Increased biliary secretion of cysteinyl-leukotrienes in human bile duct obstruction.

BACKGROUND/AIMS: The pathophysiological role of leukotrienes in liver disease is not well understood. Redistribution or enhanced formation in cholestatic states may result in increased hepatic concentrations that are expected to contribute to liver injury. To disclose the potential role of cysteinyl-leukotrienes in chronic liver diseases, we studied biliary and urinary secretion in the model situation of relief of bile duct obstruction. METHODS: Concentrations of cysteinyl-leukotrienes were determined in bile and urine of patients with extrahepatic biliary obstruction in the course of therapeutic decompression by endoscopic or transhepatic techniques. Leukotrienes were measured by radioimmunoassay after HPLC separation. Concentrations of bile acids in bile and serum were measured for comparison. RESULTS: Bile collected 2 h after decompression contained high concentrations of leukotrienes (57.5 +/- 22 microM). Biliary secretion decreased over 24 h reaching equilibrium values after 48-72 h (2.8 +/- 1.7 mM and 6.4 +/- 6.6 microM, respectively). Total bile acid concentration in serum followed a similar time course. In contrast, biliary bile acid concentration showed high interindividual variations. Bile contained all leukotriene C4, D4, E4 and NAc-LTE4, but LTC4 was predominant. Urinary leukotriene secretion in cholestasis (199.7 pmol/mmol creatinine) was less than 7% of maximal biliary secretion. It further decreased to 116.4 pmol/mmol creatinine within 72 h. Urine also contained all species of cysteinyl-leukotrienes, but the relative amounts of LTE4 and NAc-LTE4 were higher than in bile. CONCLUSIONS: Formation of cysteinyl-leukotrienes is increased in obstructive jaundice resulting in increased urinary excretion before and both biliary and urinary excretion after relief of the obstruction. Predominance of LTC4 suggests that the secreted leukotrienes are newly formed. Increased synthesis and retention of hepatic cysteinyl-leukotrienes may contribute to hepatic and extrahepatic consequences of cholestasis.

Zinc concentration in serum and leucocytes in chronic inflammatory diseases.

In connection with inflammatory diseases of various origins an increasing amount of research has been directed towards trace elements, in particular zinc. However, proving changes in the zinc concentration of organisms or single cells still encounters major methodological difficulties. This is also probably the reason why many of the research findings to be found in current publications still significantly differ from one another. The present study utilizes a novel leucocyte-preparation method, which proved to be very reliable as regards the constancy of measured values. Thus, results obtained in 1989 which show a significant decrease in leucocyte zinc concentrations of patients with Crohn's disease were confirmed (1) as well as extended to the clinical pictures of Colitis ulcerosa and chronic pancreatitis of ethylic origin by studying a group of patients. Significant changes in serum zinc levels could not be proved. There was also no correlation between the leucocyte zinc concentrations and the serum zinc concentrations, on the one hand and the clinical parameters C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), body weight, humeral muscle area as well as activity of the disease, on the other.

Autoantibodies to Auerbach's plexus in achalasia.

Achalasia is a motor disorder of the oesopagus characterized by decrease in ganglion cell density in Auerbach's plexus. The cause of the lesion is unknown. This is to repeat on the occurrence of autoimmune phenomena in patients with achalasia, in particular circulating antibodies against Auerbach's plexus and its possible meaning. IgG-antibodies against Auerbach's plexus were determined by standard indirect immunofluorescence. Antibodies to the cytoplasm of Auerbach's plexus were found in 37 of 58 patients with achalasia at variable stages of the disease (I-IV) with a disease duration ranging from 1 to 20 years but only in 4 out of 54 healthy controls (specificity 93%, sensitivity 64%, p < 0.0001), and in none of 12 patients with Hirschsprung's disease as well as 12 patients with cancer of oesophagus and only in one of 11 patients with peptic oesophagitis as well as in one of 13 patients with myasthenia gravis. The present observations suggest that autoimmunity to Auerbach's plexus plays a role in the pathogenesis of achalasia, the mechanism of action is unknown.

[Early evening and late evening administration of roxatidine acetate in therapy of duodenal ulcer].

In a multicenter, placebo-controlled, comparative trial with 196 general practitioners and internists 730 18 to 70 year-old patients with endoscopic evidence of a duodenal ulcer were enrolled. Each group of 365 patients received 150 mg Roxatidine acetate once daily, either late in the evening (bedtime administration, between 10.00 p.m. and 11.00 p.m.) or early in the evening (after dinner, between 6.00 p.m. and 7.00 p.m.). The endoscopic control revealed that the ulcers healed after four weeks of treatment in 74% (bedtime administration) and 77% (administration early in the evening), respectively. There was no significant difference ascertained between both groups. At the last visit, epigastric pain was removed during the day in 86% and during the night in 90% in both groups.

Interrelations between electrical and biochemical processes in ischemic porcine livers at low temperature.

Recently it has been shown that during liver ischemia at 25 degrees C the presence of glycogen, by supporting glycolytic supply, not only retards ATP decay but also leads to a corresponding delay of the rise of the electrical impedance of the ischemic organ. A sudden rise of impedance during ischemia is supposed to indicate the closure of gap junctions. Although similar effects on energy state do exist at low temperature, the impact of glycogen on the electrical impedance under storage conditions has still to be evaluated. Therefore, in a model with protected porcine livers, we examined the intraischemic effects of a preischemic glucose and potassium feeding on impedance changes, lactate production and ATP-content at a storing temperature of 5 degrees C. Impedance was measured both in the low frequency alpha- and the higher frequency beta-dispersion range. In addition, the same parameters were determined in a group of unprotected livers. In this group all animals had received glucose and potassium orally prior to ischemia. Whereas in case of preischemic glucose feeding the rise of impedance in the range of the beta-dispersion (e.g. 5kHz) roughly coincided with the exhaustion of ATP, the corresponding impedance changes in the protected group without a glucose premedication only occurred when glycolysis had already stopped and ATP had reached basal values for some hours. In contrast, in the alpha-dispersion range the impedance changes in the latter group just began at the time when ATP became exhausted and lactate production ceased.(ABSTRACT TRUNCATED AT 250 WORDS)

[Comparison of 2 methemalbumin measurements and of free hemoglobin in view of their value in the differential diagnosis of edematous and hemorrhagic pancreatitis].

