[Internal medicine aspects in patients with operable carotid lesions].

Cerebral insufficiency with or without neurological deficits (n.d.) may be treated effectively surgically by carotid artery reconstruction or by extra-intracranial anastomoses. The low mortality of these operations and the favourable results including prevention of new n.d. and amelioration of long standing n.d. suggest consideration of operation in geriatric patients. Noninvasive screening methods for cranial artery pathology such as neck auscultation, direct and indirect Doppler ultrasonic investigations and dynamic and static cerebral scan therefore should be performed not only in all patients with overt cerebrovascular insufficiency whatever degree but also in patients with hypertension, diabetes mellitus, ischemic heart disease and occlusive disease of the extremities on account of the high incidence of multivascular disease in these patients. We compared 60 consecutive patients before and beyond the age of 70 respectively, treated surgically for cerebrovascular insufficiency. The only significant differences apart from age were a higher rate of pathologic findings of the heart on x-ray and on the ecg in the aged. Despite this mortality, major complications and favourable results were not different for both age groups. These results favour an active approach to diagnosis and indication to surgical treatment not only in young patients but also in the symptomatic geriatric patient.

Thyroglossal duct remnant carcinoma.

Thyroglossal duct remnant carcinoma is a very rare malignancy arising from aberrant thyroid tissue. This diagnosis, however, should be considered in patients presenting with anterior neck masses. If there is no invasion of the capsule of the tumor, local excision with diligent follow-up is recommended as the treatment of choice.

[Clinical course and therapy of carotid stenosis in old age].

From 1977 to 1981 266 patients suffering from partial carotid occlusive diseases were operated, 50% of them older than 65 years, the eldest 87 years. The prognosis is the same in both groups, the good results range higher than 80%, mortality was even lower than in younger patients. Local anesthesia is preferred, it allows an optimal observation and earliest detection of operative mistakes and neurological deficits. The incidence of contralateral involvement is 33%. 25% of male patients needed preceding contralateral EIA-operation because of a missed complete thrombosis. Furthermore the importance of kinking is stressed, because its reconstruction without arteriotomy and risk was possible in nearly all cases. Despite all advantages in vascular surgery, cerebral symptoms always call for investigation of the brain and not for vascular tests only: the amount of cerebral operations is still five times higher than that of carotid disorders and 500 times higher than procedures on the aortic arch.

[Investigations to gastric tolerance of a sustained release theophylline formulation (author's transl)].

The measurement of the transmural gastric potential difference (PD) is a suitable method to quantify irritations of the human gastric mucosa caused by drugs. Purpose of this investigation was to check the gastric tolerance of Bronchoretard, a new developed "divided-dose" drug formulation of theophylline as retarded pellets in capsules in comparison to pure theophylline solution. After having checked the suitability of the procedure in respect of the obtained results, a study on 7 healthy male volunteers was considered to be justifiable, as the development of retarded theophylline preparations, which cause lower irritations in the stomach by slower release of pure theophylline, has a high therapeutic value. With the gastric PD model, developed at our institute following variables were calculated: the area under the baseline (AUB), the "Reizindex" (RI), the mean instability time (MIT) of the membrane and the maximum potential difference (Pdmax). The volunteers wee admitted to the ward of our institute 36 h before the first application. On 2 different days 350 mg theophylline, dissolved in 50 ml physiological saline as well as 1 capsule Bronchoretard containing 350 mg theophylline, were administered Cross over according to a randomisation plan and the deviation of the PD was measured as a function of time. The study led to the following results: after administration of Bronchoretard the AUB is statistically significant smaller than after administration of theophylline solution (P less than 0.05). After administration of Bronchoretard the RI is statistically significant smaller than after administration of theophylline solution (p less than 0.05). After administration of Bronchoretard the mean instability time is statistically significant smaller than after administration of theophylline solution (p less than 0.05). After administration of Bronchoretard Pdmax is smaller in trend than theophylline solution (p greater than 0.05). The oral administration of Bronchoretard leads to considerably lower irritation than the instillation of the same dose of theophylline in solution.

Medullary carcinoma of the thyroid.

Three patients with medullary carcinoma of the thyroid gland, one sporadic and two familial, are described. One of the patients presented at a late stage of the disease with evidence of wide dissemination and severe diarrhea, which was controlled by indomethacin. The value of venous catheterization and plasma thyrocalcitonin measurements in evaluating the extent of the disease was demonstrated in this case.

Selective splenectomy in Hodgkin's disease, stages I and II. Results of treatment.

Sixty-three patients with Hodgkin's disease, in stages I or II, asymptomatic (A) or symptomatic (B), were diagnosed and followed at the Chaim Sheba Medical Center from 1969 to 1976. Only 14 were staged pathologically. Until 1971, the patients received mantle or "inverted Y" therapy only; thereafter, an extended field that included mantle, upper abdomen and spleen irradiation was given. Symptomatic patients, as well as patients with extranodal involvement, received MOPP chemotherapy (nitrogen mustard, vincristine, procarbazine and prednisone) after termination of radiotherapy. Of 51 patients who were in stage IA or IIA, six relapsed 20 to 43 months after irradiation. Three had a pelvic recurrence; two of them were surgically staged. Thus, in only 1 of 51 patients could staging laparotomy possibly have detected pelvic disease and resulted in different therapy. Our results suggest that total nodal irradiation and staging laparotomy are not mandatory in stages IA and IIA of Hodgkin's disease. The group of 12 symptomatic patients is too small to allow us to draw definite conclusions as to the role of staging laparotomy and adjuvant chemotherapy. However, in view of the high relapse rate in the upstaged symptomatic patients, it seems that chemotherapy should be given to these patients.

[A case of reversible deafness in cystic arachnoiditis (author's transl)].

A patient studied preoperatively by a neuro-otological test battery showed signs of an extensive cerebellopontine angle tumour. During the operation dense fibrotic tissue was found extending from the choroid plexus to the right acoustic nerve, which was freed and thus decompressed. Postoperatively, low frequency hearing improved markedly in the right ear. In contrast to the well-known documented cases of successful decompression of the optic nerve it has not been possible up to now to achieve a successful outcome in similar procedures on the acoustic nerve. In the described case the fibrotic lesion had obviously just caused reversible damage to the myelin sheaths of the eighth nerve, which can be interpreted as "neurapraxia" (Seddon, 1943). Further investigations in regard to the systematic elucidation of possibly reversible damage to the acoustic nerve are currently being performed at the IInd ENT department of Vienna University.

Childhood Hodgkin's disease in Israel. A study of 17 cases.

A study of 17 children suffering from Hodgkin's disease (H.D.) is presented. The special ethnic distribution with predominance in Arab and Oriental Jewish children is noted. A high incidence of mixed cellularity histology in Arab children is emphasized. A conservative approach to therapy, i.e. extended field regional radiotherapy only, resulted in a long first complete remission period in lymphocyte predominance and nodular sclerosis H.D. Stages IA and IIA. All children in Stage IIIB and IV were treated by six courses of combined chemotherapy, and were maintained either on chlorambucil and vinblastine or on MOPP. Patients with mixed cellularity H.D. Stage IIA and all patients in Stage IIIA were treated by four courses of MOPP after the completion of extended regional radiotherapy. The 5 children who did not receive the schedules treatment died. The remaining 12 are well, with a survival of 8 to 104 months.
