Controlled trial of homoeopathic treatment of osteoarthritis.

In a double-blind, placebo-controlled crossover study to compare the homoeopathic remedy Rhus tox. 6X with fenoprofen in osteoarthritis of the hip and knee, fenoprofen was shown to have beneficial analgesic and anti-inflammatory effects which differed significantly from those of placebo. The effects of Rhus tox. 6X and placebo did not differ significantly. Patient preference was for fenoprofen. Side-effects were not severe but were seen more frequently with fenoprofen. Similar results were seen in all patients regardless of whether they had been referred to and assessed by a homoeopathic physician or a rheumatologist.

Measurement of nerve conduction--a comparison of orthodromic and antidromic methods.

The validity of the antidromic method in the measurement of median and ulnar sensory nerve conduction is determined. Analysis of our results show that both the orthodromic and antidromic methods provide consistent results. Both methods were accurate in diagnosing Carpal Tunnel Syndrome (CTS), with an upper limit of normal for distal sensory latency of 4.5 msecs motor, and 4.0 msecs sensory in the median nerve. Variation with age and sex was studied. Latency when measured by either method showed no variation. Amplitude showed a consistent decrease with age. No sex difference was detected. We conclude that the antidromic stimulatory method is accurate, reproducible, and convenient, and therefore is at least as good if not better than the orthodromic method in the context of a busy routine electrodiagnostic clinic.

Comparison of timegadine and naproxen in rheumatoid arthritis. A placebo controlled trial.

A double blind, cross-over design was used to compare the acute effects of timegadine, naproxen and placebo in rheumatoid arthritis. Timegadine appeared to be more effective than naproxen in those variables related to disease activity although there were no statistically significant differences (apart from morning stiffness) where variables were examined individually. There was a tendency to a rise in alkaline phosphatase, and possibly gamma glutamyl transpeptidase, during the 2-week timegadine treatment period. No other biochemical or haematological abnormality was observed.

Antrafenine, naproxen and placebo in osteoarthritis: a comparative study.

Antrafenine is a new non-narcotic analgesic. In a double-blind, cross-over study the efficacy of antrafenine at doses of 450 mg/day and 900 mg/day was compared to naproxen 750 mg/day and placebo in patients with osteoarthritis. Each drug treatment was given for two weeks, the total duration of the study being eight consecutive weeks. Antrafenine, at either dose, was effective in relieving pain associated with osteoarthritis; the efficacy was comparable to naproxen. There was no definite indication that 900 mg/day was more effective than 450 mg/day. Antrafenine, at both dosage levels, was well tolerated. Any noted side-effects were mild. There was a total of 12 side-effects in nine patients with the high dose, five side-effects in five patients with the low dose, compared with 11 side-effects in nine patients with naproxen and 10 side-effects in seven patients with placebo.

Surgical treatment for distal tubal occlusion. A review of 167 cases.

Surgery was performed on 167 patients for distal tubal occlusion. In 143 cases a terminal microsurgical salpingostomy was performed and in 24, a cuff neostomy. The overall intrauterine pregnancy rate was 20.4%, and 16.8% of the patients had live births. The ectopic gestation rate was 1.8%. None of the patients with a cuff neostomy became pregnant.

Lymphocyte studies in a patient with neurological complications of Behçet's syndrome.

We rapport here a case of Behcet's disease presenting with neurological complications who made a remarkable recovery to high dose steroids and azathioprine. Prior to initiation of steroid therapy the percentage of T lymphocytes (OKT3) was low, that of helper T lymphocytes (OKT4) was normal and suppressor T lymphocytes (OKT8) was slightly increased. The OKT4: OKT8 ratio was slightly reduced and the percentage of B lymphocytes was high. With steroid therapy there was an increase in the percentage of OKT3 and fall in the percentages of OKT4 and OKT8 and the OKT4: OKT8 ratio was maintained fairly normal. The percentage of B lymphocytes was reduced. On addition of azathioprine the percentage of OKT3 and OKT8 were substantially increased and the percentage of OKT4 was slightly increased. The net result was a significant reduction in the OKT4: OKT8 ratio. The percentage of B lymphocytes remained normal. On reviewing the absolute counts there was a paradoxical rise in OKT4 and OKT8 a week after high dose steroid therapy due to a substantial rise in the total number of lymphocytes. On reducing the dose of steroids the absolute count of OKT4 and OKT8 decreased in parallel with their percentages. A disparity was noted in the percentage of OKT3 and the sum of the percentages of OKT4 and OKT8, prior to commencement of therapy which was significantly reduced after treatment. It is likely that this disparity is due to the presence of doubly labelled cells (cortical thymocytes) released into circulation prematurely.

