The treatment of lead poisoning from gunshot wounds with succimer (DMSA)

Lead poisoning is an unusual complication of gunshot wounds that occurs when retained lead bullet fragments are in contact with body fluids capable of solubilizing lead. The epidemic of violence by gunfire may result in increasing numbers of lead poisoning cases from this exposure. The use of oral chelation for toxicity resulting from this mode of exposure has not been previously discussed. Cases of lead poisoning arising from bullet lead in the synovial cavity of the hip, synovial cavity of the chest, and pleural space are reported. A combination of surgical debridement and chelation therapy with oral succimer produced a satisfactory outcome in all three cases. Oral succimer may be a safe and effective chelation agent for treating lead toxicity in adults with high lead levels secondary to gun shot wounds.

Long term results of subtotal parathyroidectomy in patients with end-stage renal disease.

This is a retrospective, clinical study evaluating the long-term outcome of subtotal parathyroidectomy (PTX) in 60 patients with chronic renal failure and severe secondary hyperparathyroidism. Patients were 41 +/- 2 years old (mean +/- SE) at the time of PTX, and followed for 69 +/- 6 months since the procedure. At the time of PTX, three patients had chronic renal failure, 53 had been on chronic hemodialysis, and four had received successful kidney transplants. In more than 80 per cent of patients, symptoms of hyperparathyroidism (bone pain and muscle weakness) resolved within weeks, and biochemical signs (hypercalcemia, and high plasma alkaline phosphatase and parathyroid hormone concentrations) returned to normal ranges within a year. Subperiosteal resorption, bone fractures, and soft tissue calcification frequently improved. Osteosclerosis (rugger-jersey spine), cystic bone changes, osteopenia, and vascular calcifications were, however, often unchanged or progressive. Five patients (8%) who had either persistent or recurrent hyperparathyroidism required additional surgical procedures, and two had subsequent improvement. Twelve patients who had aluminum associated bone disease diagnosed later continued to progress with a high incidence of bone fractures and severe osteopenia. Cystic bone changes, especially of the carpal bones, in association with carpal tunnel syndrome, probably representing amyloid bone disease, also did not respond to PTX. In conclusion, PTX is an effective surgical procedure to reverse complications of hyperparathyroidism in patients with end-stage renal disease, provided that other causes of osteodystrophy, such as aluminum or amyloid-associated bone diseases, are adequately excluded. We feel that subtotal PTX, leaving a small remnant in place, is the procedure of choice.

Acute promyelocytic leukemia in pregnancy: all-trans retinoic acid as a newer therapeutic option.

BACKGROUND: Acute promyelocytic leukemia is a unique subset of acute myelogenous leukemia, characterized by a neoplastic proliferation of promyelocytes and a prompt response to all-trans retinoic acid (tretinoin), which induces differentiation of immature leukemic promyelocytes into mature neutrophils. Because of the high incidence of disseminated intravascular coagulation (DIC) associated with acute promyelocytic leukemia and the danger of exacerbation of DIC with pregnancy, management of acute promyelocytic leukemia during pregnancy requires prompt and careful attention. CASE: A 29-year-old woman in her third trimester was diagnosed with acute promyelocytic leukemia and DIC. The infant was delivered by cesarean and the mother was successfully treated with tretinoin, inducing the leukemic promyelocytes to differentiate into mature granulocytes and possibly reversing the DIC. CONCLUSION: If the fetus can be delivered safely, tretinoin as a single agent is an option for the initial treatment of maternal acute promyelocytic leukemia because it does not suppress the bone marrow and may ameliorate DIC. Because of the danger of hyperleukocytosis, chemotherapy should be added initially if the white blood cell count is greater than 5000/microL. If the fetus cannot be delivered at a viable stage, conventional cytotoxic chemotherapy is the alternative option.

Molecular cloning reveals the existence of a fourth gamma subunit of the vertebrate brain GABAA receptor.

We have isolated a cDNA, from the chicken, that encodes a fourth type of gamma subunit of the vertebrate brain GABAA receptor. The mature polypeptide (which we name gamma 4) displays 67%, 69% and 70% identity, respectively, to the rat gamma 1, gamma 2 and gamma 3 subunits. In the developing chicken brain, the gamma 4-subunit mRNA is first detected at embryonic day 13; the transcript level then increases progressively during embryogenesis. In situ hybridization reveals that the gamma 4-subunit mRNA is abundant in several brain regions, including the ectostriatum, nucleus rotundus and hyperstriatum ventrale, which are involved in visual processing and learning.

Hemolytic anemia associated with lead poisoning from shotgun pellets and the response to Succimer treatment.

Symptomatic lead poisoning with severe hemolytic anemia was observed in a patient with retained shot gun pellets. Surgical resection of the retained pellets and the use of a newer chelating agent, Succimer (2,3-dimercaptosuccinic acid) successfully lowered blood lead level. Hemolytic anemia was associated with deficient erythrocyte pyrimidine 5'-nucleotidase, and lowering of the lead level corrected the deficiency, suggesting that the enzyme deficiency is responsible for the hemolysis associated with lead poisoning. This case illustrates that retained lead pellets from shotgun wounds can cause severe lead poisoning.

