[Radionuclide studies in chronically hemodialyzed patients. Bone scintigraphy for the evaluation and control of renal osteopathy].

The clinical applicability of bone scintigraphy (Tc99m MDP) was evaluated in 42 patients on maintenance hemodialysis. Typical scintigraphic findings are shown which were related to hormonal and biochemical parameters of calcium and phosphate metabolism. Visual grading of representative regions for metabolic bone disease in bone scans was compared to scintimetry which applies a bone to soft tissue ratio to grade osseous abnormalities. It could be shown that visual interpretation and grading of the findings according to a score is sufficient to assess the degree and extent of renal bone disease. Semiquantitative analysis of bone scintigrams by scintimetry did not improve the diagnostic information.

[Sonography and scintigraphy in the differential diagnosis of diffuse liver parenchymal diseases].

70 patients with established diffuse liver disease (steatosis, fibrosis, cirrhosis) were investigated by sonography and scintigraphy. Sonographic parenchymal morphology and scintigraphic activity pattern were correlated with the histological diagnosis. Scintigraphy as a functional diagnostic technique revealed early fibrotic changes of the liver not seen by ultrasound. Cirrhosis could be demonstrated equally by both methods. The two major advances of ultrasound were: high accuracy in steatosis and ability of simultaneous analysis of the other abdominal organs.

[Real-time sonography of the skull in the newborn].

The results of 293 examinations of the skull by sonography in 170 newborn are presented. In 73 patients the results could be compared with the findings on CT or at post mortem. In 48%, abnormal sonograms of the skull were obtained, due to congenital abnormalities, hydrocephalus or intracerebral bleeds. High accuracy and wide application make real time sonography the diagnostic procedure of first choice for the examination of the brain of the newborn. An attempt has been made to determine the optimum time for the examination and suitable intervals for follow-up.

[Mirizzi syndrome: a sonographic diagnosis?].

The ultrasound appearances of 11 patients with operatively confirmed Mirizzi syndrome have been analysed. The trio 'dilated intrahepatic bile ducts, concretions in the neighbourhood of the dilated common hepatic duct with a normal distal duct' permit the diagnosis of the Mirizzi syndrome with considerable certainty. In five patients these features were found by sonography and no other diagnostic procedure was necessary. In six patients, ERC was carried out in order to evaluate the distal common bile duct. In one case PTC was carried out, since the liver hilum could not be seen on sonography.

Acquired cystic disease of the kidneys in chronic hemodialyzed and renal transplant patients.

In this study, the appearance of renal cysts in 43 chronic hemodialyzed patients (mean duration of dialysis treatment: 26.3 months) was investigated by sonography with a high resolution (3.5 MHz) sector scanner. In an investigation of the patients' own kidneys, 23 renal transplant patients (mean observation time: 51.3 months after transplantation; mean dialysis treatment before transplantation: 22.8 months) were also studied by sonography. Cysts could be demonstrated in 21 of 43 (49%) patients on maintenance hemodialysis. In 10 of these patients a previous investigation at the beginning of dialysis did not demonstrate any cysts. The diameter of the cysts varied between 5 and 30 mm. With regard to the duration of dialysis, cysts could be demonstrated by sonography in 9 of 23 (39%) dialyzed patients, with a maximum dialysis duration of 2 years, and in 12 of 20 (60%) patients who had been dialyzed for more than 2 years. The development of acquired cystic disease of the kidneys in dialysis patients seems to be promoted by the longer survival of uremic patients. Cysts could be found in the patients' own kidneys in only 4 of 23 (17%) renal transplant patients. The difference in the demonstration of cysts between patients on maintenance hemodialysis and renal transplant patients was statistically significant. This suggests that cystic transformation may possibly be a reversible process.

Small bowel double-contrast enema in stage III ovarian cancer.

The efficiency of small bowel double-contrast enema in the detection and localization of tumor- or therapy-induced lesions of the intestine was studied retrospectively in 43 patients with stage III ovarian carcinoma. The radiographic findings in 62 examinations were verified by operative and autopsy findings and by the clinical course. Postoperative changes in the small bowel were noted in 69% of the patients (63% moderate, 6% severe). Signs of acute radiation enteritis were found in 36% (all moderate). Signs of chronic radiation enteropathy were detected in 71% (53% moderate, 18% severe). Small bowel obstruction due to recurrent tumor was correctly identified in 9%. Nonobstructing peritoneal implants were detected in 27% of the patients. The small bowel double-contrast enema is accurate in localizing lesions resulting from adhesions, acute and chronic radiation enteritis, or obstructing tumor; it is less efficient in detecting nonobstructive peritoneal metastases. The major clinical value of this examination is its ability to differentiate "dysfunctional intestine," which is managed conservatively, from focal obstruction requiring surgery. The radiographic features of chronic radiation enteritis on double-contrast enema examination are discussed in detail.

Prevention of postvenographic thrombosis by heparin flush: fibrinogen uptake measurements.

