[Comparative studies with scanning electron microscope on the quality of corneal disk incision using various corneal trephine (author's transl)].

The cut surface of corneal stroma from rabbits was studied with a scanning-electron microscope and compared after using a manual trephine, a motor-driven rotor trephine and a rotor trephine with mechanical guidance. It became evident that the cut surface obtained with the motor-driven trephine is clearly smoother than that obtained with manual trephine. A further improvement in the quality of the cut is evident with a mechanically guided trephine.

[The influence of local anesthetics on corneal epithelium. A scanning electron microscopic study (author's transl)].

The effect of different local anesthetics (Cocain 4%, Lidocaine 2%, Proparacain) on the corneal epithelium in rabbits was examined under scanning electron microscope. The experiment was divided into three groups. Group 1 received one application of two drops of the given local anesthetic for a reaction time of 5 minutes. Group 2 received two drops of the given anesthetic after 0, 5 and 10 minutes. The cornea was excised after 15 minutes. Group 3 were measured after a single application of Proparacain using a Schiötz or hand applanation tonometer according to Draeger. After a single dose of a local anesthetic principally the same changes in the surface of the cornea were observed with all the preparations used: a distinct decrease in the number of microvilli and microplicae, disruption of the intercellular spaces and the prominence of the cell nuclei which under normal conditions are not visible. After several applications the greater toxicity of Cocain compared with the other preparations was clearly seen through the disruption of the plasma membrane and the cytoplasm. The damage effected several layers of cells. Tonometry when correctly performed causes no additional damage to the cell surface.- The effect of local anesthetics on the cell membrane can only take place after the disruption of the tear film. The results emphasize that local anesthetics should only be applied when absolutely essential.

[Use of an automatic laboratory rinsing machine for cleaning and disinfection of ophthalmosurgical instruments (author's transl)].

A report is given on the use of an automatic rinsing machine for the cleaning of surgical eye instruments combining the processes of disinfection, cleaning and drying in one operation and thus providing good protection against corrosion and optimal protective treatment of the cutting blades. To allow use in ophthalmology, a special insert was constructed.

[Choroid neurilemmoma. Considerations for clinical differential diagnostics].

A report is given on an extremely rare case of neurilemmoma of the choroid and observations are made on the difficult differential diagnosis of malignant melanoma. Clinical diagnosis and treatment apart from enucleation, was impossible. The recognition of the nature of this tumor could be made only on histological examination.

[Diabetes mellitus--not an aetiological factor of uveitis? (author's transl)].

Out of 103 patients with uveitis, 14.5% of the cases showed pre-clinical diabetes mellitus. This number exceeds by far the average frequency of approx. 6% of pre-clinical diabetes. On account of these findings, the diabetes mellitus is assumed to be a co-factor of the development of uveitis. In order to clarify the cause of uveitis, the glucose tolerancetest should be given preference in the diagnosis.

[Basaliom of the eyelids (author's transl)].

From 1959-1973 128 patients with tumors of the eyelids were treated at the "Augenklinik im Krankenhaus Nordstadt Hannover"; in 103 cases the diagnosis of basal cell carcinoma was histologically proved. With 62.1% of male patients prevailed the female patients with 37.9%. The age of the patients, localisation of the tumors, previous treatment and duration of involvement are discussed in detail. With the exception of 3 cases, which had a primary or secondary radiation they were surgically treated. A short explanation of the technics is given. In 82 out of 101 surgically treated cases (81%) the patients were clinically healed by a single operative treatment. During our observation there was no recurrence. Together with the number of patients, who were treated for several time, the over-all-cure-rate was 98.2%. Complications of surgical treatment are described and compared with those of other types of therapy. According to our results and as mentioned in the literature basal cell carcinomas of the peri-orbital region should be treated only by ophthalmologists. Lifelong control is necessary because of the extraordinary character of the tumor, which shows many exceptions in regard to basal cell carcinomas on other parts of the body.

[Lid basaliomas with malignant courses (author's transl)].

3 cases of lid basalioma with malignant course are described - in the first case with destruction of the eye-lids, in the second destruction of the orbit and nearby sinuses, and in the third with infiltration of the brain. Hence the necessity of early active treatment is shown. The diagnosis of the tumor and the degree of infiltration into healthy tissue must be made histologically - this is as important as the life-long follow-up of tumor patients.
