In the course of a randomised prospective study involving 98 patients who had undergone cardiac surgery, 51 patients were given 20 g human immunoglobulin i.v. over four months, 47 received merely blood transfusions, like all other patients, averaging 4.4 units per patient. Antibodies against HBs antigen or Hbc antigen were present in 20% of patients before operation. After operation (plus blood transfusions) antibodies against hepatitis-B virus were demonstrated in 43% of the control group but 94% of those treated with immunoglobulins. These antibodies persisted in the course of further immunoglobulin administrations, while they fell in the control group. Hepatitis-B infections occurred in ten patients of the control group, after 30-90 days; three of them developed acute hepatitis. In the group treated with immunoglobulins there were only two patients with an infection, one of whom developed hepatitis. Antibodies against cytomegalic virus increased in the control group but not in the treatment group. In both groups there was the same incidence of non-A-non-B hepatitis.

Congenital malformations of the heart have to be expected in approximately 0,8% of all newborns. The majority of them needs surgical treatment and can be corrected anatomically with a low operative risk (atrial and ventricular septal defects, patent ductus arteriosus, coarctation, valvular aortic and pulmonary stenosis etc.). Surgical repair of complex anomalies includes either palliative or functionally corrective procedures and has a much higher operative and early postoperative mortality (transposition of the great arteries, tetralogy of Fallot, tricuspid atresia etc.). Surgery becomes urgent in babies suffering from congestive heart failure and/or progressive severe hypoxia during the first months of life. Early total surgical repair or primary palliation and two-stage correction depends on the kind of anomaly and has to be selected individually. Hemodynamical classification, general distribution and surgical considerations of the most common congenital malformations of the heart are described.

In 22 mongrel dogs the protective effect on the myocardium of Bretschneider- and Kirsch-Cardioplegia (90 minutes of cardiac arrest) was studied and compared with ischemic arrest (45 minutes at hypothermia of 30 degrees C). Myocardial contractility was evaluated by the following indices of contractility: t--dp/dt max., Krayenbühl-Index of contractility and max. Vce. Depression of contractility of 40 % was observed after ischemic arrest and Kirsch-Cardioplegia, whereas in Bretschneider-Cardioplegia depression was only 7 %. Forty-five minutes after reperfusion contractility had returned to normal in the last group. It is concluded that Bretschneider-Cardioplegia results in little reduction of postischemic contractility, and has a good protective effect on the myocardium.
A mobile left-ventricular tumor was detected by echocardiography in a 28-years-old patient, who has sustained an anteroseptal infarction four years before. The patient was admitted to hospital because of arterial embolism. The echocardiographic finding was validated by left-ventricular angiography. In open-heart surgery a pedunculated tumor was excided, in microscopic study the tumor revealed the findings of an organized thrombus. The echocardiographic and angiographic findings are shown, the echocardiographic differential diagnosis is pointed out.
Chest X-rays were analysed in 22 male and 16 female patients (mean age 30.7 years) with coarctation of the aorta. The results were correlated with angiography, haemodynamic pressures and operative findings. The left subclavian artery was prominent in 33 cases, signs indicating a collateral circulation (rib notching, internal mammary artery) were present in 26 cases. In addition there were changes of cardiac and aortic configuration. The least reliable sign was constriction of the aorta due to the stenosis itself. Frequent combinations of X-ray signs were dilatation of the left subclavian artery and a collateral circulation, in addition to signs of increased pressure (n = 25). Less frequently (n = 8) only a dilated subclavian artery and signs of increased pressure were found. The prominence of the subclavian artery was particularly marked in systolic pressure gradients through the stenosis of greater than 40 mm Hg, and the formation of a collateral circulation when average aortic pressure differences above and below the stenosis were less than 40 mm Hg. Collaterals were to be found most frequently with a correlation of 2:1 of the systolic gradient to the mean pressure gradient delta Ps/delta Pm. A very good correlation existed comparing X-ray signs with findings at operation. The diagnosis of coarctation of the aorta can thus be made with sufficient certainty from the plain chest X-ray.
Implantation of an aortic valve prosthesis in patient with aberrant left circumflex coronary artery arising from the initial portion of the right coronary artery resulted in compression of this vessel by the prosthetic valve and in pump failure after interruption of cardiopulmonary bypass. Incidence and significance of variations in coronary artery origin and course are mentioned.
In 3 patients with a congenital aneurysm of the membranous portion of the ventricular septum diagnosis could be made by angiography. One of these patients was clinically asymptomatic, while the two other patients were transferred for surgery because of right ventricular outflow tract obstruction or intracardiac left-to-right shunt. These both patients underwent successful open heart corrections. Aneurysms with a different intracardiac localization could be found. One of the cases represents an unusual association with corrected transposition of the great vessels.