Methemalbumin was estimated by means of electrophoresis and the method of Walberg, respectively in 59 patients (25 pat. with edematous, 34 patients with hemorrhagic pancreatitis) partially over three days. In addition the estimation of free hemoglobin in ascites has been carried out. By both approaches of methemalbumin estimation there was no differentiation of the two pancreatitis forms possible. Though the method of Walberg was much more sensitive especially in the serum its specificity was correspondingly lower. The tendency to a correlation of the methemalbumin-concentration to the severity of the course of the disease could be observed. Such a correlation has been proved statistically for the free hemoglobin.

[Possible uses of laser in gastroenterology].

In the therapy of certain benign and malignant changes in the intestinal tract the laser stood the test. The absolute necessity of its use is limited, among others by existing alternative methods, so that a broadly disseminated equipment is not necessary. By the development of new types of laser and better handling systems (fibres with diffuse emission, fibres with lateral delivery of light and so on) its possibilities of application will enhance.

[Papilla carcinoma and adenoma--diagnosis and therapy].

26 peripapillary adenomas and 64 papillary carcinomas are the background for a discussion of the adenoma-carcinoma-sequence in these tumours. An early diagnosis of the adenomas means in this context secondary prophylaxis of papillary carcinomas. Nowadays, the greatest difficulties consist in the differentiation of adenomyomatosis-like processes of the papilla and in the certainty that an adenoma has not changed to a carcinoma. In patients who cannot tolerate a greater surgical procedure the endoscopic papillotomy, possibly in combination with a prosthesis, is an appropriate measure.

[Hyperplasiogenic polyps and stomach carcinoma risk--experiences following 1074 polypectomies and follow-up studies].

In 731 hyperplasiogenic polyps which have been removed by endoscopical polypectomy there were ten polyps with an early cancer and eight polyps with severe dysplasias (borderline lesions) as well as one polyp, in which an advanced carcinoma had developed. This is to date the greatest series of hyperplasiogenic polyps with malignant transformation. In addition, in nine patients with hyperplasiogenic polyps carcinomas could be discovered, i.e. a rate for malignancy of 3.8%. It seems that stomachs with multiple polyps are particularly endangered. The recurrence rate on follow-up examinations covering a period of up to eight years was 36.1%.

[Combination of glandular and foveolar hyperplasia of the gastric mucosa--a rare mixed type].

In a 28 year-old man with epigastric pain endoscopy detected giant gastric folds and multiple superficial erosions. Histologically a mixed form of glandular and foveolar hyper plasia of the gastric mucosa was suggested (Ming's type III). Basal and pentagastrin stimulated secretion volume and acid output were moderately elevated, hypersecretion of protein was not found. Serum levels of calcium and gastrin were normal, also after secretin stimulation. The mucosa of the corpus was extensively infested with Campylobacter pylori. Therapy with cimetidine, antacids, pirenzepine and metronidazol resulted in relief of symptoms but not of histological findings. Bismuth (JatroxR) was successful in eradicating Campylobacter pylori and decreasing inflammation of the mucosa. Cause and prognosis of this mixed hyperplasia are not known.

[Pathophysiologic aspects of diarrhea].

Introductory the anatomical particularities of the small and large bowel in their connection to absorption and secretion are represented. According to that the paper of pathophysiological processes by the hand of typical examples of the different kinds of diarrhoea (osmotic-, secretoric-, motility induced diarrhoea, diarrhoea by morphological changes of the small- and/or large bowel, diarrhoea by disturbances of the active ionic transport) has been taken place.

[Primary clinical, endoscopic and histologic findings in solitary rectal ulcer].

This study presents clinical, endoscopical and histological primary findings of solitary ulcer of the rectum obtained in 24 patients. Solitary ulcer of the rectum is preferably a disease of young women. The main symptom is recurrent rectal bleeding. Endoscopically in 50% of all cases mucosal ulcerations can be observed. The other cases are non-ulcerous lesions with uncharacteristic endoscopical findings. The majority of the lesions are within 6-8 cm of the anal margin. In 44% of the patients there are findings, which are suspicious of preexisting rectal prolapse. The histological main criterion is a fibromuscular hyperplasia of the lamina propria. Full agreement of endoscopical and histological diagnosis at primary examination could be obtained in only 17% of the cases.

[Hyperplasiogenic polyps and risk of stomach cancer].

In 731 hyperplasiogenic polyps which have been removed by endoscopical polypectomy there were 9 polyps with an early cancer and 8 polyps with severe dysplasias (borderline lesions) as well as 1 polyp, in which an advanced carcinoma has been developed. This is the outstanding greatest number of hyperplasiogenic polyps with a malignant transformation. In addition in 9 patients with hyperplasiogenic polyps carcinomas could be discovered, i.e. a rate for malignant developments of 3.7%. It seems that stomachs with multiple polyps are endangered in particular. The recurrence rate of follow-up examinations covering a period of up to 8 years was 36.1%.
