Specific and total serum IgE measurements in the diagnosis of penicillin allergy. A long term follow-up study.

Sera from 204 patients with acute or former reactions following penicillin treatment were investigated by RAST with specificity for benzylpenicilloyl and phenoxymethyl-penicilloyl. The IgE antibody levels were followed up for of more than 2 years. Positive results could be observed at the time of the acute reactions and the weeks following; later months or years in many cases. Occasionally negative RAST results occurred within 30 days; on the other hand high levels of specific IgE persisted for years in some patients. Total serum IgE measured by RIST and the RAST results showed similar patterns, with mean levels of 568 u/ml between 0 and 30 days after the penicillin allergic reaction, 286 u/ml between 30 days and 1 year, and 195 u/ml in cases after 1 year corresponded well with the observed decline in specific IgE. Skin tests performed with penicilloyl-polylysine in 118 patients showed an overall correlation of 91-5% with the RAST results. Differences between the two test systems were observed in patients with a long interval between the test and the last allergic attack. None of the patients with proved penicillin allergy had atopic diseases or a history of atopy. On the basis of these findings it is suggested that the determination of both circulating specific IgE and total IgE should be of great value for diagnostic purposes during acute drug reactions and in the months immediately afterwards.

Cell proliferation in the Lichen planus infiltrate. Investigations by light- and electron-microscopic autoradiography.

In seven patients suffering from lichen planus (LP) light- and electron-microscopic 3H-thymidine (3HT) labeling studies were performed and the following results were obtained. 1. Using the light-microscope, a large number of 3HT labeled cells can be observed in the dermal infiltrate of LP. Most of the labeled nuclei belong to mononuclear, 'round cells' while isolated endothelial cells, fibroblasts, histiocytes and mast cells could be found labeled. 2. The mean labeling index of 'round cells' in our patients was 4.27 +/- 1.57%. 3. The evaluation of the electron microscopic autoradiographs showed that, as far as 'round' cells were concerned, exclusively nuclei of cells showing the morphological criteria of stimulated lymphocytes were labeled. These findings support the view that in LP, as in allergic contact dermatitis, a high proliferative activity of lymphocytes takes place.

The epidermal cell proliferation in lichen planus.

The proliferative activity of epidermal structures was determined in 6 patients suffering from lichen planus (i.p.). By using the in vitro 3H-thymidine labelling technique, semithin Epon sections of lesional and perilesional uninvolved skin were compared autoradiographically with normal controls. The following results were obtained. The labelling index of epidermal cells was significantly increased within the i.p. papules. The mean values ranged from 41% in acanthotic i.p. to 23% in atrophic i.p. whereas in adjacent unaffected skin (15% and 11% respectively) as well as in normal controls (6%) a lower 3H-index was observed. As regards damage to basal cells within i.p. lesions, a factor--the relation between the number of basal cells in involved and uninvolved skin in reference to the epidermal length--was calculated in order to correct the autoradiographic results in involved skin. The differences between i.p. papules, adjacent skin and controls remained statistically significant even after correction. The intradermal eccrine ducts revealed a significantly increased labelling index in i.p. lesions as compared with perilesional and normal skin. In this respect no difference could be observed between the acanthotic and the atrophic variant of i.p. These findings indicate that replacement of damaged basal cells in i.p. is achieved by an increase in actively dividing keratinocytes in both epidermis and skin appendages within the lesion.

[Septic complications of gonorrhea].

Septic gonococcal complications consist in intermittent fever, arthralgia and skin lesions. In recent years predominantly females suffering from this disease were observed. This diagnosis is made by the demonstration of gonococcal infection combined with the above mentioned clinical symptoms. A further confirmation is possible by blood culture and the demonstration of gonococci in skin lesions or joint fluid.

[Immunological investigations in lichen planus patients (author's transl)].

