Inherited pyruvate kinase deficiency and normal haematologic values in Australian Basenji dogs.

We have carried out a survey to determine the prevalence of carriers of pyruvate kinase deficiency haemolytic anaemia in Basenjis in Victoria. Of 186 dogs tested, 20 were found to be carriers for pyruvate kinase deficiency, a prevalence similar to that reported overseas. Of the 20 carriers found, 17 were born in Australia and were traced directly to 1 of the 3 imported carriers. Despite the relatively high frequency of carriers, no mating of 2 carriers has yet been detected and no dogs with haemolytic anaemia have been observed. The inheritance of the pyruvate kinase deficiency has been traced through up to 5 generations of dogs in Australia and is consistent with co-dominance for pyruvate kinase activity and with an autosomal recessive gene for haemolytic anemia.

Hand infections.

Superficial infections include paronychia, cellulitis and lymphangitis. Grave infections include felon, purulent tenosynovitis, thenar infections, septic arthritis and human bites. Treatment by incision and drainage, moist heat, elevation, splinting and specific antibiotics or other agents applies to most hand infections. When pus is found, adequate drainage usually gives good results. Without drainage, pressure necrosis and gangrene often follow. Rest for the infected part and for the patient is as important as any other facet of treatment.

The immunizing effect of influenza A/New Jersey/76 (Hsw1N1) virus vaccine administered intradermally and intramuscularly to adults.

2The immunogenicity and reactogenicity of inactivated influenza A/New Jersey/76 (Hsw1N1) whole-virus vaccine administered intradermally (40 chick cell-agglutinating units/0.1-ml dose) and intramuscularly (im; 200 chick cell-agglutinating units/0.5-ml dose) to human adults were evaluated. Among 18-24-year-old persons initially free of detectable antibody, intradermal vaccination induced lower titers of hemagglutination-inhibiting antibodies than did im vaccination, and a sequence of intradermal and im vaccinations did not offer any serologic advantage over one im dose. In contrast, persons over the age of 24 who initially lacked detectable antibody had as good a serologic response to intradermal vaccination as to im vaccination. Among individuals who had antibody before vaccination, immunization by either route induced greater increases in titers of hemagglutination-inhibiting antibody than occurred in initially antibody-negative persons, even when the latter were given two doses of vaccine. Somewhat fewer systemic reactions occurred after intradermal than after im vaccination, but the intradermal route was associated with local reactions in almost all vaccinees and with some residual pigmentary changes. These results suggest that intradermal vaccination should be used only in very selected circumstances.

Transiently reduced activity of carbamyl phosphate synthetase and ornithine transcarbamylase in liver of children with Reye's syndrome.

Since Reye's syndrome is associated with hyperammonemia, we measured the urea-cycle enzymes in hepatic tissue of 13 patients. Expressed as nanomoles of citrulline per milligram of hepatic protein per minute, mean activity of carbamyl phosphate synthetase (6.27 +/- 2.45 S.D.) and ornithine transcarbamylase (136.19 +/- 41.83) in Reye's syndrome was reduced significantly (P less than 0.005) when compared with that of 25 "normal" controls (11.54 +/- 4.24 and 307.49 +/- 94.15, respectively). Activity was maximally reduced during the first days of clinical symptoms; it returned toward normal during the following week regardless of whether the disease ended in death or recovery. The activity of the two enzymes was normal in patients with salicylate intoxication or heritable argininosuccinic acid synthetase deficiency. The apparent Km of hepatic ornithine transcarbamylase for ornithine was in the normal range in patients with Reye's syndrome (mean 0.24 mM). These observations indicate that Reye's syndrome is associated with acquired and transient dysfunction of hepatic mitochondrial urea-cycle enzymes.

The stability of short ragweed pollen extract as measured by skin test and antigen E.

A short ragweed pollen extract stored at 4 degrees C was shown to retain its skin reactivity for 18 mo, although 60% of the antigen E was lost. When stored at higher temperatures, the extracts had a markedly reduced skin reactivity for some individuals, but retained full reactivity in others even though the antigen E was markedly diminished. Thus, some ragweed-sensitive individuals are very sensitive to antigen E, which is heat-labile, but some have a major sensitivity to other allergens, which are heat-stable. During the 18-mo course of this study, skin test reactions to a similar extract did not show significant variability.

Prolonged interval hyposensitization therapy in hymenoptera-sensitive patients.

The Committe on Insects of the American Academy of Allergy has conducted a 10-year study of 397 patients with Hymenoptera allergy treated with glycerosaline or aqueous whole body mixed Hymenoptera extracts at intervals of 2 or 3 months for the maintenance doses. Patients had few reactions, although in some cases local reactions presented lengthening of the interval. The clinical response was similar to that from conventional therapy using the 4-wk interval for maintenance doses. This type of therapy offers the added advantages of fewer patient visits and decreased cost.

Prognostic implications of left ventricular function in endomyocardial disease in infants and children.

Hemodynamic factors measured at cardiac catheterization in 40 infants and children with chronic endomyocardial disease were analyzed in regard to subsequent clinical condition. The patients were followed for periods ranging from 2 months to 11 years (average 4.3 years) after initial cardiac catheterization. There were no statistically significant differences in left ventricular end-diastolic volume among survivors with no symptoms, survivors with persistent congestive heart failure, and nonsurvivors. Ejection fractions were depressed in about four fifths of patients with chronic endomyocardial disease and were significantly lower in the group of patients who died subsequently. LVEDP in patients who did not survive was significantly higher than in asymptomatic survivors, but there was too much overlapping of individual values to be of prognostic value. Left ventricular pressure-volume loops appeared to offer an improved, although more tedious, method of assessing LV function. In addition to offering information on LVEDV, LVEDP, and EjF, LV stroke work may easily be estimated from pressure-volume loops. There was generally good correlation between hemodynamic status assessed from pressure-volume loops and subsequent clinical outcome.

Interhemispheric subdural empyema: angiographic diagnosis and surgical treatment.

Interhemispheric subdural empyema, secondary to frontal sinusitis in two girls is described. Headache, hemiparesis more marked in the lower extremity, fever, focal seizures, stupor and stiff neck were the principal features of the clinical course. The angiographic appearance of the lesion was the key to the preoperative diagnosis. Surgical evacuation of the purulent collection resulted in complete cure in both cases.
