Association of subacute motor neuronopathy with thymoma.

A case of subacute motor neuronopathy in association with thymoma is described. Subacute motor neuronopathy is marked by a painless, progressive, and asymmetric muscle weakness that usually affects the lower extremities. It is a rare paraneoplastic effect of tumors that has been described with both Hodgkin's and non-Hodgkin's lymphoma. This is the first case report of its association with thymoma.

Perinatal coaching--a new beginning.

A hospital-based perinatal coaching program fosters communication between first-time parents and their infants. Support provided includes learning about babies' individual differences, normal responses, bonding principles, modeling techniques, early childhood development, as well as realistic expectations of new parents.

Use and misuse of the tape-measure as a means of assessing muscle strength and power.

The tape-measure is frequently used by the medical profession to assess muscle function following injury. It is assumed that increasing limb girth is reflected by increasing muscle power and strength; but this assumption is empirical and may not in practice be justified. The relationship between thigh circumference and muscle strength and power, measured as the torque produced on an isokinetic dynamometer, was assessed in 64 subjects--24 non-injured sportsmen and 40 injured sportsmen. The correlation between the torque produced at the knee by the knee extensors and flexors and the thigh circumference at three levels was not significant in either group. Comparison of the injured with the non-injured limb also failed to show any correlation. By contrast, in eight subjects, repeated measurements over a six to eight month period showed a significant relationship between change in thigh circumference and change in quadriceps power. It is concluded that a single measurement of circumference at the same point in both thighs of a subject with unilateral injury should not be used as an assessment of muscle function. Serial measurements, however, at the 4 and 10 cm levels may be of value as an index of quadriceps power. Reasons for this are discussed.

Treating adolescent obesity: a pilot project in a school.

A weight control program for junior high school students was developed to change eating, exercise and habit patterns. The course included dietary management, physical activity and behavior modification techniques. Classes were structured to emphasize peer group involvement and daily practical application. The curriculum was compiled in a book for educational personnel. The program was tested as a pilot project with a selected group of eighth grade obese adolescent girls and a comparison group. Immediately after the course, the subjects' percent of overweight had decreased; at a subsequent eight-month follow-up, additional losses were noted. The percent of overweight of the girls in the comparison group remained relatively unchanged. While the program was successful with this particular group of students, several key questions, requiring further study, were defined.

A physiological role for calcitonin: protection of the maternal skeleton.

Plasma-calcitonin levels, measured with an established and reliable extraction radioimmunoassay technique, were significantly higher throughout normal pregnancy and lactation than in normal non-pregnant women, and were not immediately influenced by the acute stimulation of breast-feeding. Thus, more calcitonin circulates at times of physiologically increased calcium need. It is suggested that an important function of calcitonin is the protection of the healthy maternal skeleton from excessive resorption by opposing the resorptive action of 1,25-dihydroxycholecalciferol on bone.

Spurious primary amenorrhoea: a case report.

An unusual case of 'primary amenorrhoea' is described, in which clinical, cytogenetic, biochemical and hormonal features were all normal. Failure to obtain withdrawal uterine bleeding after therapy with a combined oestrogen/progestagen preparation, together with the finding of normal cyclical oestrogen and progesterone secretion, the latter reaching normal luteal phase levels, strongly suggested end-organ (endometrial) failure. However, 6 weeks after a searching interview by one of us (H.C.), during which sufficient evidence was uncovered to implicate major emotional factors as contributing to her state of amenorrhoea, the patient began menstruating and has menstruated regularly ever since. It is likely that the patient denied the existence of her menstrua and that the term 'spurious' primary amenorrhoea would most appropriately described this condition.
