[Choroid ganglioneuroma in childhood].

The authors report on a case of a 4 1/2-year-old male child, who was referred because of a convergent microstrabisms of the right eye combined with amblyopia and minimal anisometropia and progressive decrease of the visual function from 0.6 to 0.05 although occlussion-therapy was performed. Funduscopy showed a melanoma-like choroidal tumor temporal-inferior of the macula with shallow retinal detachment and severe cellular vitreal infiltration. Because of the progression the tumor was resected by the transscleral technique of Foulds (1983). Histology showed a ganglioneuroma of the choroid, which has not been reported in this localisation up to now. Usually it is found in the retroperitoneal area and in the mediastinum, specially in case of neurofibromatosis (Recklinghausen). This disease could not be objectified in this meanwhile 7-year-old boy. At the last control visual acuity was 1/20, ophthalmoscopy showed gross streaky depigmentation in the macular region, the retina was attached.

[Pseudotumor orbitae--a chameleon in diagnosis and therapy?].

Between 1972 and 1988, 25 patients were diagnosed and treated for "pseudotumor orbitae" in the Departments of Radiotherapy and Ophthalmology of Münster University Clinics. All cases had been initially confirmed by histology. Later in seven cases (28%) the histology showed a definitive alteration, and eventually turned to malignancy in five patients. Criteria indicating such a shift are seen in higher age, subacute onset of the disease, low pain, minor signs of inflammation, low echoes in ultrasound diagnostics, and the absence of "Dutcher bodies" in histiocytes, if these are evident in histology. These patients at increased risk of malignant transformation should be considered for radiotherapy at an earlier stage than prescribed by the current routine.

[Holographic analysis of displacement of the bovine cornea after disruption of intact structures].

Displacement of the surface of the cornea of bovine eyes after disruption of intact structures was investigated by means of holographic interferometry. Double-T-incisions of different depths were made with a diamond knife. Starting at about 80% of the incision depth there is a change in the central holographic interference fringe pattern. These effects can be clearly demonstrated by means of double-exposure holographic interferometry and real-time holographic interferometry. The stability of an existing corneal scar was analyzed by double-exposure holographic interferometry. The region of the scar showed a higher density of holographic interference fringes compared with the non-affected corneal areas, indicating a higher degree of elasticity. All eyes were investigated at an intraocular pressure of 1340 Pa (about 10 mmHg) and an intraocular pressure change of 10 Pa and 20 Pa. Further potential ophthalmic applications of holographic interferometry are discussed.

[Ocular symptoms in a family with pseudo-Ullrich-Turner syndrome].

We report on a family with some features of the Pseudo-Ullrich-Turner-Syndrome, so-called Noonan-Syndrome. Besides low-set ears, microgenia, short neck, pterygium colli, low-anterior hair line and small stature as well as partial scoliosis, partial cubitus valgus and camptodactylia, retinal detachment, disturbances of the eye motility, keratoconus, unilateral ptosis and antimongoloid slant of the palpebral fissures in different expression are described. The caryotype was normal. Some of the features can be seen within at least 2 generations of the family. It seems to be an autosomal genetic mode of transmission. Differences and common characteristics in comparison to the literature are shown. Differential diagnostic aspects are described. A definite relation to a syndrome already described is not possible.

[Induction of chromosome aberrations in human lymphocytes as a model for evaluating the mutagenic effect of excimer laser irradiation in ophthalmology].

A model for testing the mutagenicity of excimer laser radiation in ophthalmology is presented. In contrast to other studies using cell monolayers, in our study with this model human peripheral lymphocytes (heparinized whole blood) are exposed to laser irradiation at different conditions. The possible mutagenicity of secondary radiation after exposure of the cornea is also considered. The heparinized blood is either injected into the anterior chamber of an enucleated bovine eye or filled into a quartz chamber (positive control). After irradiation the lymphocytes are stimulated and fixed during metaphase, after which the metaphases are scored for chromatid and chromosome aberrations. In the positive control group, exposure to 248 nm excimer laser radiation (500 J, 277 mJ/cm2, 10 and 20 Hz) was followed by a highly significant increase in the yield of chromatid aberrations (chromatid breaks and achromatic lesions).

[Reasons for failure after episcleral buckling operation of the retina].

In this retrospective study about failures of epiretinal buckling procedures performed over 12 years the causes of failure are analysed. The main ones are: new retinal holes, PVR, wrong localization, and inadequate buckling effect. The aim of this investigation was to find indications of the best technique, with a view to improving the success rate, currently, 80-90%, even further.

[Surgical therapy of uveitis].

Cryotherapy is a good method of treating chronic uveitis, because it is minimally invasive and there are normally no complications. It has proved possible to reduce the frequency and intensity of inflammatory episodes, so that in many cases systemic drug therapy is superfluous. Stabilization or even regression of secondary conditions such as cystoid macular edema or cataract has been achieved, a phenomenon also observed among the present authors' own patients. In cases which did not respond to cryotherapy or where there were severe vitreous opacities vitrectomy was performed. A total of 207 eyes were followed up, on average for 1 1/2 years. During this time visual acuity remained stable or improved in 76% of those treated by cryotherapy, and in over 86% of those treated by vitrectomy.

