Correlation between hip joint laxity and subsequent coxarthrosis in dogs.

In a prospective study the degree of hip joint laxity was compared with subsequent hip joint development. A total of 70 non-selected adult dogs from 34 breeds presented for hip dysplasia screening was examined. The degree of hip laxity was quantitated using a newly developed radiographic stress technique. The dogs were re-examined according to the standard radiographic technique after 1 year or more. There was a positive correlation (r = 0.58) between the degree of subluxation (SI) and the subsequent Canine hip dysplasia (CHD) grade. All dogs with an SI value of up to 0.35 developed hip joints ranging between normal and mildly dysplastic at worst. Of the dogs with an SI value higher than 0.35, 76% developed hip dysplasia. Of those 40 dogs grading normal or borderline normal on standard radiographs, 15% demonstrated lax hip joints with an SI of more than 0.35. Breed-specific differences in the correlation of degree of hip joint laxity to the development of coxarthrosis were noted. If both excessive hip joint laxity and development of arthrosis are considered exclusion criteria for breeding dogs, then the current mode of selection does not adequately restrict potential breeding stock. Only dogs demonstrating an SI of 0.35 or less on stress radiographs and graded normal or borderline normal on standard radiographs should be used for breeding, equivalent to 49% of all dogs examined in this study. A further 36% of the dogs examined, most of them currently still accepted as breeding dogs, would not pass. Breed-specific acceptable degree of hip joint laxity in breeding dogs should be determined. Selecting breeding dogs based on the results of hip joint laxity assessment may further decrease the incidence of CHD in the offspring.

Effect of intra-abdominally induced pressure on the urethral pressure profiles of healthy and incontinent bitches.

OBJECTIVE: To determine whether reduced pressure transmission is of importance in the pathophysiologic mechanism of urinary incontinence in bitches. ANIMALS: 20 sexually intact, continent bitches and 21 spayed, incontinent bitches. PROCEDURE: Urethral pressure profiles before (resting) and after (stressed) insufflation of gas in the abdominal cavity were recorded in bitches under general anesthesia. Differences (stressed minus resting) were calculated for all variables. On the basis of these values, the pressure transmission ratio was determined. RESULTS: Resting pressure profiles of incontinent bitches indicated significantly (P < 0.05) lower maximal closure pressure (4.5 +/- 3.0 cm of H2O) than did those of continent bitches (11.2 +/- 7.2 cm of H2O). The intra-abdominal pressure increase lead to a shortening of total profile length, which was equal in both groups and caused an increase in maximal urethral pressure. The change in maximal urethral pressure was significantly (P < 0.05) greater in incontinent bitches (12.4 +/- 4.1 cm of H2O) than in continent bitches (8.8 +/- 4.5 cm of H2O). The profile areas and the pressure transmission ratios did not differ significantly (P < 0.05) between the 2 groups. CONCLUSION: The effect of decreased pressure transmission on the urethra is not a factor in the pathophysiologic mechanism of urinary incontinence attributable to urethral incompetence in bitches.

[Von Willebrand factor concentrations in blood plasma of Bernese mountain dogs].

Many Bernese Mountain dogs have been found to exhibit an increased hemorrhagic tendency of unknown etiology. Since other bleeding disorders have been excluded by routine tests and Bernese Mountain dogs have been listed to have von Willebrand's disease (vWD), we analyzed the plasma concentration of the von Willebrand Factor (vWF) in 160 Bernese Mountain dogs that were used for breeding in Switzerland in 1992. We also evaluated the suitability of the commercial Asserachrom vWF test kit to quantitate vWF in canine plasma by comparing the plasma vWF determination with validated vWF ELISA test. The vWF plasma concentration in Bernese Mountain dogs ranged from 13% to 162%, with the Asserachrom test kit (normal range 67% to 124%). Similar values were obtained with the research vWF ELISA kit (10% to 166%), and there was a close correlation between the two test methods. In 8 of the 9 Bernese Mountain dogs with initially low vWF concentration (< 60%), the determination was repeated on another sample. Since the values were well within the normal range, a problem with the collection of blood for the first determination is suspected. We conclude that vWD does not appear to be a clinical issue in Bernese Mountain dogs in Switzerland and is, therefore, not likely to be the cause of the observed bleeding tendency. The commercially available Asserachrom vWF test kit seems suitable for the determination of canine vWF plasma concentrations. It is recommended that low vWF values will be confirmed by the determination in second samples.

