[Transcutaneous measurement of pO2.(author's transl)].

An apparatus which allowed the measurement of transcutaneous paO2 (tcpO2) was tested on adults in an intensive care unit. The correlation between tcpO2 and blood paO2 was satisfactory (r = 0.91, n = 111), but the slope of the regression line was variable from one subject to another. On account of the poor transfer of information from the arterial blood to the skin surface, this slope could be very flat (0.43 in one of the subjects in the study). The response time of the apparatus was very short and allowed a study of the effect on the individual of variations in FIO2. The equipment appears useful for continuous non invasive monitoring of the progress of the pO2, but it cannot directly give the true value of the paO2.

Group Embedded-Figures Test: psychometric data.

The study examined the performance of 266 undergraduates on the Group Embedded-figures Test. Item analyses, distribution characteristics, internal consistency, sex differences, and differences across independent samples were examined.

A double-lumen subclavian cannula (DLSC) for temporary hemodialysis access.

Vascular access with an indwelling subclavian cannula is now being widely used as an alternative to silastic-teflon shunts or femoral cannulation in patients with end-stage renal failure who are temporarily lacking an arteriovenous fistula. Advantages are the preservation of blood vessels and the avoidance of repeated vein puncture for each dialysis. The only disadvantage of the standard single-lumen subclavian cannula is the need to use a single needle machine. This is an encumbrance especially in the management of acute renal failure. We have developed a double-lumen subclavian cannula which, having 2 blood pathways, does not require the use of a single needle machine. This device function well for temporary vascular access in patients with terminal renal failure who are awaiting AV fistula construction or repair. It is also especially convenient in the management of acute renal failure when there are difficulties in the construction of a shunt.

Malignant papillary mesothelioma of tunica vaginalis testis.

A malignant papillary mesothelioma of the tunica vaginalis testis with retroperitoneal lymph node and pulmonary metastases is reported in a seventy-seven-year-old man. Electron microscopy of a local recurrence documented its mesothelial origin. The clinical and pathologic features of these rare neoplasms are discussed.

An unusual multifocal leiomyosarcoma of the stomach: a light and electron microscopic study.

A multifocal leiomyosarcoma of the stomach originating from the muscularis mucosae with lymph node and distant metastases is described in a 66 year old man. The electron microscopic features of a representative tumor mass and a metastasis confirmed the smooth muscle histogenesis. The light microscopic appearance consistently suggested malignant fibrous histocytoma. The pathological features of gasttric leiomyosarcomas are reviewed with special emphasis on the problem of practical diagnosis. This case also indicates that not all sarcomas with storiform features are necessarily histiocytic in origin.

Histiocytosis X of the skin in an elderly man.

A diffuse truncal cutaneous eruption of six months' duration in a 76-year-old man is described. Light and electron microscopy revealed infiltration of the papillary dermis and overlying epidermis by histiocytic cells containing Langerhans granules, findings characteristic of histiocytosis X. The Langerhans cell as a probable cell of origin and unifying feature for the subtypes of histiocytosis X is discussed. It is suggested that with increased awareness of the variability of histiocytosis X, atypical presentations will be identified more frequently.

Allergic encephalomyelitis in monkeys induced by a peptide from the A1 protein.

A major disease-inducing site for induction of experimental allergic encephalomyelitis in monkeys exists in Peptide P 14, the 37-residue segment of the A1 protein comprising its COOH-terminus. The peptide appears to contain the dominant encephalitogenic determinant, since it was as active as the A1 protein on a molar basis. By contrast, the 9-residue tryptophan region and the Peptide R region, active in guinea pigs and rabbits, respectively, were comparatively inactive in monkeys. The clinical and histologic expression of the disease produced by Peptide P 14 appeared identical to that induced by the intact A1 protein.

Disability support services and the reforms.

Features of disability support service provision in New Zealand are examined. Factors influencing the restructuring of the disability support services are identified in the context of international comparisons. Reference is made to the debate on the health/welfare interface, policy trends that have favoured a shift from institutional to community care and the need to contain escalating public health service expenditure. Changes to the disability support services under the new Regional Health Authority structure are outlined. Thereafter, the influence of the reforms in redressing the problems inherent in the long-term care services is analysed, as well as the possible consequences of the changes.

Diversity among hospice volunteers: a challenge for the development of a responsive volunteer program.

Voluntary organizations such as hospices are increasingly being called upon to substitute for state health and social welfare delivery at a time when volunteer contributions are threatened because of organizational and demographic changes. These changes include: the adoption by the nonprofit sector of market strategies in response to health and social welfare reforms, increases in the size and complexity of hospices, increasing professionalization of staff, and the move of women away from voluntary work in the health and welfare services to paid employment. With these changes, hospices must be knowledgeable about their volunteer workforce and resourceful if they are to continue to attract and retain volunteers. A survey of volunteers conducted at the Mary Potter Hospice, Wellington, New Zealand, examined differences in volunteers' characteristics, motivations for joining hospice, and factors affecting work satisfaction according to age, gender, and the length of time volunteers had worked. Those findings of particular relevance to program development were: (1) the tendency for the recently recruited volunteers to be in paid employment; (2) the mixed motivations of the younger volunteers, reflecting both altruistic and personal gain needs; and (3) the rewards commonly identified by volunteers, such as feeling their work is of value, being accepted by hospice staff, and feeling like important team members. Practical outcomes of the study are described.
