[Relation of the biliary tract to other upper abdominal organs].

Anatomically and functionally the biliary ducts belong to the duodenal connection system and in their disease they adequately influence liver, pancreas, duodenum, stomach and oesophagus, as they on the other hand not infrequently are affected by diseases of these organs. Particularly frequently the coincidence of diseases of the biliary duct and the liver and of diseases of biliary ducts and pancreas, respectively, are observed. But interesting results may also be expected by the clarification of the relations between cholelithiasis and condition after cholecystectomy, respectively, to the frequency of reflux of bile into the stomach and/or oesophagus and the development of stomach and oesophagus.

[Carcinoma of the gastric stump in routine gastroscopies].

205-patients after partial resection of the stomach for benign gastroduodenal ulcers (104 patients for gastric ulcers, 81 patients for duodenal ulcers, 14 patients for combined ulcers) were examined gastroscopically from 1970--1976. The operation was performed more than 5 years ago. 35 carcinomas of the gastric stump could be found (17%). The operation was done in 695 of all cases more than 20 years ago and the male/female relation was estimated to be 6:1. --In 170 other patients with resection of the stomach (61 patients for primary gastric carcinoma, 103 for benign ulcers and 6 for other reasons) with a postoperative interval fewer than 5 years, 10 relapse of carcinoma and 4 carcinomas of the stump were observed. These 4 carcinomas of the stump must be seen as relapses of an carcinoma primarily diagnosed as an ulcer. Gastroscopic-bioptic examinations of patients with an operation performed more than 10 years ago for benign lesions should be done every year. If the cause of operation had been a carcinoma than from the first year after operation.

[Value and results of coloscopic polypectomy].

From August 1976 till the end of February 1979 158 polyp in 83 patients (48 male and 35 female) were removed by means of the rectocoloscopic polypectomy with the high frequency diathermy loop. 66% of the polypi had a diameter of less than 1 cm. Most the polypi were with about 70% in the sigma and in the rectum. With 60.7% the adenomas appeared most frequently, followed by the hyperplastic polypi in 20% and by the inflammatory ones in 13.3% of the cases. With the increase of size of the polypi the proportion of the adenomas clearly increased in comparison to the other kinds of polypi. Severe epithelial atypias (altogether 23.1%) were more frequently in adenomas larger than 2 cm than in smaller ones. An invasive carcinoma was found in no polypus. Thus the endoscopic polypectomy in every case meant a curative therapy with great economic use. In patients a haemorrhage appeared as a complication of the coloscopic polypectomy. Only the removal of the polypi in toto allows an exact histological classification and the answer of the decisive question, whether in one adenoma only severe epithelial atypias are present and thus the polypectomy is therapeutically sufficient or whether there exists an adenocarcinoma, for the removal of which an operation according to the principles of the carcinoma surgery must be carried out. The endoscopic ablation of the adenomas thus means a genuine prophylaxis of cancer.

[Emergency endoscopy].

The possibilities and advantages of the emergency endoscopy of the upper intestinal tract are emphasized. With the help of 475 own examinations from 1971 to 1973 and from 1973 to June 1977 the improvement of the diagnostic gain by instruments with prograde optics is proved. Ulcerous diseases of the stomach and the duodenum are found as most frequent cause of haemorrhage which appear in the oesophagus or in the oesophagogastric transition. With 20% the number of the multiple findings was significantly high, the proof of which is to be regarded as an essential advantage of the emergency endoscopy. But at the same time they also emphasize the necessity not to be satisfied with the proof of a source of haemorrhage. The coloscopic emergency endoscopy is more difficult. Endoscopico-therapeutic possiblities increase the importance of the emergency endoscopy.

[Toxic megacolon in Crohn's disease (author's transl)].

The toxic megacolon is well known in colitis ulcerosa, in colitis ulcerosa, in colitis granulomatosa--Crohn's disease--it is by far rarer. The authors observed two cases among their patients. The emergency operation consisted in applying an ileostoma and several colostomata. The definitive treatment with colectomy and ileorectostomy was done after weeks respectively months.

[The value of endoscopic retrograde cholangiopancreaticography in the diagnosis of bile duct and pancreatic diseases. Results of 756 studies].

Endoscopic retrograde cholangiopancreatography was successful in 626 of 756 examinations (82,8%). Choledocholithiasis in view of bile ducts and chronic pancreatitits on the score of bile ductal system are the most lesions we could observe. An acute pancreatitis in 2 cases and an infection of a pancreatic pseudocyst in 1 case were the 3 complications in our material. Meanwhile the rate of successful cannulations reached more than 90%.

[Crohn's disease--observations on own patient material].

On the basis of own clinical material, consisting of 32 patients with Crohn's disease the picture of this disease is discussed. Patients with an acute terminal ileitis in whom no development in the sense of a Crohn's disease took place were not included in the study. It is typical that also in the number of our own patients that the perferred terminal ileum alone or in connection with other parts of the intestinum as well as a high coefficient of relapses could be proved. Of the complaints cramplike pains in the abdomen were the most frequent symptom. Of the laboratory findings an increased blood sedimentation rate, a decreased iron level, an anaemia and a pathologicial Schilling test were foremost. In 2 patients during a short period a toxic megacolon developed. In the two patients it was the first shift of the disease. In another patient the relatively seldom affection of the duodenum and the stomach by Crohn's disease could be diagnosed.

[Arthropathies in intestinal diseases].

Together with gastroenterologists rheumatologists describe diagnostically significant disturbances of the supporting apparatus and the locomotor system which frequently occur in ulcerous colitis, enterocolitis regionalis Crohn, Whipple's disease and non-tropical sprue. Important conclusion for the diagnostic approach supplement the description.

[The so called early stomach carcinoma and its histological definition].

The histological findings of twenty bioptically diagnosed early cancers of the stomach are described. In the delimitation of the so-called borderline-lesions the intestinal metaplasia in the mucous membrane of the stomach must particularly be taken into consideration. The differentiated ripe form of the intestinal metaplasia is no precancerosis. However, within the process of intestinalisation atypias may appear, which represent possible transitional forms to carcinoma.

[Stomach ulcer in the emergency endoscopy].

After explanation of the diagnostic gain in acute gastrointestinal haemorrhages by use of the emergency endoscopy the results and experiences are reported got in 200 emergency examinations. As cause of haemorrhage most frequency were found peptic ulcers followed by gastric erosions. In one third the erosions were associated with other sources of haemorrhage and should be accepted as cause of haemorrhage only with cautiousness. Flat acute ulcerations cannot be diagnosed endoscopically already by relatively thin-layered linings and chronic ulcerations by plications.
