[A case of exostosis of the base of the acetabulum in a child with multiple exostoses].

The authors report a case of an exostosis of the base of the acetabulum seen at the age of four months in a routine radiograph of the pelvis in a child suffering from osteochondromatosis. The growth in size of this exostosis led to subluxation of the hip at the age of three years. Surgical removal resulted in reduction of the subluxation and restoration of joint stability three years after operation. An intra-acetabular site for an exostosis has rarely been described.

[Post-traumatic algodystrophy in children].

The authors report two cases of Reflex Sympathetic Dystrophy (RSD) significant by their releasing factor: initial trauma followed by surgical act. They describe the particularities of RSD in children compared with RSD in adult: differents localizations, X Ray films often normals or showing a local homogeneous osteoporosis, bone scintigraphy with variable result (increased or normal or less uptake of isotope), more rapidly favorable evolution.

[Traumatic rupture of the aortic isthmus in a child, revealed by paraplegia].

After a severe polytraumatism, a 14 year-old boy presents paraplegia without vertebral lesion. Angiocardiography showed rupture of aortic isthmus. Surgical treatment resulted in successful outcome. The authors insist on the rarity of traumatic rupture of aorta in children and or the particularity of the diagnosis revealed by paraplegia.

[Fractures and slipped epiphyses of the proximal humerus in children. Place and methods of surgical treatment].

Fractures and Epiphyseal separations of the proximal humerus are usually non-operatively treated with a generally good result. However, sometimes, a surgical treatment may be required. Twenty fractures of the proximal humerus occurring in children were treated operatively. From these 20 cases, the authors precise the indications and the ways of surgical treatment in this type of lesion. They insist upon the value of the angulation on the lateral radiograph of the shoulder which, if important, may often signify an interposition of the tendon of the long head of the biceps between the fracture fragments causing irreductibility; they propose a classification of the fractures referring to this angle. Considering surgical procedure, delto-pectoral approach is the most commonly used, although axillary way is more esthaetic. Kirschner wires and screws are the best means of synthesis. Anyway, this surgical treatment has to remain exceptional.

[Polydactyly].

The authors propose an original classification of the polydactyly. They opposed: polydactyly "symptom" and polydactyly "disease". Discussion about five personal pedigrees.

[Serious pelvic traumatisms in children].

The rareness of pelvic fractures in children is due to the resilience of bony tissue and to the thickness of the periosteum at this age. Therefore, such fractures are most often seen after violent traumatisms, which explains their frequent association with visceral lesions that determine the gravity and dominate the prognosis. The authors have presented 9 observations of fractures of the pelvis, with or without pelvis dislocation, which were the most often associated with urological lesions either acute (urethral rupture, bladder rupture) neither chronic (hydronephrosis) but sometimes with vasculo-nervous, gynecological or others lesions. The different therapeutic techniques and their indications in the treatment of such fractures are similar to those used in the adult. Excepting those cases of polytraumatisms, where mortality remains elevated, the treatment of such fractures is most often satisfactory in the short term, but requires a thorough and prolonged monitoring in children presenting associated urological and gynecological lesions in order to evaluate with precision the long term prognosis.

[Traumatic displacement of the upper femoral epiphysis (author's transl)].

Three cases of traumatic displacement of the upper femoral epiphysis in young children are presented. The authors emphasized that these lesions are very rare (about fifty cases in the literature) and that the incidence of major complications including avascular necrosis of the proximal fragment, coxa magna and premature epiphyseal fusion is very high. However, an early orthopedic management including soft reduction with X-ray control and fixation by percutaneous kirschner pinning give the best chance for these complications not to occur. Two of the three described in this paper can be considered as a good result after a course of four years. The third case including traumatic separation through the epiphyseal plate with dislocation of the hip and fracture of the femoral head needed a surgical ablation with poor result.

[Skin lesions and joint involvement in purpura fulminans (author's transl)].

Until recently, purpura fulminans had a very prognosis. It can now be treated and the patient may survive but extensive skin necrosis may lead to septic arthritis. The authors have observed 4 cases of this disease. 3 had to suffer amputation of a lower limb. They describe the type of care recommended in order to limit the extent of the necrosis.

[Essential deformans osteolysis. Report of a case with progressive and generalized evolution (author's transl)].

The authors report a clinical and radiographic course of a 16 years old caucasian girl with a progressive deformation of her skeleton. The primary lesions occur in knees and wrists. This disease was at first considered as a rheumatoid arthritis, then as a dysplasia epiphysialis multiplex, and now as essential deformans osteolysis (with carpal lysis, shortening of the forearm bones, dislocation of the elbows, disparition of the humeral and femoral heads, contracture of hips and knees, posterior tarsal lysis, and kypho-scoliosis). The clinical (particularly ophtalmologic), biological (including inflammatory, phospho-calcic and nephrologic evaluation with mucopolysaccharidosis urinary excretion) and histological (skin, muscle and bone) check-up were normal. They review the different classification established on lesion topography, on association or not with a nephropathy, finally on an dominant or recessive autosomal inheritance. Then the authors think that their case is similar to the ones of Winchester and Hollister. They discuss the etiopathogenic factors, and do not consider their case as a new mucopolysaccharidosis, but rather as a generalized disease of the collagen of bones.

[The place of lavage-irrigation of the peritonium in peritonitis from the appendix in children. A report of 10 cases (author's transl)].

Confronted with the difficult problems presented in acute appendicular peritonitis, whether generalized or multiple focused, the authors used the "lavage irrigation" method in ten cases. They analysed the local and general conditions that led them to employ this method, in light of the evolution of this technic which they had observed in their experiences whit it. They have attempted to elucidate, using information previously available in the literature, the indications, the steps, and the eventual complications of the "lavage irrigation" of the peritonemum in the child.
