Survey of the implementation of workplace alcohol and smoking policies among employers in Fife.

A survey of major employers in Fife shows that at present 33% of respondents have implemented written alcohol policies, while 40% have implemented written smoking policies. A total of 19% have both alcohol and smoking policies in place. The initiative for policy development appears to have arisen mainly from management, but trade unions and management have co-operated well during policy formulation. The majority of policies have been implemented for more than three years. A small number of companies in Fife appear to be interested in the development and implementation of written alcohol and smoking policies in the workplace.

Measles--mumps--rubella immunization: the role of the general practitioner in achieving a high uptake.

A survey of all general practitioners in Fife conducted prior to the introduction of measles-mumps-rubella immunization on a pilot basis in May 1987 showed that 85% considered mumps worth preventing and 94% believed the rubella programme worth augmenting with universal childhood immunization. Ninety seven per cent considered measles worth preventing and 98% were prepared to recommend measles-mumps-rubella immunization to parents instead of measles vaccine. One year after introducing the measles-mumps-rubella vaccine in Fife, 91% of children had been immunized with the combined vaccine or measles vaccine before their second birthday. This compares with the 83% that received measles vaccine before the combined vaccine was introduced. Eighty per cent of preschool children were also immunized with the combined vaccine at school entry in a catch-up exercise. This study demonstrates that there are few major professional barriers to achieving a high uptake of the measles-mumps-rubella vaccine in this area. The vaccine was introduced nationally on 1 October 1988 and its uptake is likely to exceed the current unsatisfactory level achieved with measles vaccine. However, this outcome will largely depend on the commitment of doctors to the programme.

[Expression of pain in rheumatic disease. Exploratory study].

An investigation done in 34 rheumatology departments on 259 patients in order to analyse their perception of pain and its repercussions on daily living. The main three diseases justifying hospitalization are: rheumatoid arthritis, lumbosciatica and bone neoplasia. In these patients, pain occurs daily in 75 p. cent of them and continuous in 32 p. cent; the intensity of the pain varies according to the time. There are repercussions on work (63 p. cent), walking (81 p. cent), but also on leisure (74 p. cent), friendly interelations (42 p. cent) and disposition which is disturbed in 62 p. cent of patients. Behavioral analysis in the presence of pain brings up differences depending on the patients: the elder the patient, the least they are able to respond to the handicap caused by pain. Analysis of the pain vocabulary suggest that if the terms expressing the experience of the patient are different according to the diseases, the usual description of painful phenomena are common to patients and the diseases in question.

[Failure of chemonucleolysis and its surgical treatment. Apropos of 268 cases].

The authors have reviewed 268 cases of herniated lumbar discs treated by chemonucleolysis between 1980 and 1984. 52 patients had a laminectomy (19,40 p. 100) and 18 of theses failures were due to a large disc herniation (34,61 p. 100). The result of the surgical treatment after chemonucleolysis failure was good in 67,30 p. 100 of cases. A chymopapain injection is not indicated in patients with a large disc herniation.

[Geographic distribution of multiple myeloma in the Loiret region. 10-year retrospective epidemiological survey].

Suspecting the presence of a myeloma focus in a forest area of the Loiret department, we have undertaken a retrospective epidemiological survey on this disease. Three surveys were conducted, between 1972 and 1982: the first one among general practitioners; the second one was carried out from immuno-electrophoreses performed at the University Hospital in Orleans; the third survey was carried out from reports on the medical causes of deaths. The mean number of myelomas in the Loiret (0.029 p. cent) is comparable to the national mean (0.025 p. cent), with a sex ratio of 47.09 p. cent of men, also quite similar to the national sex ratio (46.7 p. cent). The different districts were classified according to the percentages observed in "neutral areas" (mean plus or minus a typical variation), "cold areas" (between one and two typical variations below the mean) and "hot areas" (between one and two typical variations above the mean). These results do not permit to ascertain the existence of a geographical focus of myelomas in Loiret but require a more in-depth study of "hot areas".

[Treatment of sarcoid dactylitis with brief intravenous perfusion of high-dose methylprednisolone].

Sarcoid phalangeal involvement is a rare complication of a chronic disease process. Some forms pose difficult therapeutic problems, as steroids, even at high doses, do not always arrest the destructive changes. We report 3 cases of sarcoid phalangeal disease treated by intravenous infusion of 1 g of Methylprednisolone given in 3 hours on 3 consecutive days at monthly intervals for 4, 5, 7 and 8 months, respectively. Clinical improvement was rapid, constant and of excellent quality. The concentrations of the angiotensin converting enzyme returned to normal but Gallium 67 scintigraphy did not seem to be a helpful follow-up investigation. The radiological lesions were observed to regress during treatment in 1 case. Tolerance was excellent in the absence of maintenance steroid therapy. Further studies are required to assess the long term outcome.
