by reflectance spectrophotometry. The hepatic hemodynamics and blood oxygenation were assessed by reflectance spectrophotometry. The hepatic ischemia was induced by partial ligation of portal vein and hepatic hypoxia was induced by inhalation of low concentration of oxygen. 1. The ischemia decreased hepatic blood volume index and hepatic blood oxygenation, and diminished bile flow. 2. Respiratory hypoxia suppressed hepatic oxygenation with minimal change of hepatic blood volume, and it also reduced the bile flow. 3. Bile flow was related hyperbolically with hepatic oxygenation and its dependency in hepatic ischemia and respiratory hepatic blood hypoxia identical. It is concluded that the hepatic tissue blood oxygenation affects hepatic energy metabolism, thus affecting the bile secretion.

In vivo estimation of oxygen saturation of hemoglobin in hepatic lobules in rats.

We developed a system for the in vivo estimation of oxygen saturation of hemoglobin in the sinusoidal blood of periportal and pericentral regions of hepatic lobules in rats. There was a marked heterogeneity in blood oxygenation and oxygen gradients within hepatic lobules. The sinusoidal blood oxygenation was reduced in some pericentral regions within hepatic lobules following acute ethanol ingestion, suggesting an occurrence of perivenular hypoxia in the liver after acute ethanol consumption.

Development and regression of intimal thickening of arterially transplanted autologous vein grafts in dogs.

To investigate the influence of hemodynamic conditions on intimal thickening of arterially transplanted autologous vein grafts, two experimental models of canine femoral arteries were prepared. In group I, in which an autologous vein graft was transplanted under abnormal flow conditions (distal poor runoff), intimal thickening gradually developed and reached 358 +/- 33 microns at 8 months, whereas no thickening was observed under normal flow conditions at any time throughout the observation period. In group II the thickened intima, which developed under abnormal flow conditions for 1 month, was reimplanted into the contralateral leg under normal flow conditions. The thickness of the intima markedly regressed to about 66% at 1 month, 50% at 3 months, and 25% at 8 months, respectively, whereas no regression of the thickened intima was observed under continued abnormal flow conditions. Electron microscopic studies revealed that the thickened intima in group I was composed of proliferation of transformed smooth muscle cells with a marked increase in the mitochondria, the rough endoplasmic reticulum, and an abundant fibrous matrix, whereas with the regressed thickness of the intima of group II, the smooth muscle cells were spindle-shaped with distinct myofibrillae. These results provide pertinent data on the process involved in the intimal thickening in cases of graft placement.

Heterogeneous response in hepatic sinusoidal blood flow to acute ethanol ingestion in rats.

The effect of ethanol ingestion on the hepatic sinusoidal blood flow velocity was investigated in rats, using in vivo microscopy system with dual-slit photometric method. In steady state, before ethanol ingestion, the hepatic sinusoidal blood flow velocity determined at 18 points ranged widely from 54 to 245 micron/sec and the mean value was 144 +/- 13 micron/sec. After acute ethanol ingestion (1g/kg), the averaged sinusoidal blood flow velocity increased significantly to 197 +/- 17 micron/sec, while the width of sinusoids showed no apparent change. In some sinusoids, however, the blood flow velocity did not increase. Thus, the heterogeneous response in hepatic sinusoidal blood flow to the ethanol metabolism might induce the development of an imbalance between delivery and demand of oxygen to hepatocytes around those sinusoids after acute ethanol ingestion.

Hepatic microcirculation in Zucker fatty rats.

We have developed a microscopic analyzing system for studying in vivo hepatic microcirculation, and measured the sinusoidal erythrocyte flow velocity simultaneously in the various sinusoids. With this system and organ reflectance spectrophotometry, the pathophysiological role of microcirculation and of energy metabolism in Zucker fatty rats were studied. The results were as follows: The erythrocyte flow velocity in the predominant sinusoids in the fatty rats was similar to that of the control rats, but the intersinusoidal erythrocyte flow was undetected in the fatty liver. Index of regional hepatic blood volume, regional hepatic blood flow and oxygen saturation of Hb decreased significantly in the fatty rats. The estimated in vivo oxygen consumption was not changed in the fatty liver. From these data, it is concluded that in Zucker fatty rats a marked fatty infiltration causes a decreased hepatic tissue blood flow and volume, but relatively homogeneous erythrocyte flow with an increased extraction of oxygen compensated the decreased vascular beds and maintained normal energy metabolism.

Modified vein allograft for small arterial reconstruction in dogs.

The ideal graft for peripheral arterial reconstruction has yet to be developed. We evaluated on the effectiveness of porosity to intimization of the graft. Modified vein allografts newly constructed through digestion of vein allografts with elastase, glutaraldehyde tanning and lyophilization were implanted in thirty-nine canine femoral arteries. These grafts had a higher porosity than conventional vein allografts and the overall patency rate was 70 per cent at 6 months after implantation. Microscopic examination showed that graft interstitial healing and subsequent intimization were completed by ingrowth of connective tissue from outside through the graft wall at 3 weeks and later. Aneurysmal change of the modified vein allografts was never evident. These results indicated that porosity of the graft is indeed an important factor for intimization and durability of grafts, even in bioderivative substitutes.

Flow waveform assessment of polytetrafluoroethylene grafts for reconstruction of lower extremity arteries. A preliminary report.

On the basis of our findings that the outcome of reconstructive surgery for lower limbs could well be predicted by flow waveform analysis, we reviewed femoropopliteal arterial bypass operations involving the use of polytetrafluoroethylene (PTFE) grafts for 35 patients. Of 38 grafts, 26 were PTFE alone, and 12 were PTFE/vein composite grafts. Neither ankle pressure index nor angiographic distal runoff was of predictive value as a prognostic indicator. The cumulative patency rate, calculated by the life-table method, revealed the usefulness of flow waveform analysis for prediction of the outcome of PTFE grafts. In type 0 or I flow, the patency rate was 94% at 1 and 2 years and 79% at 3 years. In contrast, in type II flow, the patency rate was 74% at 1 year, 66% at 2 years, and 49% at 3 years, with a statistical significance at 2 and 3 years (p less than 0.05). Comparison of the result of PTFE grafts with that of PTFE/vein composite grafts showed superior results of composite grafts at 3 years after implantation (42% vs. 83%, p less than 0.05). These findings indicate that flow waveform analysis is useful for prediction of the outcome of PTFE grafts and that PTFE/vein composite grafts should probably be used, particularly in cases of abnormal blood flow.