Naproxen sodium, diflunisal, and placebo in the treatment of chronic back pain.

Thirty-seven patients with chronic back pain were entered into a randomised, 3-way, double-blind, cross-over comparison of naproxen sodium 550 mg twice daily, diflunisal 500 mg twice daily, and placebo. Each treatment was given for 14 days after a preadmission wash-out week during which only paracetamol was allowed. Patients were assessed on admission and at the end of each treatment with respect to global pain, night pain, pain on movement, and pain on standing. Both visual analogue scales and simple descriptive scales were used to measure pain. Side effects were elicited by a nonleading question. Both methods of pain measurement gave similar results and were highly correlated. Naproxen sodium was superior to placebo in relieving global pain and depending on the method of measurement, in relieving night pain and pain on movement. Diflunisal showed no significant differences from placebo. Side effects were similar on all 3 treatments. The final preference of the patients was significantly in favour of the active treatments.

Axillary nerve palsy following blunt trauma to the shoulder region: a clinical and electrophysiological review.

Although the commonest type of axillary nerve palsy occurs following shoulder dislocation on humeral fracture, another form is seen after blunt trauma to the shoulder region without associated fracture or dislocation. The former usually goes on to a full recovery whereas a failure to recover is common in the latter group. In our review of 13 patients with palsy after blunt shoulder trauma, seven patients showed minimal or no recovery of deltoid muscle function and six patients went on to complete or near complete recovery. Serial electromyographic examinations usually revealed the lesion to be in continuity although eventual clinical recovery was not satisfactory in a number of these patients. The mechanism of the palsy appeared to involve a stretch injury and this was confirmed at operation in two patients. Glenohumeral fixation was a troublesome complication which limited recovery of function in four patients. Further details of the type of trauma, clinical and electromyographic examination, assessment and management are discussed.

Correlation in rheumatoid arthritis of concentrations of plasma C3d, serum rheumatoid factor, immune complexes and C-reactive protein with each other and with clinical features of disease activity.

The concentrations of C3d in the plasma and of C-reactive protein (CRP), immune complexes and rheumatoid factor in the serum were measured in 99 patients with rheumatoid arthritis. Most patients had raised levels, the values of which correlated with disease activity assessed according to a newly described index based on multivariate analysis of subjective, semi-objective and objective features of the disease. There were also significant correlations between the values for plasma C3d and circulating immune complexes, immune complexes and rheumatoid factor, serum CRP and immune complexes, and serum CRP and plasma C3d. Measurement of plasma C3d provides a useful means of detecting in vivo complement activation, which may be involved in the pathogenesis of rheumatoid arthritis, but neither C3d levels nor any of the other variables correlated as closely with disease activity as did the serum CRP concentration.

A double-blind study of benorylate and chlormezanone in musculoskeletal disease.

In a double-blind, cross-over study of 90 patients with degenerative disease of the hip, knee, cervical or lumbar spine, and capsulitis of the shoulder the analgesic drug benorylate (either alone or in combination with chlormezanone, a muscle relaxant anxiolytic drug) favourably modified pain, stiffness, quality of sleep and ability to work. Chlormezanone significantly reduced the number of breaks in sleep. There was no significant difference in the number of patients reporting side-effects on each of the four treatments, but drowsiness occurred significantly more in the chlormezanone weeks. There appeared to be no advantage in adding chlormezanone in patients suffering from osteoarthritis of the hip or knee, lumbar spondylosis or capsulitis of the shoulder, but there was significant improvement in both pain relief and quality of sleep in those patients with neck pain.

