Retinal complications of gas myelography.

Bilateral retinal and preretinal hemorrhages with an acute and permanent partial loss of visual acuity developed in a 60-year-old man after an oxygen myelogram. In order to increase the contrast of the myelogram, a greater volume of oxygen was injected in the subarachnoid space than the volume of CSF that was removed. It is believed that the hemorrhages and visual loss were secondary to the sudden increase in CSF pressure caused by the excess volume of oxygen injected.

Marginal corneal ulcers with acute beta streptococcal conjunctivitis and chronic dacryocystitis.

A 73-year-old woman had right dacryocystitis, intense conjunctival hyperemia and chemosis, marginal corneal ulcers, and abscesses and conjunctival cultures that were positive for beta hemolytic streptococci. A distinct lucid interval separated the peripheral corneal ulcers and infiltrates from the corneoscleral limbus. Gram stain of corneal scrapings revealed polymorphonuclear leukocytes but no bacteria, and corneal cultures were negative for bacteria. The peripheral corneal ulcers and abscesses in our patient with the lacrimal conjunctivitis of Morax clinically resembled the catarrhal ulcers found with staphylococcal blepharitis. A hypersensitivity or toxic reaction to streptococci or their products may have played a role in the development of the marginal ulcers in this patient.

Myocardial infarction in dermatomyositis.

A case of dermatomyositis manifesting electrocardiographic and vectorcardiographic changes compatible with an inferior and anterolateral wall myocardial infarction is presented herein. The nonocclusive nature of the heart muscle damage is evidenced by perfectly normal coronary artery angiography. We believe that this is the first reported case of a myocardial infarction pattern apparently due to dermatomyositis.

Appraisal of the PAM cell effect as a diagnostic test for multiple sclerosis.

The selective reduction of PAM cell yield reported by Carp and associates in the presence of tissue from patients with multiple sclerosis (MS) has been attributed to replication in culture of a viral agent associated with MS (MSAA). We investigated the diagnostic potential of the PAM cell effect in MS and in optic neuritis (ON). Six serum and 7 CSF samples from 7 patients with MS, 3 serum and 3 CSF samples from 4 patients with idiopathic ON, and 4 sera from 4 patients with ON induced by ethambutal toxicity were tested. Blind counting showed no reduction in PAM cell yield in the MS group nor any significant difference between the two ON groups. Disturbing inconsistencies in PAM cell growth rates over time and between control flasks were demonstrated.

Suicide in Jewish legal and religious tradition.

While God's primordial warning that He will require a reckoning for the blood shed by suicide has lost nothing of its admonitory and deterrent purpose, Jewish law, as it developed in the course of time, in actual practice takes cognizance only of two kinds of suicide: One that is permissible, by reason of its motivation, and that may in given situations even be highly laudable; and one that is the outcome or symptom of mental disturbances or otherwise legally excusable. Nor is the law relating to suicide the only instance where Jewish jurisprudence succeeded in neutralizing threats of divine punishment by an embarras de richesse of human justifications.

Nerve fiber analysis and optic disc parameters with the glaucoma-scope.

PURPOSE: We evaluated the ability of optic disc parameters and nerve fiber analysis with the Glaucoma-scope to distinguish glaucoma from normal or hypertensive eyes. METHODS: Glaucoma-scope examination was performed on 68 eyes of 44 patients split into following groups based on Humphrey 24-2 visual field and clinical criteria of glaucoma: normal eyes (n = 17), ocular hypertension (n = 19), early glaucoma (n = 17), moderately advanced glaucoma (n = 15). The contour of the juxta papillary nerve fiber layers was measured in the four groups. We also evaluated a global index as the mean disc depth corrected for the disc size by dividing by the disc area (MP/D). RESULTS: We found a statistical difference (p < 0.001) between the four groups with the global index MP/D: normal eyes MP/D = -17.42, hypertensive eyes MP/D = -26.34, early glaucoma MP/D = -82.33, moderately advanced glaucoma MP/D = -99.69. We also found a statistical difference between the first two groups (normal visual field) compared to the two glaucoma groups for the mean fiber layer height in the superior and the inferior quadrant. CONCLUSION: Nerve fiber analysis and optic nerve head index calculated with the Glaucoma-scope could prove to be an interesting tool for glaucoma diagnosis and follow-up.

Isolated left-sided scimitar vein connecting all left pulmonary veins to the right inferior vena cava.

When the common pulmonary vein fails to develop, the embryonic connections of the pulmonary veins to one or more of the systemic veins almost always persist. Anomalous pulmonary venous connections to the inferior vena cava (IVC) are typically characterized by hypoplasia of the involved pulmonary veins and pulmonary artery, as well as abnormal parenchyma of the involved lung. Such cases have been described as "scimitar syndrome." We report the case of a young female patient in whom all the left pulmonary veins converged into a common vessel that drained into the IVC but who had a normal left pulmonary artery and left lung. Surgical intervention was successful, and our patient is still alive.

Granular cell tumor of the trachea.

Granular cell tumors are rare neoplasms that arise in different regions of the body. Their cell of origin is still debated among many pathologists, but recent authors consider them to be neurogenic. These tumors are believed to be benign; however, local recurrences after excision and multifocality arouse suspicions for malignancy. In the head and neck, granular cell tumors occur most frequently in the tongue. In the laryngotracheobronchial tree, most tumors reported have been in the larynx and the next most in the bronchi. Only six cases of true tracheal occurrences have been reported previously; two additional cases are reported in this paper. The literature of the cell of origin of these tumors is reviewed, the different theories are presented, and diagnosis, treatment, and follow-up are discussed.

Effusion cytology of desmoplastic small round cell tumor of the pleura. A case report.

Desmoplastic small round cell tumor is a recently described neoplasm with multidirectional differentiation. Although it occurs predominantly in adolescent males as an intraabdominal mass with multiple peritoneal implants, we report a case with primary pleural involvement. The cytologic features correspond to previously reported histologic findings and include tightly cohesive clusters of small round poorly differentiated malignant cells with accompanying desmoplastic stromal cells. Immunohistochemical studies revealed characteristic reactivity to epithelial markers, desmin and one neuroendocrine marker (chromogranin). We believe that together with clinical and immunohistochemical data, desmoplastic small round cell tumor can be diagnosed in effusion or aspiration cytology specimens.