Obstetrical brachial plexus palsy (OBPP) outcome with conservative management.

Resurgence of neurosurgical intervention of obstetrical brachial plexus palsy prompted our review of 186 patients evaluated between 1981 and 1993, correlating clinical examination, electrodiagnosis, and functional outcome with conservative management. Eighty-eight percent had upper brachial plexus palsies, and 63% were mild. Forty-two infants required no long-term follow-up because they rated 1 or 2 on initial physical examination. Comparing first and last follow-up clinical findings of the remaining 149 patients, there was high agreement (correlation r= 0.81; P < 0.001). Pearson correlation of initial physical exam with electrodiagnosis at three intervals was relatively stable (r= 0.87, 0.88, 0.69). One hundred eight (72%) of the patients remained in their original severity groups. Thirty-three of 41 patients with discrepant follow-up scores improved by at least one category. Eight patients deteriorated. The natural pathophysiology and recovery of OBPP is presented.

[Measurement of stress parameters in farm animals using active telemetry].

We investigated the effect of an acute stressor on body temperature and heart rate in cows. Both parameters were recorded by active telemetry. For the experiments, five cyclic Brown Swiss cows were used, each of them exposed to an acute stressor during estrus and the luteal phase of the cycle. The stressor consisted in restraining the cows in a crush for hoof treatments. During the 2-hour stress period and for additional 6 hours, body temperature and heart rate were measured every 10 minutes. Control animals remained in their accustomed environment during the whole experiment of 8 hours. The course of body temperature and heart rate was clearly influenced by the stressor. While heart rate was maximal already at the beginning of the stress period, the body temperature showed highest values only one hour after stress and then continuously decreased. Changes in body temperature and heart rate under stress differed significantly from the values of control animals. Both parameters are reliable indicators of stress in the bovine species. The transmitters used in this investigation allowed us to register exact data without manipulation of the animal, which could possibly falsify the results. This advantage as well as the functional longevity of the transmitters (ca. 6 months) make active telemetry a useful tool for stress research in farm animals.

[Reproductive medicine in transition: new developments in embryo transfer].

Successful application of embryo transfer (ET) has become common practice in cattle, horses, sheep, goats and a variety of other species held in captivity. Yet in cattle only has the technique been established commercially. In 1994 more than 100,000 bovine embryos have been transferred in European countries. Important progress in transvaginal ovum pick up (OPU), in vitro production (IVP) and cryopreservation have further improved the applicability of ET. Direct transfer simplifies the procedure considerably allowing individual transfers and eliminating the need of synchronizing recipients. In Switzerland the organization 'Veterinary Society for Embryo Transfer' (TIGET) has been founded in 1995 to support practitioners performing embryo transfer.

[Training in cognitive functions in neurologic rehabilitation of craniocerebral trauma].

This study evaluates a new cognitive rehabilitation therapy for patients after severe head injury. In addition to the standard neurological rehabilitation therapy, one group was trained by the Wiener Determinationsgerät (WDT), a second group was treated by the new program REHACOM, while a third group received only conventional neurological rehabilitation therapy. The three groups each consisted of 12 patients; two groups received 20 sessions of training, each lasting 40 minutes. At the beginning as well as after the therapy a psychological test battery was applied, consisting of HAWIE, TULUC, AACHENER APHASIETEST, and BENTON-Test. They were also tested by a specific neuropsychological battery regarding hemispheric specialization. REHACOM showed significantly higher values on the HAWIE as well as on BENTON-Test than the other two groups. REHACOM also improved in right-hemispheric dimensions while WDT group did not improve in attention. Right-hemispheric training was more effective than attentional stimulation.

[Comparison of vaginal devices CIDR and PRID in ovariectomized cows using hormone analysis].

Plasma concentrations of progesterone and estradiol were compared in 7 ovariectomised cows after application of 2 different intravaginal releasing devices. Peak concentrations of 4-6 ng/ml progesterone were found within 2 hours after application of the devices. From day 2 to day 6 concentrations sank continuously to 2 ng/ml. Progesterone levels of 1-2 ng/ml prevailed from day 6 to the day of removal of the pessaries (day 12 for PRID, day 15 for CIDR). No differences between the two devices were detected. Increased concentrations of estradiol were found for PRID only, reaching maximum levels of 6-7 pg/ml plasma 150 minutes after application.

