The present prospective longitudinal study describes and explores the impact of demographic change and political reforms on the growing risk of suffering from mental disorders and subsequent hospitalization. The statistical analysis is based on all insured individuals who were 50 years and older during the years 1993 to 2003 (n = 453 609) from a statutory sickness fund, the Gmünder Ersatzkasse. Event-history analyses show no linear but a rather u-shaped relationship between age and the risk to be given in-patient treatment for mental disorders. However, the age effect shrinks significantly when the number of previous hospital stays, the proximity to death, and the marital status are statistically controlled. On the other hand, the number of psychic disorders as a minor diagnosis has grown robustly after the legal requirement for fully documenting hospital cases according to section sign 301 SGB V and after the legal decision to introduce DRG refunding in 2000. Since the risk of in-hospital treatment of a psychic minor diagnosis increases most in somatic and not in psychiatric clinics, we suspect an up-grading effect.
Since more than 200 years, it has been well known that a marriage prolongs one's life. Current research distinguishes between protective and selective processes. However, it has been missed that today's marital biographies are not characterized by one marital status but through a sequence of different events and transitions. It is the aim of the present study to try to determine the effect of marital biographies and mortality. An East-West German comparison tests, if and how historical events influence this connection. The analysis is based on data from the German Socio-Economic Panel. It can be shown that the protective effect of being married cumulates over years. Furthermore, each change in marital status increases the risk of mortality. However, persons recover from negative events (divorce, widowhood) over time. In East Germany, despite of historical change in 1989/90, the impact of the marital biography is at least as significant as in West Germany.
A 31-year old male patient had suffered from systemic lupus erythematosus (SLE) for 21 years. During the last 8 years he exhibited no clinical symptoms and did not receive any medical SLE treatment. He was admitted with a two-day history of dyspnea and fever. Laboratory studies revealed microcytic anemia and elevated levels of inflammation markers. Chest X-ray showed pulmonary infiltrates. The respiratory status rapidly deteriorated and the patient died 16 hours after admission. An autopsy was performed and diffuse alveolar hemorrhage in all parts of the lungs were seen, which was confirmed by microscopic examination. In contrast, lung histology did not show evidence of infection or inflammatory lesions. Additionally, membranous glomerulonephritis could be identified by light and electron microscopy. Diffuse alveolar hemorrhage (DAH) and concomittant lupus nephritis as manifestations of the known SLE were diagnosed. Acute pulmonary hemorrhage was determined as the cause of death. DAH is a rare, but serious manifestation of SLE. The diagnosis is difficult since the occurrence is abrupt and both symptoms and histology of the lesion are non-uniform and unspecific. The present case demonstrates that DAH also develops in patients that have not had clinical symptoms for several years. Since the early diagnosis is essential for the outcome, DAH should be considered in every case of SLE patients with severe pulmonary symptoms. Corticosteroids are the recommended form of therapy for this disorder.
OBJECTIVES: Detection of disease activity in Crohn's disease (CD) is of crucial importance for diagnosis and management of the disease. Noninvasive methods for monitoring are desirable and comprise hydromagnetic resonance imaging (hydro-MRI) and leukocyte scintigraphy. In addition, a recent case report indicated the potential of 18F-fluorodeoxyglucose positron emission tomography (FDG-PET) to assess CD activity. However, comparative prospective studies are lacking. METHODS: Between February, 1999 and August, 2000, 59 patients with CD were enrolled in a prospective study to assess disease activity by FDG-PET, hydro-MRI, and immunoscintigraphy with anti-nonspecific cross-reacting antigen 95 antigranulocyte antibodies. In 28 of these patients, colonoscopy could be performed. Twelve patients with irritable bowel syndrome and 20 tumor patients without gut inflammation served as controls. Results were compared by statistical analysis. RESULTS: FDG-PET detected 127 pathological findings (average maximum standardized uptake value = 4.4 +/- 1.1) in the terminal/neoterminal ileum (37), small bowel (24), and colon (66) of 54 patients with CD, whereas no pathological findings were seen in five patients with CD, the control patients with irritable bowel syndrome, and the tumor patients without gut inflammation. In contrast, examination with hydro-MRI or granulocyte antibodies detected less pathological findings in CD patients. Forty-five of the detected foci were accessible to endoscopic verification. The correlation of the foci with endoscopic findings showed a high specificity (>89%) of all three methods to detect inflamed areas in the terminal ileum and colon of patients with CD, although analyses by hydro-MRI and granulocyte antibody scan had strikingly lower sensitivities (40.9% and 66.7%) than FDG-PET analysis (85.4%). CONCLUSIONS: FDG-PET appears to be a reliable noninvasive tool for simultaneous detection of inflamed areas in the small and large bowel of patients with CD. FDG-PET can be used to detect disease activity in the terminal ileum and colon of CD patients with high sensitivity and specificity.
