Ovarian mucinous tumor with mural nodules of anaplastic carcinoma.

A case of mucinous cystic ovarian tumor with mural nodules of anaplastic carcinoma in a 30-year-old woman is described. The carcinomatous components within the nodules showed strong immunopositivity for cytokeratin and carcinoembryonic antigen, and ultrastructurally they displayed epithelial and glandular differentiation. Omental metastasis had already developed in the patient, and she received postoperative adjuvant chemotherapy consisting of cyclophosphamide and cis-platinum. No sign of recurrence was evident 4 months after the operation. The literature is reviewed and the importance of adjuvant chemotherapy in the postoperative management of such patients highlighted. The salient pathologic features differentiating mural nodules of anaplastic carcinoma and true sarcoma from prognostically favorable sarcoma-like nodules are presented.

Multiple sclerosis amongst Chinese in Hong Kong.

A territory-wide investigation of southern Chinese patients with multiple sclerosis (MS) was conducted in Hong Kong. There were 47 patients, 35 of whom were clinically definite (CDMS), 6 laboratory-supported definite (LSDMS) and 6 clinically probable (CPMS). The prevalence rate was 0.88 per 10(5) population, which is of the same order as in other Oriental populations but much lower than in Caucasoid populations. Comparisons with major Oriental and Caucasian series showed essentially the same clinical picture with only minor variations. In one autopsy case, the lesions were found mainly in the optic nerves and spinal cord, with marked softening in addition to the classical demyelination features. In contrast to findings in Caucasians, the detection rate of oligoclonal bands in the cerebrospinal fluid was low (33% in CDMS patients) and there was no association with human leucocyte antigens. The possible implications of these findings on the pathogenesis of MS are discussed.

Pompe's disease presenting as supraventricular tachycardia.

A Chinese infant with Pompe's disease (type II glycogenosis) had an unusual presentation of supraventricular tachycardia. The patient subsequently died of ventricular fibrillation. The clinical presentation and the mechanism of the arrhythmia are discussed.

A randomized study of high-dose versus low-dose cis-platinum combined with cyclophosphamide in the treatment of advanced ovarian cancer. Hong Kong Ovarian Carcinoma Study Group.

A randomized clinical study of patients with advanced epithelial ovarian cancer after debulking surgery showed that high-dose (120 mg/m2) cis-platinum (DDP) in combination with cyclophosphamide (600 mg/m2) had a significantly higher response and survival rate than the low-dose DDP (60 mg/m2) and cyclophosphamide combination. The 3-year actuarial survival rate of the high-dose group was 60% and that of the low-dose group was 30%. Though moderate to severe marrow toxicity was evident in 80% of the patients in the high-dose group and 40% of the low-dose group, no serious sepsis or death developed as a result of the marrow depression. Mild neurotoxicity was observed in 55% of the patients in the high-dose group and only 20% in the low-dose group. Mild nephrotoxicity was seen in 25 and 17% of patients in the high- and low-dose groups, respectively. It was concluded that the 120 mg/m2 dose DDP and cyclophosphamide combination should be used in the treatment of carcinoma of the ovary in spite of its toxicities. However, it should only be used in institutions with supportive facilities in the management of patients with severe marrow depression.

Creutzfeldt-Jakob disease in Hong Kong. A case report.

This is a report of a Chinese male, aged 67, who had been ill for 6 months and who was admitted with left hemiparesis and dementia. The diagnosis of Creutzfeldt-Jakob disease (CJD) was confirmed by serial electroencephalographic (EEG), computed tomographic (CT) and neuropathological studies. This is the first formal report of CJD occurring in Hong Kong.

The iliolumbar ligament. A study of its anatomy, development and clinical significance.

The development of the iliolumbar ligament and its anatomy and histology were studied in cadavers from the newborn to the ninth decade. The structure was entirely muscular in the newborn and became ligamentous only from the second decade, being formed by metaplasia from fibres of the quadratus lumborum muscle. By the third decade, the definitive ligament was well formed; degenerative changes were noted in older specimens. The iliolumbar ligament may have an important role in maintaining lumbosacral stability in patients with lumbar disc degeneration, degenerative spondylolisthesis and pelvic obliquity secondary to neuromuscular scoliosis.

Squamous cysts in renal dysplasia.

We report two cases of renal dysplasia with squamous cysts. The histological features in one case suggest that the cysts may develop through metaplasia of the tubular epithelium.

Massive subchorionic hematoma.

Massive subchorionic hematoma (Breus' mole) of the placenta occurred in a stillbirth. The mother was receiving oral warfarin sodium. The extravasated blood was mostly fetal in origin.

Intracytoplasmic desmosomes in tumor cells.

The occurrence of intracytoplasmic desmosomes in some normal and tumor cells was suggested to develop by cell fusion by previous investigators. In this study, similar structures were found in tumor biopsies and tumor cells in culture. It is suggested that these structures develop by membrane incorporation and dissolution in cells with active membrane turnover.

