[A case of hepatocellular carcinoma with a duodenal invasion from metastatic lymph node].

A 64-year-old male, who had been diagnosed as having hepatocellular carcinoma, was admitted to hospital and underwent transcatheter arterial infusion therapy. CT and US examinations revealed a large tumor with air in its center. Barium meal examination of the duodenum showed a large filling defect with a central excavation. At endoscopic examination, a dark reddish tumor was seen protruding into the duodenal lumen. A biopsy specimen from this tumor revealed a hepatocellular carcinoma. The patient subsequently died of hepatic coma following gastrointestinal bleeding. An autopsy revealed that metastatic lymph nodes had infiltrated into the adjacent duodenum. Nine previous cases of duodenal metastasis from a hepatocellular carcinoma have been reported in the Japanese literature.

Eaton-lambert syndrome associated with pituitary apoplexy--a case report.

A case of Eaton-Lambert syndrome associated with pituitary apoplexy is presented. It is thought to be the first reported case. The clinical features, electrophysiological findings and effects and side effects of therapeutic drugs are noted. Endocrinological dysfunctions are discussed in relation to pituitary disorder.

[A case of hepatocellular carcinoma (HCC) with a huge tumor thrombus in the right atrium].

A case of HCC with a huge tumor thrombus in the right atrium is reported. CT and celiac angiography revealed HCC in the liver. Inferior vena cavography and cardiac blood pool scintigraphy demonstrated a large defect in the right atrium, and 67Ga-citrate scintigraphy showed a lesion of abnormal accumulation in the right atrium. With these findings, diagnosis of HCC with a tumor thrombus in the right atrium was made, and the clinical diagnosis was confirmed by autopsy.

[Mechanical properties and phase transformation of super-elastic Ni-Ti alloy wires. Part 1: Relation between super-elasticity and phase transformation].

The relationship between mechanical properties and thermal behavior of super-elastic Ni-Ti alloy orthodontic wires was investigated in order to use them effectively in clinics. Three types of super-elastic Ni-Ti alloy orthodontic wires, SENTALLOY-BLUE(SB), YELLOW(SY) and RED(SR), were used. Two parameters were introduced from the load-deflection curves in three point bending test to evaluate their mechanical properties. One was the super-elastic index (SEI) for super-elasticity, and the other was the E-load for the load in the super-elastic region. Their thermal behavior in the course of the transformation analyzed with DSC, and compared with the mechanical properties. The specimens heat-treated at 500 degrees C for 30 min were also examined. The differences in mechanical properties of the super-elastic Ni-Ti alloy orthodontic wires used in this study were thought to be more affected by the height of the thermic peak and the form of the DSC curves than the transformation temperature. The higher the thermic peak, the higher tended to be the SEI and the lower E-load. To the contrary, the lower thermic peak, the higher was the E-load and the lower was the SEI, and the continuous force could not be achieved. The remaining work-hardened structure was thought to influence these differences.

[Mechanical properties and phase transformation of super-elastic Ni-Ti alloy wires. Part 2: Changes of properties through heat treatment].

Three types of super-elastic Ni-Ti alloy orthodontic wires, SENTALLOY-BLUE (SB), YELLOW (SY) and RED (SR), on the change of properties were studied especially the reversible change of the load level, using three point bending test and DSC. The heat treatment at 440 degrees C caused the largest reduction in mechanical properties of SB and SY, and the 400 degrees C treatment did that of SR. The load level of SR was less changeable than those of SB and SY by the difference of heat treatment temperature. Mechanical properties of SY were almost reversible by the alternate heat treatment at 440 degrees C and 510 degrees C. Then the 440 degrees C-heat treatment increased the transformation temperature, and the 510 degrees C-heat treatment decreased them. The variation of the mechanical properties completely corresponded to the change in transformation temperature. The variation appears to result from some internal structure changes. Changes in mechanical properties are observed by heat treatment at 360, 400 and 480 degrees C, but the reversibilities were not so clear. The change of the properties of super-elastic wires are influenced by the kind of wire and the heat treatment condition. The reversible change in the mechanical properties seems to have many advantages over super-elastic Ni-Ti alloy orthodontic wires.

Chemotherapeutic malaria control operation by single dose of Fansidar plus primaquine in North Sumatra, Indonesia.