The present report discusses the monitoring system for approximately 340 facilities for water management of the Emschergenossenschaft and Lippeverband. It is based on the operation control system of the wastewater treatment plants (currently 61) and the pumping stations of the Lippeverband, which has been created since 1985. For the near future, the integration of the plants of the Emschergenossenschaft including storm-water tanks is scheduled. The current connection ratios of the WWTPs which are to be monitored vary between 500 and 1,940,000 Population Equivalents. The data recorded at each plant are transmitted to the allocated catchment area centre and then to the district centres. The encompassing monitoring centre serves the integrated control of all plants. The comprehensive remote control is managed through the presentation of relevant plant-type-specific system parameters. The investment costs of approximately 33,000 Euros/plant and annual costs of 9,400 Euros/plant stand against a considerable increase of the operation safety and the reduction of manpower.
BACKGROUND: Wegener's granulomatosis (WG) is characterized by systemic vasculitis with crescentic glomerulonephritis (CGN) and circulating autoantibodies directed against neutrophil cytoplasmic antigens (ANCA). Proteinase 3 (PR-3), a neutral serine proteinase in neutrophils implicated in the growth control of myeloid cells, has been identified as the target antigen for ANCA in WG. Since the kidneys are frequently involved in WG, we studied the in situ expression of PR-3 by renal parenchymal cells. METHODS: We assessed the expression of PR-3 in kidney biopsies of 15 patients with WG by immunohistochemistry (IHC) and in situ hybridization (ISH). Normal kidney tissue served as the control. RESULTS: We detected PR-3 mRNA and PR-3 protein in distal tubular epithelial cells (TECs) and glomerular epithelial cells (GECs) in normal kidney tissue and in CGN. Furthermore, a strong glomerular PR-3mRNA expression restricted to the site of cellular crescents was detected in patients with WG. The analysis of 144 glomeruli with cellular or sclerotic crescents revealed a positive correlation of glomerular PR-3mRNA expression with the percentage of cellular crescents per glomerulus. The capability of human TECs and GECs to synthesize PR-3 was confirmed by Northern blot and ISH on cultured cells. CONCLUSION: These data provide evidence that nonhematopoetic renal parenchymal cells express PR-3 and that glomerular expression of PR-3 is associated with crescent formation in WG. Our findings suggest that renal parenchymal cells may directly be involved in the pathogenesis of CGN in WG.
A recently developed electrophoretic instrumentation based on a real-time image processing system has been applied to electrophoretic fingerprinting and multiparameter analysis of cells and other particles. The comparison between theoretical and experimental electrophoretic fingerprints, completed by the analysis of differences between measured fingerprints, offers a new methodology for better understanding and controlling of the processes at solid/liquid interfaces. Moreover, the multi-parameter analysis including electrophoretic mobility, size, density and shape of cells can complete the diagnosis and prognosis of diseases.
