The burden of cancer in Kentucky. The 1998 Kentucky cancer incidence report.

The purpose of collecting this important disease burden information is just for reports like this. The real value lies in use of data for cancer control research, program planning, resource allocation, program design, and evaluation. Data in the annual KCR report and on the website should be a valuable resource for health agencies, clinicians, policymakers, voluntary organizations, etc in their assessment of health problems in their area and as critical decisions are made about how to utilize limited intervention resources. The Kentucky Cancer Registry will continue to work very closely with the Kentucky Cancer Program, Kentucky Medical Association, and programs and research efforts at the University of Kentucky Cancer Control Program (Mid South Cancer Information Service, Appalachia Cancer Network, Kentucky Prevention Research Center) and the Brown Cancer Center at the University of Louisville to make these data available to partners throughout the Commonwealth to help guide their assessment and planning processes. Breast cancer and retroperitoneal metastasis.

Retroperitoneal fibrosis is an uncommon disease of diverse etiology. Carcinoma of the breast is rarely considered as a cause of retroperitoneal metastasis or fibrosis. This paper describes five patients with retroperitoneal fibrosis secondary to breast cancer. The mean duration from initial diagnosis to onset of symptoms was greater than ten years. The patients had received a variety of treatments for breast carcinoma and had extensive metastatic disease. Presenting symptoms included abdominal, flank, or low back pain. Two had small bowel obstruction and four had ureteral obstruction. It is possible that with long-term survival this complication may be seen more frequently.

Acute acetaminophen intoxication.

Acetaminophen is a readily available, widely used drug which has been thought safer than aspirin. Overdosage which may result in fatal hepatic necrosis is commonly seen in the United Kingdom but is rarely reported in the United States. In a two-month period, three patients were admitted to a general hospital because of acetaminophen overdose, suggesting that this problem may occur more commonly than expected. They were successfully treated with N-acetylcysteine. Our experience in management of these three patients is presented and the pertinent literature concerning diagnosis and treatment of acute acetaminophen intoxication in briefly reviewed.

Bone marrow necrosis. Diagnosis and assessment of extent of involvement by radioisotope studies.

In two patients with extensive marrow necrosis, the diagnosis of marrow necrosis was established by morphologic and radioisotopic studies, and the extent of involvement was accurately assessed by marrow scanning with technetium Tc 99m sulfur colloid while the patients were still alive. The literature on marrow necrosis was briefly reviewed and the clinical features of this condition were characterized. It was found that patients with this condition often have malignancies, underlying marrow disorders, sepsis, bone pain, and pancytopenia. Their marrow is often difficult to aspirate and they may require frequent transfusions to maintain a stable hemoglobin level. Radioisotopic studies are useful in the diagnosis and assessment of extent of involvement of this condition. They should be used in patients with clinical findings suggestive of marrow necrosis.
