[Control of respiratory therapy by means of rheography (author's transl)].

Rheography measures changes in electrical conductivity. In measurements taken at the thorax it was appeared that the changes in conductivity caused by breathing are superimposed by the changes caused by circulatory factors and exceed them in amplitude quite far. This makes the expansion of rheography for electrical respiration control possible. The changes in electrical conductivity are explained by inspiration into the alveoles, by the increase in the distance of the electrodes on thoracic expansion, and by the change in the intrathoracic blood volume. When using respiratory therapy the physiotherapist must pay great attention how the patient reacts to respiratory therapy and whether he is adapted correctly to the mechanical respiratory support devices. The method of "rheospirography" was tested as to whether it allows an objective, direct evaluation of the interplay between respiratory therapy and patient. The method appears to be able to control respiratory therapeutic measures as it allows an evaluation of the respiratory type, the regional ventilation, and the adaption of the patient to the device.

[Influencing of the kininogen-Kinin-System by means of a kallikrein-inhibitor during extracorporeal circulation in open heart surgery (author's transl)].

During extracorporeal circulation for open heart surgery an activation of the kininogen-kininsystem occurs with a decrease in plasma-kininogen-levels significantly exceeding the effect of haemodilution, and thus being due to an increased liberation of kinin. This effect could be suppressed by kallikrein inhibition by means of a protease-inhibitor.

[Ophthalmodynamic findings during cardiac surgery with extracorporeal circulation (author's transl)].

Neurological disturbances were seen in patients who underwent open heart surgery with prolonged extracorporeal circulation. These were found mainly in patients older than 50 years and when episodes of hypotension lower than 50 mm Hg occurred during cardiopulmonary perfusion. A method of recognizing cerebral hypoperfusion would be most helpful to prevent brain damage. According to the experimental and clinical findings of Hager the method of ophthalmodynamography allows some conclusion of analogy in relation to the reaction of the brain vessels. The intraoperative ophthalmodynamogram was recorded in 8 patients undergoing surgery using extracorporeal circulation. The ophthalmodynamograms seen during surgery on the beating heart showed changes associated with surgical manipulation. When the arterial pressure fell the orbital pulsation was decreased. The orbital pulsation stopped when the spontaneous heat beat was replaced by extracorporeal circulation. There was no change attributable to the perfusion pressure produced by the roller pump. Cessation of the orbital oscilogram during extracorporeal circulation does not indicate a cerebral hypoperfusion. No neurological lesion was seen in our patients. We were able to show that amplitude of the ophthalmodynamogram is influenced more by the blood pressure amplitued than the mean arterial pressure.

[Rheographic monitoring of lung ventilation (author's transl)].

Electrical conductivity changes are measured with rheography. Changes of conductivity on the thorax caused by respiration, superimpose and exceed those caused by different circulatory conditions. Therefore rheography was adapted for monitoring of respiration. Changes in electric conductivity can be explained by the inflow of air to the alveoli, by increase of the distance between electrode, and by changes of the intrathoracic blood volume. Rheography developed by Polzer and Schuhfried was tested in the intensive care unit and the respiration therapy department. It was shown that this method is suitable for control of respiration, for demonstrating regional differences of pulmonary ventilation, for checking of respiration therapy and for monitoring the ventilated patient.

[Pulmonary edema due to unilateral pneumothorax (unilateral shock lung) (author's transl)].

Unilateral pulmonary edema due to chest drainage is reported in the present paper. Drainage was carried out, because of pneumothorax. The disorder could be treated successfully with differenciated mechanical respirator therapy and additionally with methylprednisolon, heparin, spirolactone and dopamin. Based upon clinical symptoms and on present literature we suggest to account this so called "pulmonary edema exvacuo" to the main complex of acute respiratory insufficiency (unilateral shock lung).

[Measurement of bronchopulmonary elastance and airway resistance: comparison between common methods and automatic continuous measurement by the system AUPREM (author's transl)].

The system AUPREM is a new measurement device, which allows continuous measurement of the mechanical properties of the lung and bronchial system. Pulmonary elastance and airway resistance can be measured continuously even in extremly debilitated or unconscious patients. Transpulmonary pressure change, airflow and tidal volume are used as input. From these values airway resistance and pulmonary elastance are calculated. Comparative measurements in a group of healthy subjects and in a group of patients with pathological resistance and elastance values revealed that there was no significant correlation between the system AUPREM and the commonly used methods bodyplethysmography and elastance measurement by oesophageal balloon. However, the differences are in a low order of magnitude.

[Clinical and experimental results with acupuncture-analgesia (author's transl)].

Operations under acupuncture analgesia have been performed in Vienna since March 1972. A total of 102 procedures has been performed with "classical" Chinese acupuncture analgesia. Additionally given analgesics, psychodynamic factors and neurophysiological components are of joint importance. Our success rate in these cases was 64 p.c. Since October 1972 we call "two-phase-acupuncture-analgesia". Up to June 1975 72 operations were performed under some modifications of this method. In a neurophysiological study 17 volunteers and in 2 patients undergoing stereothalamotomies depression or even disappearance of evoked potentials was be observed during acupuncture with alectrical stimulation. Thus an interaction between pain stimuli and acupuncture stimuli in the thalamus could for the first time be demonstrated in man.

[On the application of electric stimulation currents in operations. Concept of a clinically applicable device (author's transl)].