Selective age group treatment and village scale chemotherapeutic malaria control operation were carried out in east-coast villages in North Sumatra, Indonesia in 1987/1988. A single dose of Fansidar plus primaquine was adopted as the drug regimen to cut the transmission of malaria at the gametocyte stage. After the treatment on day seven, the gametocyte positive rate was reduced to only 2.7% in 72 Plasmodium falciparum gametocyte carriers. A significant reduction of P. falciparum prevalence in the community was observed after successive selective age group treatment in primary school, however P. vivax prevalence persisted. Village scale active case detection was carried out by one health center staff and two village health volunteers. After eight months P. falciparum prevalence was reduced from 14% to 1%. As the result of the chemotherapeutic control activities covering high-prevalence villages in the coastal area, malaria prevalence in 1988 became very low, as compared with the status in 1985/1986.

[A case of successful repair of a left subclavian arterial aneurysm ruptured into the esophagus--review of 89 cases in Japan].

This report concerns a case of a left arteriosclerotic subclavian arterial aneurysm ruptured into esophagus. A 61-year-old man who complained of fever, dyspnea, and dysphagia was diagnosed by chest X-ray, selective left subclavian arteriogram, and chest CT. Because he vomited a large amount of blood before an operation, we suspected that the aneurysm had ruptured into the esophagus, and therefore performed an emergency operation. The operative procedure consisted of exposing the aneurysm through a median sternotomy incision with the addition of a left supraclavicular incision, proximal and distal ligation, and reconstruction of blood flow by an aorto-left subclavian artery bypass with a Gore-tex prosthetic graft; however the aneurysm was not resected because of adhesion to the surrounding tissues. The post-operative course was uneventful and the patient is now normal and back at work, 2 years after the operation. Subclavian arterial aneurysms are relatively rare among peripheral arterial aneurysms. The common causes of subclavian aneurysms are arteriosclerosis (18%), trauma (14.6%), non specific inflammation (13.5%), and thoracic outlet syndrome (11%). Of the only 15 ruptures in 89 cases that have been reported in Japan, none have ruptured into the esophagus. Subclavian artery aneurysms represent a risk of rupture or peripheral circulatory failure, and consequently, should be considered for surgical treatment.

[Changes in serum pancreatic secretory trypsin inhibitor (PSTI) during and after heart surgery].

Serum pancreatic secretory trypsin inhibitor (PSTI) is considered to be one of the acute phase reactants which increase after tissue injuries like surgery or major trauma. 25 patients were studied to investigate the changes in serum PSTI during and after heart surgery. 18 patients underwent the operation with the aid of cardiopulmonary bypass (CPB)-CPB group, and the other 7 patients, all pediatric cases, were treated under simple deep hypothermia by surface cooling without CPB-simple hypothermia group. Serum amylase, serum C reactive protein (CRP), cardiac index and A-aDO2 after surgery were examined, and the duration of CPB or circulatory arrest were recorded. The correlation between serum level of PSTI after surgery and these parameters were also studied. During surgery, a transient elevation of serum PSTI was noticed in the period of CPB, but no significant change of serum PSTI was seen under simple deep hypothermia and circulatory arrest. On the first post operative day, a slight but significant increase of serum PSTI was seen in CPB group, on the other hand no significant increase was seen in simple hypothermia group. Postoperative increase of serum PSTI was correlated to circulatory arrest time in simple hypothermia group, and was inversely correlated to cardiac index determined immediately after the operation in CPB group. No correlation was seen between serum level of PSTI after surgery and serum amylase, CRP, A-aDO2 or duration of CPB. The magnitude of increase of serum PSTI after heart surgery was small, and did not seem to be well correlated to the extent of surgical insults.

Establishment of mouse thymic nurse cell clones from a spontaneous BALB/c thymic tumor.