[Fluoridation measures and caries prevalence in school children in the former DDR].

Statistical analysis of 3,576 DMFT counts from three million observations of children aged 6 to 16 in East Germany shows a strong time-lagged relationship with fluoridation activities. More significant results, however, are found due to age and a score of socioeconomic factors. Additionally, caries prevalence decreases with time. Conversely, DMFT is not affected by size or type of dental staff, neither absolutely nor relatively.

[Neurovascular compression of the optic nerve as a cause of progressive visual dysfunctions and decompression treatment].

On the basis of two cases, the authors show that the optic nerve and the chiasma may be compressed due to vascular anomalies at the skull base. The symptoms are usually misinterpreted as opticochiasmatic arachnopathy or optic neuritis with severe progressive chiasma syndrome. If the condition is diagnosed early and treated by neurosurgical decompression, the prognosis may be satisfactory.

[Experimental transvitreal fibrin gluing of the retina].

The authors report on the transvitreal application of fibrin glue in retinal detachment surgery. Methods of application and the course of postoperative resorption are illustrated with reference to experimental studies on rabbit eyes. The prospects for this new method of creating chorioretinal adhesions are pointed out and discussed.

[Radiotherapy in (localized) non-Hodgkin's lymphoma of the orbit].

Orbital NHLs are rare and manifest themselves primarily in localized stages. The value of radiotherapy is discussed, reviewing retrospectively the treatment results obtained in 41 patients irradiated for orbital NHL between 1958 and 1966. All were histologically verified. There were 15 lymphomas of low and nine of high malignancy according to the Kiel classification. Among the 17 remaining patients, 15 cases had been classified as reticulum cell sarcoma and two as lymphosarcoma. Fourteen cases were in stage IE, 19 in stage IIE, and eight in stage IIIE/IVE. After surgical biopsy (n = 25) or resection (n = 5), megavoltage irradiation was delivered in conventional fractionation from 20 Gy up to a total dose of 50 Gy. During the last decade treatment has been based on CT scans and corresponding computer assisted treatment planning. Clinically, complete remission was achieved in 13 out of 14 patients (93%) with stage IE, in 17 out of 19 patients (90%) with stage IIE, and in five out of eight patients (63%) with stage IIIE/IVE. The nonrecurrence rate is 65% (10 out of 14) for stage IE and 75% (13 out of 17) for stage IIE after a mean follow-up time of 36 months. As regards lymphomas of low and high malignancy, the nonrecurrence rate is 89% and 43% respectively. Recurrence (n = 8) and dissemination (n = 6) occurred only twice more than two years after radiotherapy had been discontinued. The incidence of side effects was low. Two patients developed a "dry eye." In two eyes the lens had to be removed because of progressive cataract. Radiotherapy with individual treatment planning based on CT scans is the curative treatment of choice fo localized orbital NHL.

[Occurrence of retinal complications by reproliferation following vitreoretinal silicone surgery].

Clinical follow-ups of 235 patients following vitrectomy and silicone oil injection were evaluated with special regard to the occurrence and time course of reproliferations. The total incidence of reproliferations was 33%. The highest rate was found after ocular injuries (47%), followed by rhegmatogenous retinal detachment (30%) and proliferative diabetic retinopathy (23%). In all three groups the incidence of reproliferation was highest during the first three postoperative months. In cases with ocular injuries and rhegmatogenous retinal detachment, the rate of reproliferation was found to be highest during the first postoperative month, while in cases of proliferative diabetic retinopathy the incidence peaked during the second and third months. Therefore, in order to prevent complications due to the silicone oil, the oil should be evacuated between the third and sixth postoperative month.

Evaluation of different approaches for identification of xenobiotic- degrading pseudomonads.

Different approaches were evaluated to identify aerobic, gram-negative, biodegradative isolates assumed to be pseudomonads. Quinone and polyamine analysis allowed rapid identification to the genus level, i.e., allocation of those isolates belonging to the Pseudomonas fluorescens complex which represents the phylogenetically defined core of the heterogeneous genus Pseudomonas. Subsequent studies concentrated only on these true pseudomonads. The multiple-test system API 20NE, determination of the moles percent G+C content, and polyacrylamide gel electrophoresis of soluble proteins aided in identification on the species level. Polyacrylamide gel electrophoresis of both soluble proteins and whole-cell lipopolysaccharides allowed recognition of identical strains and double isolates, which were confirmed by DNA-DNA hybridization.

Decompression of the orbit and the optic nerve in different diseases.

During the past six years, visual loss in 60 patients was caused by infectious processes and mucoceles of the ethmoid region, benign paranasal tumours, endocrine ophthalmopathies, head trauma etc. Comparison of preoperative and postoperative visual function in 48 cases treated by decompression of the orbit and in 12 cases treated by decompression of the optic nerve demonstrated visual improvement of 68 percent on average. Surgically, we preferred the transethmoidal route and in some cases the lateral osteoplastic approach. With regard to the success of the surgical therapy, a correlation between the time of manifestation of the visual loss and the time of surgery was found. The results mainly depended on the underlying diseases.
