[Splenorenal anastomoses in portal hypertension (author's transl)].

The classical splenorenal shunt of Linton modified by Clatworthy is definitely superior to portocaval anastomosis. Between 1968 and 1976 in 225 patients with bleeding oesophageal varices a splenorenal anastomosis was performed in 47 cases, a portocaval in 30 cases and a mesentericocaval in 2 cases. There were 55 palliative operations as compared with 91 conservative therapeutic approaches. Over the last five years central termino-lateral splenorenal anastomoses have been performed almost exclusively.

Esophageal and Gastric Varices↗

[Pancreatic pseudocysts--clinical picture and surgical treatment].

Report about 98 cases of pancreatic pseudocysts and also elaboration on the etiology, pathogenesis and details of this disease. The operative method of choice is to establish the internal anastomosis. As long as the anatomic conditions and especially the structure of the cyst permit it, such anastomosis should be performed. We applied this method in 57.2 p.c. of the cases altogether. Y-anastomosis (Roux) proved to be the best. We applied it in 35.9 p.c. of our patients. For patients in bad general condition, outside drainage operation in the sense of marsupialisation is often the only possible and at the same time life-saving intervention. We performed this operation in 42.8 p.c. of our cases. Because of the clear increase in number of pancreas diseases in the last years, there is also a higher frequency of pancreatic pseudocysts. Their diagnosis and therapy are therefore important.

[Percutaneous, transhepatic thrombosis of the coronary vein in the treatment of bleeding oesophageal varices (author's transl)].

Percutaneous, transhepatic catheterisation of the portal vein in order to occlude the gastric coronary vein was carried out in six patients with bleeding oesophageal varices. Bleeding stopped in three patients, but in the other three ceased only temporarily. Complications were one pleural effusion and one haemothorax. At autopsy, marginal portal vein thrombosis was found in two cases. The indication for this procedure is the presence of bleeding from oesophageal varices which cannot be controlled by conservative means in a patient unfit for operation. It provides an opportunity for carrying out a decompression shunt operation in the interva between the bleeds.

Embolization, Therapeutic↗

[Liver blood flow studies using cineangiodensitometry].

In 12 anesthetized beagle dogs breathing spontaneously flow in the hepatic artery (168 ml/min + 10 SEM) and portal vein (576 ml/min +/- 20 SEM) was measured by means of angiodensitometry. Total hepatic blood flow (THBF) was 185 ml/100 g/min +/- 11 SEM. In our opinion this relatively high THBF depends upon the method used.

[The overlooked stone of the biliary tract].

A report is given of current diagnosis and treatment of overlooked stones of the common bile duct, reviewing 81 patients seen in our hospital. Problems arise mainly in icteric and postoperatively critical patients. There is still not enough clinical experience in medical or endoscopic treatment in the disease. Therefore surgical procedures are still the treatment of choice although there is a higher mortality in relaparotomy than in first procedure.

Cholelithiasis↗

[Iatrogenic injuries of the human bile duct].

The problems of diagnosis and treatment of iatrogenic injuries to the extrahepatic biliary tract are discussed, reviewing 31 own cases. End-to-end anastomoses should only be performed in immediatly detected lesions. In cases of delayed diagnosis only biliodigestive anastomoses, e.g. hepaticojejunostomy, are indicated to avoid stenosis of the anastomosis. Injuries to the biliary tract can be reduced by early diagnosis of bilary diseases and correct intraoperative diagnostic means.

Bile Ducts↗

[Malignant obstructive jaundice. Current status of surgical therapy].

424 patients with malignant jaundice were treated from 1964 to 1976. 152 carcinomas of the extrahepatic biliary tract and 272 carcinomas of the duodenopancreatic region were reported. Initial symptoms of cancer of this region are often vague, so that physicians must employ a high index of suspicion if earlier diagnosis is to be effected. Most of the carcinomas of pancreas and the biliary tract are unresectable because of the early dissemination. The dismal of the malignant tumors of the biliary tract is unchanged during the last 50 years but survival rates are better in carcinoma of the duodeno-pancreatic region.

Bile Duct Neoplasms↗

[Splenorenal anastomoses].

Of the operative procedures that can be used to relieve the pressure in portal hypertension, the authors prefer the terminolateral splenorenal anastomosis if the assessment of relative anatomical positions and the patient's hemodynamic situation has shown that these are adequate for the procedure. In contrast to portacaval shunting, after splenorenal anastomosis portal blood supply to the liver is partially maintained. Therefore less patients suffer from hepatic failure or encephalopathy. In addition a lower short-and long-term postoperative mortality has been observed. The risk of thrombosis and recurrent bleeding are the drawback of the method, occurring in about 15% of the patients. A meticulous technique can reduce this percentage.

Esophageal and Gastric Varices↗

[Sclerosing of bleeding oesophageal varices (author's transl)].

Bleeding from oesophageal varices was stopped in two patients by percutaneous transhepatic transportal injection of 50% glucose and thrombin into the gastric coronary vein and the gastro-oesophageal varices draining into it. The only complication was a right-sided pleural effusion in one of the patients.

Esophageal and Gastric Varices↗

[Ascending choledochal papillomatosis (author's transl)].

A case of benign ascending choledochal papillomatosis is reported. The pathogenetic, diagnostic and therapeutic points of view and the prognosis of this disease are discussed in the light of cases so far communicated in the literature. Although, histologically, the choledochal papillomatosis is a benign tumor, its clinical course corresponds to that of a malignant growth. That being so, a radical removal of the tumor is guaranteed exclusively by duodenopancreatectomy.

[Effects of dextran, gelatine and homologous plasma on circulation in shock caused by severe anemia in the dog].

Irreversible shock is produced in 28 anaesthetized mongrel dogs by withdrawal of erythrocytes and substitution by dextran, gelatine or plasma to an average Hb of 4.5g%. In contrast to other shock models cardiac output and perfusion pressure are not decreased by this preparation. In order to maintain arterial blood pressue above 90 mm Hg significantly more gelatine than dextran or serum has to be infused. There is no difference in hemodynamic responses after treatment with dextran or gelatine respectively. Both groups treated with colloidal solutions have the same survival rate of 38%. In the serum substituted dogs, however, survival rate is significantly greater (83%). Very likely this is due to a greater buffer capacity preventing severe acidosis, which is observed in the animals treated with colloidal solutions.
