[Nephrogenic adenoma of the urinary bladder with concomitant multiple adenomatous polyps of the large bowel. An abortive form of Gardner's syndrome? (author's transl)].

For the first time, a nephrogenic adenoma of the urinary bladder coinciding with multiple adenomatous polyps of the large bowel, in a 64-year-old male, is reported. The relationship of these tumors at different sites to a known clinical systemic disease is questionable; they could conceivably be ascribed to an abortive form of Gardner's syndrome. On the basis of a case report, the essential aspects of the nephrogenic adenoma of the urinary bladder are reviewed and discussed.

[Malacoplacia of the vagina, a rare cause of vaginal bleeding (author's transl)].

In a 74 years old woman who developed bleeding per vaginam following a paroxysm of cough colposcopic inspection revealed a yellowish-brown soft tissue of the mucous membrane of the upper vaginal vault. Microscopical examination showed a typical Malacoplacia of the vagina. Bleeding per vaginam resulted from ruptured vessels of this granulation tissue. This seems to be the 2nd case of a Malacoplacia of the vagina, as far as we are informed.

[Morphological changes in the hypopharynx-esophageal region following laryngectomy. II. Median-sagittal infolding above the ventral aspect of the hypopharynx-esophageal region (author's transl)].

This paper deals with the morphological findings in the hypopharynx-esophageal region of two patients following laryngectomy. In these patients we found a median-sagittal infolding, situated above the ventral aspect of the hypopharynx-esophageal region. This infolding was due to the surgical closure of the ventral wall of the esophagus, following laryngectomy. Histological examination revealed a hernia-like prolapse of the tunica mücosa, m. mucosae and tela submucosa with numerous proliferated macrophages and blood vessels, but no lymphatics. These macrophages are concerned with the removal of an edema, which, however, is only imperfectly removed and finally turns out into a fibrous tissue. The functional meaning of this infolding is not yet clear, but it seems intimately related to the altered function of the hypopharynx-esophageal region following laryngectomy.

[Papillary adenofibroma of the uterus: fact or fancy? (author's transl)]].

This report deals with the morphological findings in the uterus of a 40 yrs old premenopausal woman, who complained of vaginal bleedings. Macroscopical and microscopical examinations revealed a hernia-like prolaps of a part of the wall of the tuba uterina across a hole in the myometrium of the fundus uteri. Vellios et al. (1973) describe a similar lesion but without paying attention to the prolaps of the tuba uterina into the cavum uteri. They interpretate the lesion as a papillary adenofibroma of the uterus and believe it to be the benign counterpart of the mixed mesodermal tumor of Müllerian duct origin. Based upon our histological findings we believe in a dysontogenetic or traumatic etiology of this rare lesion. The diagnosis of a papillary adenofibroma of the uterus therefore should only be made when the uterus and the adnexa in toto are available for histological examinations.

The natural history of nephrogenic adenoma of the urinary bladder.

The nephrogenic adenoma is a peculiar lesion of the urinary bladder characterized partly by villous and partly by gland-like structures. The two layered surface epithelium consists of an eosinophilic cell layer, which covers a clear cell layer, the latter resting upon a delicate basal layer. The clear cells disappear when approaching the tip of the villi. There exists a 200 micron wide zone of transition between the urinary bladder epithelium and that of the neprogenic adenoma. At the base of the villi there are scattered nodules of gland-like structures lined either by eosinophilic cells or by clear cells. These epithelial cells are derived from the corresponding surface epithelial cells from which they had separated by active downwards growth.

[On morphological changes in the hypopharynx-esophageal region following laryngectomia (author's transl)].

This report deals with the morphological changes in the hypopharynx-esophageal region of two patients following laryngectomia. In these patients we found two sagittal infoldings of the lateral walls of the hypopharynx-esophageal region. Histological examination revealed a nodular circumscribed hypertrophy of the innermost fascicle of the continuous helical system which, by time, becomes replaced by a fibrous tissue. Later on, the middle and outer fascicles of the continuous helical system undergo the same changes. Both these processes result in the formation of a hypertrophic scar which finally turns out in an atrophic scar. The volume of the esophageal wall at the same time as maintained by an increase of adipose tissue in the outer third. This adipose tissue spreads towards the lumen and finally replaces all the scar tissue. In clinical experience such infoldings are not a rare finding, however it seems doubtful if they can get any meaning in the phonation. Nevertheless the formation of such infoldings seems to be intimately related to the altered function of the hypopharyngeal region following laryngectomia.

Chronic hemofiltration. A critical evaluation of a new method for the treatment of blood.