This is the first report on the establishment of the readily identifiable and functioning thymic nurse cell (TNC) clones from the mouse thymus. In the course of the culture of an epithelial cell line using a medium with a low concentration of Ca2+ from the spontaneous BALB/c thymic tumor, lymphocytes as well as thymic stromal cells which were not apparently typical epithelial cells by light microscopy seemed to grow relatively well in one flask. The culture medium was exchanged with regular Dulbecco's modified Eagle's medium in the third week, and from that flask TNC clones were established together with lymphoblast cell clones. The established cells (IT-79MTNC3) were easily identified as TNC. They formed complexes with simultaneously established lymphoblast cells and were remarkably similar to those of fresh TNC and thymocytes. Cloned TNC could express Ia antigens. In co-culture experiments, cloned TNC together with recombinant interleukin 2 (rIL 2) appeared to support the growth of fetal thymocytes which with rIL 2 alone failed to proliferate. The supernatant of IT-79MTNC3 was previously found to contain the growth factor for some T cell clones. It remains to be solved whether the TNC affects fetal thymocytes through direct contact or secretes an active growth-promoting factor. Experiments along this line are now in progress.

Thymic stroma-derived T-cell growth factor (TSTGF): II. Biochemical and functional characterization.

The culture supernatant (SN) from a cloned line of thymic stroma-derived cells in fibroblastic form (TSCF) contained a factor capable of supporting the growth of the interleukin (IL) 2-dependent, antigen-specific helper T cell (Th) clone 9-16 without requiring IL2 and antigen. This active substance, designated as thymic stroma-derived T-cell growth factor (TSTGF), was partially purified through DEAE-Sephacel chromatography and PBE 94 chromatofocusing. The original SN did not contain IL1, IL2, IL3, IL4, or interferon activities; but an appreciable magnitude of colony-stimulating factor (CSF) activity in addition to TSTGF was present, whereas the partially purified preparation of TSTGF was depleted of any type of CSF activity. The elution profile of TSTGF activity on the chromatofocusing has revealed that TSTGF has an isoelectric point (pI) of about 6.0. When a purified TSTGF sample was applied to Sephacryl S-200 column chromatography and sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis, TSTGF activity was eluted in a single peak around an apparent molecular weight of about 25,000. The activity of TSTGF also was shown to be relatively stable with heat treatment and in the wide range of pH, but it was abolished by treatment with either trypsin or dithiothreitol. These results indicate that TSTGF, a novel T-cell growth factor, is the protein that has an apparent molecular weight of about 25,000 and a pI of 6.0, and in the intact molecule, it contains the disulfide bond(s) required to maintain and/or express its biologic activity.

A rapid in vitro assay system using anti-bromodeoxyuridine for drug susceptibility of Plasmodium falciparum.

Drug effects were monitored by the measurement of bromodeoxyuridine (BrdU) incorporation into parasite deoxyribonucleic acid (DNA). Cells are pulse-labelled with BrdU and those which are synthesizing DNA incorporate BrdU into their DNA. Anti-BrdU is used to identify cells undergoing DNA synthesis at the time of the pulse. Concentration-effect curves were determined for chloroquine, pyrimethamine and quinine in culture for only 3 h in RPMI 1640 medium supplemented with BrdU. Different strains of Plasmodium falciparum could be distinguished when BrdU uptake was monitored by the ELISA. This assay offers a fast and accurate method of monitoring the effects of a variety of antimalarial drugs.

[Randomized comparative study of CE (CDDP plus etoposide) and CE-AVN (ACNU, VCR plus procarbazine) as combined anticancer chemotherapy in small cell cancer of the lung].

A randomized comparative study of anticancer chemotherapy CE (CDDP plus etoposide) and CE-AVN (ACNU, VCR plus procarbazine) was carried out on 27 patients with small cell lung cancer (SCLC) without previous chemotherapy. In CE therapy, 12 patients received injection of CDDP (80 mg/m2 on day 1) and etoposide (75 mg/m2 on day 1-5) every 4 weeks (Protocol 1). Fifteen patients received 2 courses of CE and 1 course of AVN therapy (ACNU 100 mg/m2 on day 1, vincristine 0.7 mg/m2 once a week and procarbazine 50 mg/day, daily) for 6 to 8 weeks (Protocol 2). One patient (8%) and 2 patients (20%) achieved complete response with Protocol 1 and 2, respectively. Seven patients (58%) and 9 patients (60%) achieved partial response with Protocol 1 and 2, respectively. The median survival time (MST) was 12 and 14 months, and duration of remission was 4.5 and 7 months in patients treated with Protocol 1 and 2, respectively. No significant difference in MST and duration of remission in each group was observed. However, 2 patients (13%) treated with Protocol 2 survived more than 3 years. Both protocols were well tolerated with only moderate gastrointestinal symptoms, mild bone marrow toxicity and alopecia.