Membrane/water partition of oligo(ethylene oxide) dodecyl ethers and its relevance for solubilization.

Mixed aqueous dispersions containing palmitoyloleoylphosphatidylcholine and oligo(ethylene oxide) dodecyl ethers C12EOn with n = 2-8 have been investigated. The aggregates composition has been determined as a function of the aqueous detergent concentration using Laurdan fluorescence spectroscopy. The partition of the detergent between the membranes and the aqueous phase has been analyzed on the basis of the regular solution model. Solubilization has been analyzed in terms of the limiting detergent fraction in the membrane, the minimal detergent fraction in micelles and the critical aqueous detergent concentration using thermodynamical coexistence conditions for the aqueous, bilayer and micellar pseudo-phases. A thermodynamic criterion for solubilization is presented. The standard chemical potential differences of the transfer of the detergents from water to the bilayer have been found to follow the empirical relation delta u o(n = 2-8) = -37.6 + 0.9 n kJ/mol. Thus, a similar conformation and localization of the oxyethylene units within the membrane are suggested.

Comprehensive rehabilitation of the child with osteogenesis imperfecta.

Children with osteogenesis imperfecta (OI) that results in considerable deformity are often viewed as poor candidates for aggressive physical therapy and rehabilitation. To determine if this view is realistic, we have entered almost 50 children with OI type III and OI type IV into a comprehensive graduated rehabilitation program, based at the National Institutes of Health, but designed to be implemented by continuing involvement of community resources. Children are begun in the program early with emphasis on gain of head and trunk control and progression to sitting and walking, if possible, with the aid of a variety of physical supports, including internal and external bracing. Although not conducted in a randomized fashion, the program's success in bringing children into graded exercise regimes and fostering their increased involvement in school and social situations suggest that aggressive physical therapy and rehabilitation have a major place in the overall care of the infants and children with OI.

Posturographic measurement of body sway in survivors of severe closed head injury.

Postural imbalance was measured in 39 survivors of severe closed head injury 7 to 66 months after head injury using a posturographic platform. The results were compared with those derived from age-matched healthy control subjects. Severe postural imbalance, particularly in an anteroposterior direction, was found in 16 patients, whereas 9 patients showed moderate imbalance and 14 patients showed normal results in the posturographic investigation. Low initial Glasgow Coma Scale scores and deep parenchymal brain lesions demonstrated by magnetic resonance imaging were shown to be significant indicators of subsequent severe postural imbalance. The duration of posttraumatic amnesia, the localization and size of cortical contusions and subcortical white matter lesions, on the contrary, were not associated with postural imbalance in the long-term outcome.

Rehabilitation approaches to children with osteogenesis imperfecta: a ten-year experience.

Twenty-five children with severe osteogenesis imperfecta were followed and treated with physical and occupational therapy for a mean of five years, two months, 80% since the first two years of life. According to their highest achieved motor skill, they fell into three groups: (A) able to stand in braces, (B) able to ambulate short distances without braces, and (C) able to ambulate in the community without assistance. Preventable functional impairment is caused in group A by shoulder joint and hand contractures and upper extremity weakness, group by hip flexion and plantar flexion contractures of the feet, shoulder joint contractures, and upper extremity weakness, and group C by poor lower extremity joint alignment, impaired balance, and low endurance. Appropriate rehabilitation strategies have led to slow, continued functional improvement in the majority of patients. Results of the ten-year retrospective study suggest that clinical groupings based in part on functional needs will enable the clinician to provide optimal rehabilitation services.

Behavioural and psychosocial sequelae of severe closed head injury and regional cerebral blood flow: a SPECT study.