With the use of high voltage electric currents in patients who initally have been anaesthetised, the anaesthesiologist faces new aspects. For this reason basic principles of physical medicine and medical engineering with respect to special problems are described. The majority of impulses current generators used so far do not even comply with the first principal requirement of any therapy namely knowledge of the dose of the agent applied. A system for electric currents crossing the body is outlined, which is characterized by constant current and isovoltage-free outlets in connection with a special ECG-amplifier and a control pattern. This apparatus for the first time guarantees maximal safety in clinical use. First animal experiments demonstrate the possibility of the risk if this method is applied in a inexperienced manner: in all animals ventricular fibrillation could be achieved when the electrodes were used in a position near the heart and there appeared to occur nonspecific reactions when current was directed through the brain (ear acupuncture)

Physical aspects of automatic respirators.

Objective of artificial ventilation is to maintain sufficient gas exchange with the lowest degree of mechanical burden to the lung that is possible. The basic principle of IPPV is a pressure gradient between the upper airways and the alveoli during inspiration, thus creating a positive gas flow to the lungs. Different technical arrangements can be chosen to overcome the resistances of the lung, depending upon whether the flow or the pressure is the underlying parameter for artificial ventilation. The driving system can be either compressed gas from a pipeline or an electrically driven pump or compressor. With the direct acting generator, the force of the driving mechanism directly inflates the lung, whereas the indirect acting generator has flexible coupling element between pump and patient. The switchover from inspiration to expiration is determined by the cycling parameter, uhich can either be the pressure, the flow, the volume or the time. The mode of cycling influences the reaction of the respirator to changing lung conditions. Only the volume and time cycled types maintain a constant ventilation volume with changing lung resistances. Lastly, the different behavior of respirators during expiration is discussed. An active support of expiration by means of a negative pressure has less meaning nowadays. On the other hand, a positive end expiratory pressure (PEEP) has turned out to be a very helpful tool in long-term ventilation.

Surface properties of the amniotic fluid in normal pregnancy.

The surface properties of the amniotic fluid were determined in 96 normal pregnancies between weeks 23 and 41. The y-max, y-min, hysteresis areaa, and stability index were used as parameters for evaluation of the surface tension are diagrams. These results were plotted against the gestation week and a curve for the normal course of surface tension of the amniotic fluid during the second half of gestation was obtained. The influence of addition of blood, vernix, and meconium to amniotic fluid as well as that of changes in the pH and dilution were examined. The results were unaffected by either blood, vernix, or pH changes. The slight scatter of the y-min values makes this parameter particularly suitable for determining fetal lung maturity.

[The influence of acupuncture analgesia on the sensation and feeling of pain and the autonomic state of the organism (author's transl)].

This study reports the effect of acupuncture on the sensation of pain, feeling of pain and on the autonomic state of the organism. In order to elucidate the problem physiological and psychological parameters were employed. It was observed that the insertion of the needles with electrical stimulation raised the pain threshold. The rise occurred whether a classical acupuncture point or a placebo acupuncture point was used. When the pain was assessed subjectively, it was found that the needling of a classical acupuncture point favourably influenced the experience of pain. Furthermore, persons in whom acupuncture worked well were characterized by a low subjective and objective pain threshold, a slow pulse rate and a high anxiety score.

Urinary catecholamine excretion and thyroid hormone blood level in the course of severe acute brain damage.

Urinary catecholamine excretion and thyroid hormone blood level were studied in 16 patients following severe cerebral trauma. Increased excretion rates of epinephrine and norepinephrine were found. There was no significant difference in the catecholamine excretion when compared with generally traumatized patients. The relationships between catecholamine excretion, increased metabolic rates, and negative nitrogen balance indicate that in patients with a midbrain syndrome there exists an additional diencephalic metabolic factor, which leads to a rise in fat oxidation and perpetuation of catabolism. Early high caloric parenteral nutrition seems to inhibit the initial increase of catecholamine excretion and thus protects the body from an unnecessary breakdown of its own reserves. If the course is classified according to neurological stages, it can be shown that patients with a traumatic apallic syndrome in poor condition have a high increase of catecholamine excretion. Secretion of thyroid hormones is not influenced significantly by cerebral trauma.

Telemetric ECG data (biorhythms) in operations with acupuncture analgesia in the People's Republic of China.

During seven operations with acupuncture analgesia in the People's Republic of China, recordings were made with a self-developed device for telemetric transmittance of the ECG. Among others, the amplitude of the QRS complex as well as the period of time between two cardiac cycles were electronically measured and registered for evaluation. Through comparison of the values from China with our own material, the following statements can be made: (1) All patients were in conditions ranging from restlessness and slight excitation to weariness and drowsiness. A condition similar to narcosis must be excluded. (2) In a large number of cases a preoperatively performed strong psychological sedation through pharmacological agents can be excluded with certainty. (3) The massive reaction to events such as the cleansing of the operative field refutes-at least partly-our concept of "Asiatic indifference". (4) Acupuncture cannot depress reflexes. (5) Finally, the hypothesis that acupuncture has a stabilizing effect on the circulation is invalid.

[Telemetric ECG data (biorhythms) during operations under acupuncture analgesia in the People's Republic of China (author's transl)].

On an educational journey to China, we were able to record and accumulate electrocardiograms during operations under acupuncture analgesia with the aid of a self-developed apparatus. To elaborate this among others the amplitude of the QRS-complex as well as the time period between two cardiac cycles were electronically measrued and registered. With examples from the comprehensive material it is possible to draw the following conclusions: 1. The patients were in conditions ranging from restlessness and irritation to weariness and drowsiness. A condition similar to that resulting from narcosis must be excluded. 2. A pre-operatively undertaken strong psychological sedation by means of drugs can be excluded with certainty. 3. The massive reaction to events such as the washing out of the operative field refutes, at least partly, our concept of "Asiatic indifference". 4. Acupuncture is not able to depress reflexes. 5. Finally, the hypothesis that acupuncture has a stabilizing effect on the circulation is not valid.