Bronchial cancer in Hong Kong 1976-1977.

Bronchial cancer is a disease of high and increasing annual incidence in Hong Kong, especially in women, whose age-specific death rates from this cause are amongst the highest in the world. A case-control study of the relationship of bronchial cancer with smoking was carried out during 1976--77, taking particular note of the histological type of the tumour. Two hundred and eight male and 189 female patients were interviewed, covering about half the total number of cases of bronchial cancer registered as dead from the disease in Hong Kong during the period of the survey. The association with smoking was more evident in males than in females, and in squamous and small-cell types, as a group, than in adenocarcinoma. Forty-four per cent of the women with bronchial cancer were non-smokers, their predominant tumour being adenocarcinoma, and in them on association could be detected with place of residence or occupation. There was no strong evidence of an association with the use of kerosene or gas for cooking; 23 did not use kerosene. The cause of the cancer in these non-smoking women remains unknown.

Antigenic content of an in vitro cellular immunity reactive cell surface extract of Raji cells.

Antigenic content of a Raji cell surface extract (TS) was studied by sequential abosrption of rabbit antiserum to this extract with different cells. Anti-TS was first allowed to react exhaustively with human blood cells followed by HeLa cells and human diploid fibroblasts. The residual reactivity was studied by stepwise absorption with an EBV-negative B lymphoblastoid cell line, BJAB, and then with B1--19, a clone derived from EBV transformed BJAB cells. These processes delineated three kinds of reactivities in anti-TS having specificities for antigens on B blasts, EBV-determined cell surface antigen(s) and antigens that are exclusive to Raji cells, respectively. These results are compatible with the heterogeneous protein content of TS and with the results of our earlier in vitro cell-mediated immunity studies using a similar preparation of Raji cell TS.

Biochemical significance of the hard and soft acids and bases principle.

The hard and soft acids and bases (HSAB) principle, which states that hard acids bind preferentially to hard bases and soft acids to soft bases, may serve to assess specific chemico-biological interactions. As living systems are composed mainly of "hard" elements, molecular events taking place within the cell are dominated by "hard-hard interactions". On this premise, it becomes likely that extraneous "soft" agents are particularly injurious to life. In the HSAB context a selected number of variegated phenomena are briefly discussed qualitatively; these include biocidal actions, heavy metal poisoning, chemical carcinogenesis, some enzymic reactions, and nucleic acid complexations. Although the HSAB principle cannot be used as a tool for mechanistic explanations of biochemical processes, it may provide clues to likely target molecules and the loci of action.

Factors affecting serum IgA antibody to Epstein-Barr viral capsid antigens in nasopharyngeal carcinoma.

Irrespective of the ethnic origin of the patient, nasopharyngeal carcinoma (NPC), appears to stimulate the production of IgA antibodies against VCA. These antibodies are detected at high frequency and titres in sera from NPC patients but only rarely from control subjects. A majority of relapse-free survivors tested 1-12 years after radiotherapy (RT) sustain a detectable level of IgA anti-VCA. Serum titres of IgA anti-VCA remain relatively unchaged in individual NPC patients after RT, regardless of the disease evolution. These antibodies were detected in serum from one individual 9 months before NPC and the titre rose concomitantly with its clinical onset. Titres of IgA anti-VCA in multiple serum specimens from individual NPC patients, and in sera from different NPC patients, do not correlate with titres of IgG anti-VCA or with Serum IgA. It thus seems possible that the IgA anti-VCA in the sera of NPC patients might be largely derived near NPC. Apparently healthy individuals showing detectable IgA anti-VCA tend to aggregate in families of NPC patients. The distribution of siblings of these families who have the IgA anti-VCA reaction shows the binomial distribution expected for an autosomal recessive trait, implying the involvement of an autosomal recessive gene in the IgA anti-VCA response.

Immunohistochemistry of local immunoglobulin production in nasopharyngeal carcinoma.

Immunohistochemical investigations by the immunoperoxidase method have been carried out on sections of biopsy specimens obtained from the primary tumour sites of patients with nasopharyngeal carcinoma (NPC). It was found that, in many of the sections thus examined, there was an accumulation of plasma cells, particularly of the IgA type, in the connective tissues surrounding nests of NPC cells. Similar accumulation of plasma cells in the subepithelial connective tissues was likewise observed in a case of choanal polyp. Plasma cells were rarely observed in the section of a biopsy specimen of non-neoplastic oropharyngeal mucosa. These results indicated that the nasopharynx may be a site for the local production of IgA, but the antigenic specificity of these molecules is, however, not known. The possibility that the nasopharynx is a site for the local production of antibodies to the Epstein-Barr virus (EBV)-related antigens was discussed.