The experiences which have been compiled in more than 2400 hemofiltrations confirm that this method represents an alternative way of treating uremic patients. The main advantages of chronic hemofiltration are the comfort of the patient and the ease in handling excess overhydration without extending treatment time, which is less than 3 X 3 hours/week if adequate hemofilters are used. With regard to the improvement of such typical uremic complications as severe hypertension, hypertriglyceridemia or neuropathy, hemofiltration does not seem to be superior to hemodialysis. However, since most hemofiltration patients do not require phosphate binders and, additionally, remarkable amounts of parathyroid hormone are removed during one hemofiltration, it appears possible that hemofiltration might be an important therapeutic alternative for those renal patients who suffer from severe hyperphosphatemia and secondary hyperparathyroidism.

Specific action of narcotics on reflex activation of rat alpha-motoneurones.

Specific effects of narcotics (opiates) were studied on rat extensor alpha-motoneurones. The animals were anaesthetized with halothane, artificially ventilated and immobilized with N,N'-diallyl nortoxiferinium-HCl. The alpha-motoneurones were activated by tetanic stimulation of the cut ipsilateral gastrocnemius-soleus (GS) nerve. Morphine (2 and 4 mg/kg) administered intravenously, significantly increased the frequency of reflex discharges. In most of the neurones tested, naloxone (0.25 mg/kg) given intravenously, abolished the effect of morphine. In some neurones, however, naloxone induced a further activation. The dose of naloxone employed was ineffective when given alone. The effect of morphine was mimicked by an intravenous injection of levorphanol (1 mg/kg), but not by an equimolar dose of the stereoisomer dextrorphan, which suggests that the activating effect on alpha-motoneurones is a specific one. An intraperitoneal injection of apomorphine (1 mg/kg) reduced the effect of morphine. The effect of narcotics on alpha-motoneurones parallels narcotic-induced catalepsy and muscular rigidity, with regard to dose-dependence as well as to the antagonism of naloxone and apomorphine, and suggests that both catalepsy and muscular rigidity are mainly due to an activation of extensor alpha-motoneurones. Since this activation can be inhibited by spinalization of the rats, it can be inhibited by spinalization of the rats, it can be concluded that the activation is due to a supraspinal action of morphine, resulting in a decreased dopaminergic neurotransmission in the brain.

Effects of 1,3-dimethyl-5-aminoadamantane hydrochloride (DMAA) on the stretch-induced reflex tension of flexor muscles and the excitability of the gamma-loop in decerebrate and spinal cats.

The effects of 10 mg/kg i.v. 1,3-dimethyl-5-aminoadamantane hydrochloride (memantine, DMAA) on the stretch-induced reflex tension of flexor muscles extensor digitorum longus and tibialis anterior (EDL/TA) and on the excitability of the neurons relaying transmission in the gamma-loop have been investigated in decerebrate and spinal cats. DMAA essentially reduced the relfex excitability of flexors EDL/TA induced by fusimotor activity in the decerebrate preparation. The drug did not stimulate the reflex activity in acute spinal cats. In the concentration used, DMAA suppressed largely the transmission of the fusimotor reflex in the decerebrate as well as in the spinal preparation, although in spinal cats DMAA increased the average firing rate of muscle spindle primaries originating from EDL/TA muscles. The possible mechanism of action of the compound on dopaminergic and serotonergic systems as well as its basic effects on neuronal membranes is discussed.

[Microcalcifications in squamous cell carcinoma of the cervix (author's transl)].

The findings presented indicate that localized calcifications can sometimes develop within squamous cell carcinoma of the cervix. Apparently, these deposits develop via calcium salt loading from epithelial necrosis within the cell complexes of the tumor. This means that, with microcalcifications in a metastasis of a squamous cell carcinoma, the pathologist evaluating biopsies should also consider the possibility of metastases of a tumor from the area of the cervix.

[Investigations with 3H-thymidine and electron microscopical studies in rat livers following thermocoagulation of the thoracic duct at the base of the neck (author's transl)].

Autoradiographic studies with 3H-thymidine were done in 90 male and female white Sprague Dawley rats following thermocoagulation of the thoracic duct at the base of the neck. In the acute stages of these experiments we were unable to find any significant influence of a lymphedema on the hepatocytes and Kupffer cells. In the longterm experiments the labelling indices of the hepatocytes were slightly above those of healthy controls of the same age. In another 14 male and female white Sprague Dawley rats we did electron microscopical investigations of the liver. In the acute phase of the experiment we found an intra- and extrahepatic edema as well as damages of the hepatocytes at the periphery of the liver acini. After 12-15 days these findings have regressed. It seems therefore that an intrahepatic lymphedema does not influence the regenerative capacity of the liver to a marked extent.
