Hydroxyethylstarch 450/0.7 has a prolonged intravascular volume expanding effect. Anaphylactoid reactions have not been seen in the present series, although untoward side effects of this kind have already been reported. It is to be hoped that the restricted use of plasma expanders in favour of the much more expensive human albumin will be changed in future in the direction of using plasma expanders more freely than during the last years. Within one and a half year 24 patients with arrhythmias or chest pain were investigated to detect a mitral valve prolapse syndrome which was found in 9 cases by echocardiography. Within this group 6 patients complained of fatigue, dizziness, dyspnea or syncope, 6 had chest pain, 7 paroxysmal tachycardia and 2 patients premature beats. Auscultation revealed in 3 cases a systolic click, in 1 case a systolic click with late systolic murmur and in 5 cases a systolic murmur only. The ECG showed premature ventricular contractions in 2 patients, ST-T abnormalities in 6 patients. Echocardiography showed a late systolic prolapse in 6 and a pansystolic prolapse in 3 patients. In 3 cases also an angiography was performed and in this way a mitral valve prolapse detected; hemodynamics and coronary arteries were normal in all 3 cases but in one case a mitral insufficiency and in one case an asynergy of the anterior wall was found. Pathophysiology, clinical symptoms and phonocardiographic, echocardiographic and angiographic findings in mitral valve prolapse syndrome are discussed.

Cells of Salmonella typhimurium wild type and of several well defined lipopolysaccharide mutants were treated with EDTA. The percentage release of lipopolysaccharide and phospholipid was determined. The results obtained show that the release of lipopolysaccharide by EDTA declines along with the gradually diminishing chain length of the lipopolysaccharide, although the total amount of lipopolysaccharide, although the total amount of lipopolysaccharide was found to increase at the same time in the respective mutants. Implications of these findings for the organization of the outer membrane are discussed.
A new urodynamic method is described which is based upon the principle of measuring the anteroposterior diameter of the urinary bladder during micturition by ultrasound. A 4 MHz transducer is placed vertically on the abdominal wall 6 to 7 cm above the symphysis and kept in a constant position by means of a special device. The continuously decreasing bladder diameter during micturition is recorded by M-mode echography (Echopan K, Siemens) and plotted as a diameter-time curve. Normals can be differentiated from patients with a subvesical outflow obstruction or a neurogenic disturbance of bladder emptying. For quantification the mean velocity of bladder diameter shortening during the first 30 seconds of micturition is calculated and correlated with the maximal uroflow (r = 0,77). Therefore a rough quantitative measurement of micturition seems to be possible.

As persistent bone loss is a major problem of chronic steroid therapy the effects of 3 weeks therapy with 1 alpha hydroxycholecalciferol were studied in 10 kidney transplant recipients on chronic steroid therapy. Treatment resulted in a significant increase in intestinal calcium absorption, paralleled by a significant decrease of serum iPTH, whereas neither plasma calcium nor urinary calcium excretion rose significantly, suggesting a positive calcium balance throughout the study. It is therefore suggested that administration of active vitamin D metabolites is an effective protection against one of the pathogenic mechanisms of steroid induced osteopenia.
General principles for application of red blood cells depend on the amount of blood loss, on the oxygen-transport capacity, on the function of the transfused red cells and on the risks of blood transfusion. As a rule of thumb in acute blood loss red cells are wanted if the haemoglobin falls below 10 g/dl or the haematocrit below 30% and are urgently necessary if the haemoglobin falls below 8 g/dl and the haematocrit below 25%. In chronic normovolemic anaemia the indication for red cells is completely different. In the praeoperative state erythrocytes are given below 9 to 10 g/dl haemoglobin. Advantages and disadvantages of the different red cell preparations are discussed as follows: Whole blood is only indicated as fresh blood. Fresh blood stored not longer than 12 hours is only indicated in massive blood transfusion and in acute blood loss due to not yet cleared up haemorrhagic diatheses. A partial deplasmatized blood can be safely given instead of whle blood. Packed red cells not washed show a smaller volume risk and decrease the frequency of non-haemocytic transfusion reactons. The saline washed red cell concentrate does not reduce the frequency of alloimmunisation against tissue antigens, a decrease of the risk of hepatitis-B is questionable. The indication for washed red cells are to be found in febrile transfusion reactions due to leukocyte and platelet antibodies and in the transfusion preparation of dialysis patients. Leucocyte poor red cell concentrates can be prepared practically on by mechanically taking away the buffy coat. The frequency of alloimmunisation can be lowered down to 4% by this red cell preparation. The most expensive way of deep freezing red cells has its own indications. A decrease of hepatitis-B risk is present, hepatitis-B on the other hand cannot be safely avoided by using deeply frozen and thawed red cells.

In 16 patients admitted in shock, heparin prophylaxis for DIC was carried out. The effect of heparin was monitored by a series of coagulation tests. A distinct relation was found between the effect of heparin on the aPTT and on the thrombin clotting time and the activity of antithrombin III. Even a comparatively slight diminution of this activity resulted in a considerable decrease of the heparin effect on coagulation. Since this effect is of primary importance for the efficacy of prophylaxis of DIC, regular assays of antithrombin III are proposed in shock patients receiving heparin.

A rupture of the interventricular septum, as described in a case report, is found in 2% of myocardial infarctions. Clinical symptom loud systolic murmur audible at the left sternal border associated with a thrill and with signs of cardiogenic shock. The diagnosis is made by right heart catheterisation which shows a typical oxygen stepup between right atrium and right ventricle, by which the rupture of the interventricular septum can be differentiated from acute papillary muscle rupture. The therapy should be at first hemodynamic stabilisation by drugs and intraaortic balloon pump for 4 to 6 weeks and then closure of the ventricular septal defect by operation.
A survey is presented of the variations of neuroleptanalgesia. Modification of the injection technique, the hypnotic, the analgesic or the neuroleptic partial effect of the drug combinations produces four variations. The principles of the classical type of neuroleptanalgesia, namely the ability to selectively influence the single components of anaesthesia and the predominant use of a strong analgesic must remain unaltered in the definition of a variant. The use of clear nomenclature is used in an attempt to overcome the confusion which still exists in this field.
From the results of application of various inhibitors, or combinations of inhibitors to cell suspensions of the yeast Torulopsis candida and by determination of O2-consumption and growth rates it is concluded, that a main site of action of tridemorph is localized in the pathway of the respiratory chain. Tridemorph inhibits as well the respiration as the growth of T. candida depending on the concentration and the time of exposition. Tridemorph does not uncouple respiration but inhibits the uncoupled respiration to the same extent. Inhibition of respiration and growth by tridemorph is enhanced by suboptimal concentrations of antimycin A, oligomycin, rotenone, and 2,4-dinitrophenole. That leads to the conclusion that these inhibitors essentially enlarge the permeability for tridemorph which can be more rapidly transported to the site of action.
Changes in erythropoiesis were studied in 94 patients with testicular germ cell tumours (24 seminoma, 70 nonseminomatous tumours). Additionally ferrokinetic studies with 59Fe were performed in 29 patients with proven metastases. Only 2 of 37 tumour-free patients sustained moderate anemia after recess of primary therapy (2 seminoma, after X-ray-irradiation). In patients with disseminated tumour disease the anemia and the reduction of 59Fe-utilisation correlated positively with the elevation of serum estradiol concentrations, whereas no correlation could be established with testosterone serum levels.
