D.B. Kagenaar and the ophthalmometer by Javal-Schiøtz.

In the years 1881-1890 the Utrecht instrumentmaker D.B. Kagenaar, laboratory attendant of Donders and Snellen, played an important role in the introduction on the German market of the ophthalmometer developed by Javal and Schiøtz.

Herman Snellen (1834-1908) and Müller's 'reform-auge'. A short history of the artificial eye.

The shell prothesis, originally meant to be placed over the atrophic eye, caused serious problems when used after enucleation, which became more common practice in the second half of the last century. In the nineties of the last century Snellen Sr. tried to solve these problems by designing a light prothesis that filled up the empty space. At the insistence of Snellen, the Müller brothers in Wiesbaden succeeded in 1892 to blow a hollow artificial eye which proved to be a real success.

Antonie Cramer's explanation of accommodation.

In the middle of the last century the question of the origin of accommodation was still unsolved. At the suggestion of Donders, Cramer used in 1851 a microscope to demonstrate that accommodation has to be ascribed to an increase in the curvature of the lens. Donders himself had several instruments made for the same purpose. These are still present in the collection of the Royal Netherlands Ophthalmic Hospital.

The influence of ticlopidine on the natural course of retinal vein occlusion.

A double-blind study was established to evaluate the platelet aggregation inhibiting effect of ticlopidine on the course of ocular vein occlusions. 35 patients with recent central retinal vein occlusions and 54 patients with retinal branch vein occlusions were treated for 6 months. Various ophthalmological and clinical-chemical parameters, including fluorescence angiography, were assessed. Compared with placebo therapy a significant improvement in visual acuity was observed with ticlopidine for branch vein occlusions; the same trend was seen for the central retinal vein occlusions. Increased ADP-induced platelet aggregation was frequently found: in 72% of the central retinal vein occlusions and 70% of the branch vein occlusions. For the Wu and Hoak test, these numbers were respectively 74% and 67%. Hypertension was seen in 55% of all occlusions, hyperlipaemia in 33%, and diabetes in 29%. The effect of ticlopidine was most pronounced in patients with increased platelet aggregation and least obvious in hyperlipaemia. Hypertension and diabetes did not apparently influence Ticlopidine's effects. It is concluded that the platelet aggregation inhibitor ticlopidine is effective in the treatment of recent ocular vein occlusions.

Unsaturated fats and progression of diabetic retinopathy.

Two groups of adult-onset diabetics, matched according to 10 different criteria, were followed, biochemically and fluorescein-angiographically, over a period of 6 years. One group used a saturated-fat diet, and the other group of patients were on an unsaturated-fat diet, particularly rich in linoleic acid. Statistically significant differences were found in biochemical data and in the progression of diabetic microangiopathy between the two groups. The conclusion is warranted that a linoleic acid-rich diet, administered over an extended period of time, may inhibit the development of microangiopathy, or the deterioration of diabetic retinopathy.

[Optical and non-optical aids for the vision-impaired older patient].

Increase of contrast and enlargement of retinal image may lead to an improvement of reading ability of the elderly visually handicapped patient. Pros and cons of available low visual aids are discussed based on the examination or 266 patients of over 60 years of age. High-power addition was sufficient in 27% of the patients. A hand-magnifier of standmagnifier, as well as a telescopic reading unit, or so-called 'prism-loupe' sufficed in 36% of cases. The closed circuit television ('television loupe') was advocated in 17% of cases. The immobility and relative high costs of this apparatus, however, was in a sense prohibitive for further extension of use of this unique instrument, devised for the highly visually handicapped patient. In 20 per cent of cases it was impossible to improve reading ability altogether.
