An integrated treatment program for psychogenic pain.

The family characteristics associated with the presence of recurrent abdominal pain are identified. The involvement on the pediatrician and the effects of this involvement on the patient, family, and perpetuation of the symptoms are described. A successful therapeutic program combining behavior modification and family therapy is elaborated. Changes in the structure and functioning of the family are vital to the outcome of therapy and the prevention of recurrence of symptoms.

Patterns of sarcoidosis in three population groups in New York City.

Caucasians in this study group presented in the chronic stage of sarcoidosis more frequently than the Blacks and Puerto-Rican-born. 2) Twenty-eight percent of the Caucasians and 26% of the Blacks who presented in the chronic stage of sarcoidosis, died of pulmonary insufficiency. 3) Puerto-Rican-born women are especially prone to present with erythema nodosum and Stage I radiographic sarcoidosis. 4) Sarcoidosis with extrathoracic organ involvement was more common in Black and Puerto-Rican-born patients than in Caucasians. 5) The size of the Kveim papules was larger in the Black and Puerto-Rican-born patients than among the Caucasians. 6) Two-thirds of all groups were insensitive to tuberculin. 7) The extent and duration of disease at presentation is a major determinant prognosis. 8) Ethnic factors influence the clinical patterns in sarcoidosis, but socioeconomic factors may preclude generalizations based solely upon clinic populations in New York City.

Evidence that mismatched bases in heteroduplex T4 bacteriophage are recognized in vivo.

T4 heteroduplex heterozygotes are lost selectively after prolonged incubation of phage-infected Escherichia coli cells under nonreplicating conditions. The loss of heterozygosity occurs for four out of six rII sites tested and is not dependent upon T4 v gene function. The results are interpreted to indicate the existence of a base-specific system for the recognition of mismatched bases in intracellular DNA.

[Old and new aspects of childhood tuberculosis (author's transl)].

During the preceding 8 years 50 children were treated for active tuberculosis at the University of Innsbruck Department of Pediatrics. None of them were vaccinated against tuberculosis. Three children died; one of them from a fulminant miliary tuberculosis, two others from tuberculous meningitis; both of them were neurosurgically explored due to massive increase of intracranial pressure before a definite diagnosis was established. Two other patients, out of eleven with Tb-meningitis, are suffering permanent damages. Each one of the fatal cases and those with permanent sequelae could have been avoided by early diagnosis, which would have been possible in every case. It's our impression, that, with the decrease in the number of infected patients, there is also a decrease in the awareness and knowledge of this disease. We found the same to be true for the performance and interpretation of the Tb testing. Newborns immunized with BCG have to be tested for a positive tuberculine conversion to a dilution of Mantoux 1:100, preferably 3 months after the vaccination. Children with negative delayed sensitivity to tuberculin have to be reimmunized.

Tryptophan operon read-through. Isolation and characterization of an abnormally long tryptophan synthetase alpha subunit from a frame-shift mutant of Escherichia coli.

A new mutant strain of Escherichia coli, strain ICR-47, contains a frame-shift mutation in the trpA gene, the gene most distal to the operator in the trp operon. Mapping experiments indicate that the lesion is located at a site within 10 to 15% of the end of this gene. The mutation results in "out-of-phase" translation of the distal portion of the trp mRNA; normal translational termination signal(s) are not encountered and a trpA gene product longer than the wild type protein is produced. As with the other enzymes produced from this operon, the in vivo level of the altered protein (the alpha subunit of the tryptophan synthetase enzyme complex) is controlled by exogenous L-tryptophan. The altered alpha subunit from the strain ICR-47 has been isolated and characterized. Molecular weight estimations indicate a molecular weight of approximately 37,000, an increase beyond the wild type enzyme corresponding to an additional 50 to 70 amino acid residues. The protein has a new COOH-terminal amino acid sequence. Results of preliminary hybridization experiments suggest that the ICR-47 mRNA, which is necessarily longer than that needed to code for wild type enzyme, is not detectably different in size from wild type mRNA. The enzymatic properties of the ICR-47 alpha subunit indicates a greatly reduced ability of the mutant subunit to combine functionally with wild type beta2 subunit, the second protein component in the tryptophan synthetase enzyme complex. In contrast, only 40 to 50% of the intrinsic enzymatic activity of the alpha subunit is lost.

[Different maturation of leukemic blasts in tumor--like type of acute infantile leukemia (author's transl)].

In condition, it has been found that an acute infantile leukemia in its early stages, manifested itself clinically and morphologically as a reticulosarcome. Through cytochemical examinations, the malignant disease could be identified as an acute myeloid leukemia. The blasts in the tumourous tissues show less differentiation than the leukemic blood and bone marrow cells. The lack of maturation of the blasts is discussed as the cause of tumorlike cell growth.

[The behavior of the b-wave in the electroretinogram of light-adapted frogs during serial light exposure].

In light-adapted frogs (adaptation luminance 10(3) asb) repeated flashes of high intensity (10(5) asb) caused decrease of the b-wave in the electroretinogram as compared to the results obtained with dark-adapted animals. The amplitude changes were monotonous in all cases; they were dependent on the spacing and duration of the stimulus. With short intervals (2-10 sec) the amplitude adjusted to a new level no later than after 8 flashes. With longer intervals (30 and 60 sec), however, the amplitude decreased slowly and steadily over the whole period of experiment. Under these conditions, adjustment to a new level was prevented by disturbing factors, which additionally reduced the retinal sensitivity depending on the duration of the experiment.

[Electroretinographic study on the causal relationship of the S-R effect].

In the continuation of earlier studies, further serial-stimulation experiments with prolonged duration of single stimuli were carried out on dark-adapted frogs. The results have revealed a far-going dependence of the retinal response patterns (S-R effect) on the time relations between single stimuli and stimulation intervals. From all the experimental evidence available to date it can be concluded that the demonstrated serial-stimulation effects are brought about primarily in the photorecptors. According to reports in the literature these receptors in the state of dark adaptation respond to intensive, overriding stimuli with a pronounced after-effect causing a reduction of the temporal resolution capacity and of the difference susceptibility. The suppression effect can be regarded as a consequence of this overriding reaction. On the other hand, the recovery effect is caused most probably by the suddenly occurring bright adaptation because the latter makes the after-effect disappear quickly.