The incidence of postphlebographic venous thrombosis was investigated by 125I-labeled fibrinogen uptake tests in 60 patients whose veins were flushed with saline solution containing 10,000 IU of heparin after leg phlebography. Ionic methylglucamine iodamide was used as the contrast medium. In six patients superficial thrombophlebitis extending from the contrast-medium injection site was observed after phlebography. The incidence of deep venous thrombosis was 3.3%, significantly less than that reported for studies using triiodinated ionic contrast media without flushing the veins with a heparin solution. It is comparable to the incidence of venous thrombosis reported after using nonionic contrast media. The authors conclude that flushing the veins with heparinized saline solution can improve the safety of phlebography considerably.

Primary Hodgkin's disease of the lung. Case report and review of the literature.

A patient with primary Hodgkin's disease of the lung is described. Special features of this case were alcohol-induced chest pain as the main presenting clinical symptom and the documentation of the evolution of the pulmonary mass by serial X-rays. Complete remission was achieved by lobectomy and subsequent MOPP-therapy. Since then the patient has been in unmaintained remission for 36 months.

Detection of intestinal diverticula by double-contrast small bowel enema: differentiation from other intestinal diverticula.

This report documents the roentgenographic diagnosis of diverticula in the jejunum and ileum. The experience has been gained from 7 Meckel's and 8 other diverticula shown by the small bowel enema in 400 patients. Diverticulum detection rate by this technique approaches the incidence in autopsy series. Differentiation between Meckel's, acquired, and pseudodiverticula by means of specific radiologic criteria is discussed.

[Primary aspergillosis of the lung in leukoses].

The course, radiological features and pathological changes of primary aspergillosis in four patients with acute lymphatic leukaemia are described. Two patients died despite antimycotic therapy. Breakdown of the infiltrate could not be established radiologically, nor pathologically. In a further two patients, a 'primary aspergilloma' developed during the course of several days. Both patients survived. The radiological demonstration of a fungus ball and of an air crescent in a typical aspergilloma can be regarded as a good prognostic sign.

[Radiologic and endoscopic correlation in Crohn's disease of the colon].

A comparative assessment was carried out in 47 patients with double-contrast irrigoscopy (DCI), colonoscopy and endoscopy, the confirmed diagnosis being Crohn's disease in each of the patients. However, the histological finding of Crohn's disease was established in 19% only of the endoscopic biopsies. DCI enabled a more comprehensive assessment of the entire colon than was possible via endoscopy, since the right half of the colon could be visualized in less than 50% of the patients by the endoscopic method, whereas roentgenologic visualization of this part of the colon was possible in more than 90% of the patients. There was good agreement in respect of parts of the colon visualized both via endoscopy and via x-ray examination. As far as the assessment of deeper mucosal lesions was concerned, radiology proved superior, whereas endoscopy offered advantages in respect of changes on the level of the mucosa.

[Sonographic problems in the assessment of abdominal aortic aneurysms].

Ultrasound has taken a solid position in the diagnostic management of aneurysms of the abdominal aorta as it is a non-invasive, simple and quickly practicable method with a low expenditure. After clinical examination ultrasound holds the second place in the sequence of various diagnostic procedures in the case of an aneurysm of abdominal aorta. Diagnostic problems are demonstrated in the sonographic judgement of aortic aneurysms. 42 patients with an abdominal aortic aneurysm have been evaluated in relation to the sonographic features of the aneurysm.

[Experience with double-contrast examination of the small intestine].

Experiences and results of 600 small bowel examinations in double-contrast technique are reported. Pathologic lesions were found in 40%. The double-contrast technique has proved to be an efficient method in detecting small bowel pathology. Its specific value lies in precise delineation of pattern, extent and intensity of disease.

[Non-cardiogenic pulmonary edema].

Non-cardiac pulmonary edema comprises all types of pulmonary edema not caused by increase of left ventricular filling pressure and elevated pulmonary capillary pressure. In one year 42 patients at our intensive care unit developed non-cardiac pulmonary edema. In a retrospective study the clinical, radiological and functional changes in patients with non-cardiac pulmonary edema were determined. 76% of the patients had multiple causes for development of non-cardiac pulmonary edema. Sepsis was the most frequent predisposing disease. Over-all mortality reached up to 69%. Additional organ failure caused an increase in mortality. Patients without complications had the best prognosis. Mechanical ventilation (69%), high-dose corticosteroids (50%), hemodialysis with ultrafiltration (33.3%) and hemofiltration (7.1%) were used for treatment of non-cardiac pulmonary edema.

[Results of double-contrast examination of the colon].

Experiences and results of double-contrast enemas are reported. The accuracy of double-contrast enemas is proved by 500 consecutive investigations. Correlation of endoscopic and roentgenologic investigations showed, that the double-contrast enema is a reliable method concerning the detection of polyps, carcinomas and inflammatory colon diseases. Advantages and disadvantages of roentgenology and endoscopy of the colon are discussed.