Indoprofen compared with naproxen and placebo in rheumatoid arthritis.

Thirty-six patients suffering from rheumatoid arthritis took part in a double-blind controlled cross-over trial, in which they received either indoprofen 800 mg daily, naproxen 500 mg daily, or a matching placebo. Each treatment was administered for two weeks. Indoprofen was shown to be significantly superior as an analgesic and in improving grip strength, and was the drug the patients preferred. Adverse effects were comparable, although indigestion was seen slightly more often during indoprofen treatment. Indoprofen is therefore at least as effective as existing anti-inflammatory drugs in rheumatoid arthritis and should be used as an alternative.

Arginine deficiency syndrome. Its occurrence in carbamyl phosphate synthetase deficiency.

A child with carbamyl phosphate synthetase deficiency was studied. During attempts at dietary control, a peculiar rash and cessation of growth were noted concomitant with a very low serum arginine level. A prospective study was designed so that the only variable would be the addition of 400 mg of arginine to his diet. During the two-week period without arginine, the rash and growth failure recurred and paralleled a drop in the serum arginine level from 0.048 to 0.034 nmol/mL. Then with the addition of arginine, his rash dramatically cleared and he resumed normal growth. Later, it became necessary to raise his arginine intake to 800 mg/day to maintain continued growth. These data support the concept that arginine becomes an "essential" amino acid in persons with urea cycle defects, which may partially explain the extremely variable clinical manifestations and courses seen in these patients.

Spontaneous facial movement.

Otolaryngologists should be aware that there is a large differential diagnosis to be considered in patients presenting with spontaneous facial movement. A practical classification of the various forms of such movement is presented. Current thinking regarding the etiology and treatment of the more common conditions is reviewed.

Isolated vagus nerve palsy and vagal mononeuritis.

It is useful to consider vagus nerve palsy as a form of cranial mononeuritis. In contrast with other forms of mononeuritis, the clinicoanatomical features and other aspects of vagal mononeuritis are not well defined. A series of 25 patients with isolated vagal mononeuritis were assessed by clinical and laryngoscopic examination, and laryngeal electromyography was done in all cases. Unilateral recurrent laryngeal nerve palsy was almost twice as common as unilateral superior and recurrent laryngeal nerve palsy and it appeared to have a better prognosis. The majority were on an idiopathic basis, and associated diabetes in two patients raised the possibility of diabetic mononeuritis. Three patients had bilateral palsies; bilateral vagal mononeuritis may represent a separate entity in that it occurs in younger subjects and is associated with upper respiratory tract illness. Partial or complete recovery occurred in about half of the patients.

Antirheumatic activity of fenclofenac.

This study has set out to establish whether fenclofenac has an antirheumatic effect in addition to its anti-inflammatory and analgesic activity. The results show that during the course of a 6-month study the drug improved clinical parameters, including the articular index, early morning stiffness, ring sizes, and grip strength, and produced changes in laboratory measurements such as the levels of C-reactive protein, IgG, and rheumatoid factor.

Expectation and patient preference -- does it matter?

Admission to clinical trials is often based on the assumption of homogeneity of the population. A group of 60 patients, all with pain in the neck or shoulder of at least 3 months duration, were studied. Expectation was graded before treatments were started by sharing out 100 points between freedom from side effects, pain relief, depression relief, improved mobility, improved sleep and speed of action. A double-blind crossover study of two established anti-inflammatory analgesics and placebo was carried out. Using analogue scales, patients were asked to grade their response. Side effects were recorded, and preference was established at the end of the study.Although all the patients were in sufficient pain to require medical attention, some rated relief of depression, improvement in sleep or lack of side effects as more important than pain relief. Differences between drug and placebo were most clearly demonstrated in those patients whose main concern was improved mobility. In our view it is important to select patients who are in need of a dominant property of a drug for a trial of this property and this may have ramifications across the medical spectrum.