30 patients suffering from generalized lichen planus (= L.p.) have been examined by the following methods in order to more precise information regarding their immune-status: 1. Quantitative determination of immuno-globulins including complement fraction (beta 1C/A) 2. Intracutaneous tests with recall-antigens 3. Investigation of cutaneous sensitivity to DNCB 4. Lymphocytemigrationinhibition- and Lymphocytetransformationstests. The results obtained were not statistically significant when comparing the results of this investigation in patients suffering from L.p. with the control group. Therefore, the theory that immunological processes are the basis of these occurences is still under discussion. Further studies applying more specific methods should be conducted in order to clarify this problem.

[Cytoid bodies in basal cell epithelioma].

100 basal cell epitheliomas were surveyed for the presence of cytoid bodies by histological and histochemical techniques. By this means a differentiation of the following structures was possible: 1. Elastic globes were present in 32 specimens. Their localisation corresponded in part to the typical subepidermal situation, but invasive tumor growth resulted in their translocation into the deeper corium. 2. Civatte bodies were found in the dermoepidermal junction area in 16 cases. Similar structures were present within the tumor cell islands of 37 patients. 3. Russel bodies could be observed within the inflammatory peritumoral infiltrate containing numerous plasma cells in 30 specimens. 4. Clumpy or ovoid deposits of amyloid were present within the stroma of 41 epitheliomas, whereas no amyloid was found in the tumor cell islands.

Epidermal changes in lichen planus.

Ultrastructural investigations in 15 patients with lichen planus (LP) revealed a multitude of epidermal changes. The most consistent findings were (1) multiplication, irregular folding or dislocation of the basal lamina, (2) fragmentation with degenerative changes of basal keratinocytes, (3) formation of numerous fibrillar Civatte bodies and, (4) presence of dyskeratotic elements. In addition, mitotic figures of keratinocytes and Langerhans cells were observed. These findings support the view that the primary event in LP represents an injury involving epidermal basal cells. Furthermore, the replacement of destroyed tissue seems to originate not only from the margin of the lesions or the skin appendages but also from unaffected cells within the LP papule.

[A comparative study of the value of skin tests and radio-allergo-sorbent tests in inhalation allergies (author's transl)].

Skin tests were performed and the specific IgE levels determined by means of the radio-allero-sorbent test (RAST) in fifty patients with allergic rhinitis/conjunctivitis and extrinsic bronchial asthma. An overall correlation between in vitro and in vivo diagnosis was found in 87% of cases. The results of this study indicate that in such allergies the RAST might well be of value as a screening test. In addition it is a valuable supplement to the skin tests, particularly in the elucidation of the diagnosis in problematic cases.

Light and electron microscopic differentiation of amyloid and colloid or hyaline bodies.

Hyaline or colloid bodies (Civatte bodies) and amyloid are sometimes confused because of their similar appearance under the light microscope. A differentiation can be made by histochemical means (DMAB-Nitrite method) and by fluorescence or polarization microscopy. At the ultrastructural level hyaline or colloid bodies as well as amyloid exhibit a fibrillar structure, but can be differentiated by the diameter and arrangement of fibrils and the localization of the aggregations. A clearcut differentiation between hyaline or colloid bodies and amyloid seems of great importance in any dicussion of the epidermal origin of amyloid and the classification of fibrillar aggregations associated with skin tumours.

[The local treatment of the keratoacanthoma with 5-fluorouracil].

In the last two years thirteen patients with keratoacanthomas were treated with 5% f-fluorouracil ointment at the Department of Dermatology II of the University of Vienna. Treatment lasted from two to eight weeks depending upon the size of the tumor. In all cases there was clearing of lesions with good cosmetic results. During an average follow-up period of 7.4 months for all patients there was no recurrence of the tumors.

[The angioleiomyoma of the skin].

Twelve cases of angioleiomyoma are reported. These are benign, often painful tumors occurring chiefly on the lower extremities of middle-aged women. Clinical diagnosis is usually possible, and any remaining doubt can easily be resolved by histological examinations.
