Disseminated intravascular coagulation and hypotension after intravasation of barium.

Prolonged hypotension and disseminated intravascular coagulation were seen in a patient after intravasation of barium sulfate contrast medium during a barium enema examination. High endotoxin levels were measured in the contrast material. In vitro, this material induced generation of bradykinin. The clinical features observed may be explained by contact activation of the Hageman factor-dependent pathways caused by the contrast material and/or by circulating endotoxins. Treatment of this rare but severe complication occurring during a barium enema procedure should be directed against the endotoxic shock.

Immature parietal cells in healing gastric wounds respond to stimulation. An experimental study in rats.

Ulcers were produced in the gastric mucosa in rats. After 90 days of healing, one group of animals was given pentagastrin and carbacholine via continuous subcutaneous infusion. After 1 h the animals were killed and the wounds and normal mucosa beside the wounds were prepared for electron microscopy. Stereological analyses of regenerating parietal cells from the wounds revealed that stimulation by the secretagogues increased the secretory surface density by some 76%. pH studies were performed in the wounds in another group of animals after the same stimulation as above, and it was found that the secretory product was acid. Thus, immature parietal cells in healing gastric wounds reacted normally to the secretagogues given and they were able to secrete HCl.

Quantitative ultrastructural studies on parietal cell regeneration in experimental ulcers in rat gastric mucosa.

The regeneration of parietal cells was studied in rat gastric mucosa. For this purpose wounds were produced by cauterization of the corpus mucosa in adult male Sprague-Dawley rats. The animals were killed at 30, 60, 90, 130, and 260 days postoperatively, six at each time. Sham-operated rats, killed at 60 and 260 days, served as controls. Stereologic data were obtained by electron microscopy of 10-20 parietal cells in each animal. At 90 days immature parietal cells appeared in the center of the wound. These cells were characterized by large nuclei, small mitochondrial volume density, and few tubulo-vesicles. With further regeneration the parietal cells became ultrastructurally mature, but their total volume in the mucosa remained subnormal. Also the epithelial cell volume density was lower than normal after 260 days of healing.

Anaplastic giant and spindle-cell carcinoma of the thyroid: a different therapeutic approach.

The prognosis of giant and spindle cell carcinoma of the thyroid is extremely poor. It does appear, however, that a multisciplinary approach to treatment of this aggressive cancer is more effective than a single-treatment modality. Operative ablation of gross tumor, postoperative irradiation therapy, and chemotherapy have demonstrated encouraging results. A different therapeutic approach with the addition of immunotherapy to surgical resection, postoperative irraditation, and chemotherapy is described in this case report. The preliminary result is encouraging.

Overview of four clinical studies of chemotherapy for stage III and IV non-Hodgkin's lymphomas by the Cancer and Leukemia Group B.

This paper presents an overview of four Cancer and Leukemia Group B studies in 1266 patients with stage III-IV non-Hodgkin's lymphoma. The cases were analyzed across protocols; the major prognostic determinants were prior chemotherapy, age, and histology. The four studies proved that cyclophosphamide maintenance was superior to no maintenance even after prolonged intensive induction chemotherapy. Furthermore, the reinforcement program of monthly pulse doses of vincristine and prednisone, whose value was established in the treatment of acute leukemia, led to highly significant improvement in remission duration and survival. Other facets of the chemotherapy programs are still being subjected to analysis, but this report sets out some preliminary conclusions.

False positive rubella virus haemagglutination inhibition reactions: occurrence and disclosure.

A floatation centrifugation procedure is described by which antibodies and non-specific rubella virus haemagglutination inhibitors can be completely separated. Using this method it was shown that false positive reactions occur with the conventional pretreatments of sera for removing inhibitors, especially with the heparin-McCl2 procedure. Other methods for removal of inhibitors were examined in view of recent knowledge about the chemical nature of the inhibitors. Treatment with phospholipase C proved useful for disclosure of false positive reactions, while floatation centrifugation was the most sensitive method for demonstration of low antibody concentrations.

The effect of long-term variation in sympathetic activity on testicular morphology in immature rats.

In order to investigate the effect of increased and decreased sympathetic activity on testicular development, immature male rats were injected with isoproterenol or 6-hydroxydopamine from birth up to 49 days of age. Isoproterenol treatment resulted in a marked Leydig cell hypertrophy but the development of the spermatogenic epithelium was unaffected compared to that in saline treated controls. 6-hydroxydopamine treatment in a dose inhibiting the development of the parotid gland did not influence testicular development.