Photoelastic analysis of internal screw fixation after sagittal split ramus osteotomy.

An experimental study on mechanical behavior and stability of internal screw fixation (ISF) of sagittal split ramus osteotomy has been carried out. A stress freezing method in a three-dimensional photoelastic experiment was used. Four ISF's that utilized different screw placement patterns were used for photoelastic analysis: (1) 3 screw linear pattern placed at 90 degrees to the lateral surface of the model; (2) triangular pattern at 90 degrees to the surface of the model, 2 above and 1 below the mandibular canal; (3) 2 screw linear pattern at 90 degrees, and (4) 3 screw oblique linear pattern at 75, 60 and 45 degrees to the surface of the model. The stress distribution differs markedly according to these 4 methods.

An acute toxicity study of recombinant human interferon alpha A (LBD-007) in Sprague-Dawley rats.

The acute toxicity of a recombinant human interferon alpha A (code name: LBD-007) was evaluated in both sexes of Sprague-Dawley rats, 5 weeks old, by the oral, subcutaneous and intravenous routes of administration. Based on the results, LBD-007 was not considered to induce any toxicological effect on the rats in mortalities, clinical findings, body weights and gross findings. It is suggested that LD50 values in rats would be > 48 x 10(8) IU/kg in the study of oral administration and > 24 x 10(8) IU/kg in the study of subcutaneous or intravenous administration.

A 4-week subcutaneous toxicity study of recombinant human interferon alpha A (LBD-007) in Sprague-Dawley rats.

Recombinant human interferon alpha A (code name: LBD-007) was subcutaneously administered to both sexes of Sprague-Dawley rats at the doses of 0, 6, 12 and 24 x 10(6) IU/kg of body weight five days per week for 4 weeks to evaluate the subchronic toxicity. 20 to 50% of rats except the male control group showed minimal to mild, focal to multifocal renal mineralization. Whether renal mineralization is related to the test substance is not elucidated. Male rats dosed at 24 x 10(6) IU/kg showed the decrease of the absolute kidney weights and the increase of the brain relative weights. Female rats dosed at 24 x 10(6) IU/kg showed the increase of the absolute and relative ovary weights, and the decreases of specific gravity, bilirubin and urobilinogen in urine. However, no drug-related changes were noted in clinical findings, body weights, food consumption, water consumption, hematology, blood clinical chemistry and necropsy findings. Based on the results it is concluded that the estimated subcutaneous non-toxic dose of the recombinant human interferon alpha A (LBD-007) in rats is (6 approximately 12) x 10(6) IU/kg.

A 6-month subcutaneous toxicity study of recombinant human interferon alpha A (LBD-007) in Sprague-Dawley rats.

Recombinant human interferon alpha A (code name: LBD-007) was subcutaneously administered to both sexes of Sprague-Dawley rats at the doses of 0, 3, 6 and 12 x 10(6) IU/kg of body weight six days per week for 6 months to evaluate the chronic toxicity. Male rats dosed at 12 x 10(6) IU/kg for 26 weeks showed a decrease of ketone body in the urine, and female rats dosed at 12 x 10(6) IU/kg for 26 weeks showed a decrease of serum alkaline phosphatase. Microscopic changes with remarkable incidence were nephropathy, renal mineralization and ultimobranchial cysts of the thyroid gland in all groups of both sexes of rats. These changes were not considered to be drug-related toxicity because they appeared to be minimal, occasionally mild in severity and did not have dose-dependency as well as clinical significance. In general, no drug-related change was noted in clinical findings, body weights, food consumption, water consumption, hematology, organ weights and necropsy findings. Based on the results, it is concluded that the estimated subcutaneous non-toxic dose of the recombinant human interferon alpha A (LBD-007) in rats would be 6 x 10(6) IU/kg.

Clinicopathologic parameters in predicting cervical nodal metastasis in early squamous cell carcinoma of the oral cavity.

A retrospective study of ninety patients with early squamous cell carcinoma of the oral cavity was undertaken to evaluate clinical and histopathologic parameters in predicting cervical lymph node metastasis. Medical records were examined for age, sex, primary site, growth pattern, and size. The specimens from biopsy and surgical excision were examined for the degree of differentiation, mode of invasion, tumor thickness, muscular invasion, vascular invasion, and inflammatory response. The mode of invasion and tumor thickness were important indicators in the prediction of cervical lymph node metastasis. Tumors with Grade 4 mode of invasion exceeding 3 mm in thickness or with Grade 3 and 4 modes of invasion and more than 6 mm in thickness had a high incidence of cervical nodal metastasis. These important prognostic variables were best evaluated in the deeper portions of the lesions.
