[Surgical treatment of rectal neoplasms in the very old patient].

Efforts and results of curative and palliative treatment of rectum carcinoma in the very aged (more than 75 years old) are shown by a follow-up study including 212 patients operated in our clinic. Establishing an absolute indication for radical resection of the malignant tumor the contraindications in this collective are pointed out. There is practically no difference between the results of curative and palliative treatment of rectum carcinoma in the very aged regarding letality and morbidity if contraindications are strictly considered and accurate pre- and postoperative treatment is guaranteed.

[The colonic carcinoma. Notes on clinical aspects and therapy].

1752 cases of colon carcinoma are reviewed. History and characteristics of these tumors are described and the results of operative therapy are shown. 50% of the curatively operated patients were still alive after five years, and it seems especially important to think of the possibility of such a carcinoma. Most of our cases have shown early symptoms. It is necessary to interrogate every patient in this respect.

[An intraperitoneally positioned IUD--mistaken for a Spieghel hernia].

A 40-year-old woman presented with the typical signs and symptoms of a Spigelian hernia. Upon operation, an inflammatory conglomerate tumor in the lower omentum was found, containing a copper T. As there were no signs of perforation it is to be discussed whether the IUCD migrated through the right Fallopian tube or penetrated through the uterine wall without leaving a scar.

[Struma. Results of surgical management].

Case report about 3620 struma operations and comparison of the results with the publications of other authors. The situations of malignant strumas are considered separately. Postoperative complications, causes of death and mortality are also considered. The necessity of postoperative hormon-substitution is pointed out.

[The rectal carcinoma. Characteristics of the tumor and its position within the framework of colonic neoplasms].

For the period of 15 years (May 1960 till the end of April 1975) all cases of colon carcinoma (1752) were checked. Of these 851 carcinoma were located in the rectum. Results concerning anamnesis, diagnosis and therapy between these groups are compared and discussed. The necessity of early recognition and the possibilities of practical use are underlined and described.

[Pancreatic pseudocysts--clinical picture and surgical treatment].

Report about 98 cases of pancreatic pseudocysts and also elaboration on the etiology, pathogenesis and details of this disease. The operative method of choice is to establish the internal anastomosis. As long as the anatomic conditions and especially the structure of the cyst permit it, such anastomosis should be performed. We applied this method in 57.2 p.c. of the cases altogether. Y-anastomosis (Roux) proved to be the best. We applied it in 35.9 p.c. of our patients. For patients in bad general condition, outside drainage operation in the sense of marsupialisation is often the only possible and at the same time life-saving intervention. We performed this operation in 42.8 p.c. of our cases. Because of the clear increase in number of pancreas diseases in the last years, there is also a higher frequency of pancreatic pseudocysts. Their diagnosis and therapy are therefore important.

[Coprolith-ileus of the small intestine].

The case of a 71 year old man is reported, in which coproliths developed in the small intestines over a period of 13 years and which finally lead to a complete ileus. This condition is discussed in connection with other rare forms of ileus; specific clinical phenomena are pointed out, which might be helpful in establishing a correct diagnosis. Among the transient forms of ileus associated with food intake, which may cause the patient only slight discomfort, one should bear in mind a corpus alienum-ileus. Only operative therapy of ileus is promising success.

[Carcinoma of the pancreas: palliative surgery (author's transl)].

Among 1146 patients with diseases of the pancreas observed over a 15-year period, there were 234 cases of malignant duodeno-pancreatic tumors. This report concerns the intraoperative diagnostic methods and the various palliative operations available. There was a ratio of 13 males to 10 females, and the average age was 63 years. In 24.3 percent of the patients attempts to establish the diagnosis intraoperatively by puncture or excision were unsuccessful. The cardinal symptom in 71 percent of these cases was icterus. Duodenopancreatectomy was possible only in 21 cases (= 8.9%). In 173 cases (= 75.2% of all patients) a palliative operation was performed.