Thirty six patients (31 male, 5 female) who had suffered severe closed head injury were re-examined at an average of 39.3 (SD 12.8, range 7-66) months after the injury. Behavioural symptoms were measured using the Giessen test. The relatives' reports were used for data analysis to ensure that results were valid. The neurophysical impairment subscale of the Glasgow assessment schedule was completed by two neurologists, and the number connection test was completed by each patient. The adjective mood scale was completed by each relative. All patients were investigated by single photon emission computerised tomography (SPECT). Exploratory factor analysis using the principal components method was carried out separately for SPECT results and psychological measures and correlations were sought between the resulting factors. Factor analysis of the data from the Giessen test identified social isolation, disinhibition, and aggressive behaviour as major components of post-traumatic personality changes; it indicates that these behavioural features are independent of the level of neurological and neuropsychological impairment, which loaded on a single independent factor. Relatives' psychic health seemed to be relatively resistant to physical and cognitive disability and was mainly affected by disinhibitive behaviour. The highest correlation was between frontal flow indices and disinhibitive behaviour (p less than 0.01): the severity of disinhibition increased with lower frontal flow rates. There was a significant but somewhat weaker correlation (p less than 0.05) between flow indices of the left cerebral hemisphere and social isolation. Low flow values of the right brain regions were related to aggressive behaviour (p less than 0.05). Neurological and cognitive impairment correlated negatively with the thalamus; worse neurological and cognitive performance indicate by raised scores on the neurophysical scale and on the number connection test was associated with low thalamic flow values. The results support the importance of lesion location in the production of post traumatic behavioural disorders.

[Perspective in the incidence of Alzheimer's disease in Austria by the year 2050].

Population projections of the Austrian Central Statistical Office show a dramatic increase in the proportion of people of 65 years of age and over in Austria by the year 2050. Since this population group is at higher risk to develop Alzheimer's disease (AD/SDAT), it will be necessary to modify the currently available facilities for social and medical care to meet this increasing demand. Based on official demographic data and epidemiological findings of the "Eurodem" group, our computations show that the number of persons with AD/SDAT aged 65 or over will rise from 48,000 at present to over 113,500 in 2050 (1990: 614/100,000, 2050: 1505/100,000 persons of the total population), equivalent to an increase of 140%. By forecasting the number of persons affected, it should be possible to intensify (and in some areas introduce) long-term health planning with respect to the social, financial and medical care of aged, demented people in Austria.

[Epidemiologic progression of dementia diseases in Austria until the year 2050].

Population projections of the Austrian Central Statistical Office show a dramatic increase of the proportion of people of 65 years of age and over in Austria until the year 2050. Since this population group is at higher risk to develop dementia, it will be necessary to suitably modify the presently available facilities for social and medical care to meet this increasing demand. Based on official demographic data and epidemiological findings of the "Eurodem"-group (12) our computations show that the number of demented persons, aged 65 or over, will rise from presently 80,000 to over 189,000 in 2050 (1990: 1,023/100,000, 2050: 2,509/100,000 persons of the total population), equivalent to an increase by 140%. By forecasting the number of persons affected, it should be possible to intensify (and in some areas introduce) long-term health planning in respect of aged people, dementia and social, financial and medical care in Austria.

Juvenile rheumatoid arthritis. Aquatic exercise and lower-extremity function.

This pilot study investigates the effects of aquatic therapeutic exercise on lower-extremity range of motion, gait, balance, and functional mobility in children with juvenile arthritis. Eleven patients, aged 4-13, with lower-extremity joint involvement, diagnosed as functional class I-III, completed a 6-week program of aquatic exercise aimed at increasing lower-extremity range of motion and strength. Despite the small sample size and short duration of the study program, significant improvement was noted in external and internal hip rotation, bilaterally (p < 0.05). Improvement was noted in the median scores for most other parameters; however, these did not reach statistical significance. Aquatic exercises performed in a group setting can serve as an enjoyable and beneficial part of therapy for children with arthritis. Further investigation is recommended to determine fully the effects of aquatic therapeutic exercise on mobility and fitness in children with juvenile arthritis.

MRI in basal ganglia diseases.

76 patients suffering from different basal ganglia diseases (28 cases with M. Parkinson, secondary parkinsonism and Parkinson diseases; 5 cases with Chorea Huntington; 5 cases with Fahr disease and 38 cases with M. Wilson) MRI featured 2 characteristical patterns: 1. abnormal deposition of minerals, 2. focal atrophies of involved organs. Thus MRI provides with informations about: 1. differential diagnosis in clinically misleading courses, 2. stage and, as a consequence, prognosis of some diseases, 3. biochemical processes of diseases in vivo.