FSH plays a central role in normal reproductive function, i.e. control of follicular maturation in the female and initiation and maintenance of spermatogenesis in the male. The effects of FSH are mediated by its interaction with a specific receptor that belongs to the superfamily of guanine nucleotide-binding protein-coupled receptors. Due to the microheterogeneity of gonadotropins, measurement of immunoreactivity does not necessarily reflect their bioactivity. Mutations in gonadotropin beta-subunits, which affect bioreactivity and/or immunoreactivity of gonadotropins, have been described as causes of infertility, thus highlighting the need for rapid and convenient methods to measure bioactivity. To establish a model system for recombinant in vitro bioassays for FSH that would obviate the use of live animals, we developed a strategy for efficient expression of the rat FSH receptor (FSHR) in L cells. A cell line, FSHR 7/12, was developed that bound [125I]FSH with high affinity (Kd 1.42 nM) and responded to human FSH with an increase in cAMP accumulation. Untreated human serum was found to have an unspecific inhibitory effect on cAMP formation. This effect could be thoroughly avoided by mild heating (10 min at 56 C) of serum samples before addition to cells without detectable loss of FSH immunoactivity or bioactivity. Studies on the hormone-sensitive adenylyl cyclase system of transformed FSHR 7/12 cells and of the parental Ltk- cells showed that the cellular response to FSH was highly specific. Using a parallel line assay design, FSHR 7/12 cells were used to validate a novel recombinant in vitro bioassay relying on intracellular cAMP accumulation as a readout system. Up to 10% of serum could be added to the incubation buffer without leading to nonparallelism to the standard curve. When 70 serum samples of male patients attending an infertility clinic were analyzed, the novel assay system displayed high sensitivity and a close correlation (r > 0.8; P < 0.01) to the established rat Sertoli cell aromatase bioassay and to a highly specific fluoroimmunoassay. When sera of 25 normal menstruating women were analyzed for FSH bioactivity at different stages of the menstrual cycle, a midcycle FSH peak followed by a decline in the late luteal phase could be discerned. The analysis of 26 serum samples of postmenopausal women revealed a close correlation between FSH values obtained by the novel in vitro bioassay and by a fluoroimmunoassay (r = 0.90; P < 0.01). Thus, the present in vitro bioassay represents a sensitive, rapid, and convenient model system to measure bioactive FSH in human serum.
A new bacteriochlorophyll has been isolated by thin layer chromatography from all strains of the brown-colored Chlorobiaceae Chlorobium phaeobacteroides and Chlorobium phaeovibriodes. The new bacteriochlorophylle--like the bacteriochlorophylls c and d--represents the major amounts of bacteriochlorophyll a. Bacteriochlorophyll e can be differentiated from the bacteriochlorophylls c and d by its absorption maxima in aceton and its different Rf-value in the thin layer chromatogram. The structure of the new bacteriochlorophyll e has been elucidated on the basis of mass spectra, 1H-and 13C-NMR-spectra, the UV/VIS-spectrum as well as IR-, ORD-, and CD-spectra. The new bacteriochlorophyll has the same relationship to bacteriochlorophyll c as chlorophyll b from green plants to chlorophyll a; therefore, bacteriochlorophyll e represents the first formyl-substituted chlorophyll from bacteria. Similar to the bacteriochlorophylls c and d, the new bacteriochlorophyll e consists of a mixture of at least three homologues which differ from each other by different substituents on the pyrrol rings II and III.

Chromic acid degradation of natural (-)-stercobilin (1) yields 2(R)-methyl-3(R)-ethylsuccinimide (+2), whereby the absolute configuration of 1 at the chiral centers C-1, C-2, C-7, and C-8 is established. The substituted oxo-tetrahydrodipyrromethane precursor, 5, for the total synthesis of (-)-stercobilins 3 and 4, in which the relative configuration between the asymmetric centers is known, yields 2(S)-methyl-3(S)-ethylsuccinimide (-2) under the same conditions of degradation. Nuclear magnetic resonance studies of 1 and 3 show that in 1 the hydrogen atoms at C-2 and C-2', as well as those at C-7 and C-7', are trans relative to one another. Accordingly, natural (-)-stercobilin possesses the 2'(S), 7'(S) configuration, and has the configuration formula 6(1 (R), 2(R), 2'(S), 7'(S), 7(R), 8(R)). These results, coupled with those of earlier studies, also establish the absolute configuration of the (+)-urobilin 7 and of the phycobilin 8 at C-7'.
