Noninvasive measurement of diameter changes in the distal abdominal aorta in man.

An ultrasound phase-locked, echo-tracking system was used to determine the dynamic properties of the distal abdominal aorta in 10 Caucasian male subjects (mean age, 25 years). Recordings were made at rest and during the blood pressure increase resulting from isometric exercise. The pressure diameter curve was nonlinear with an inflection at about 90-110 mmHg. Above this pressure range, the vessel was stiffer (less compliant), but the pressure diameter relationship was roughly linear above as well as below the inflection. Individual pressure diameter curves showed hysteresis, i.e., the aorta had a smaller diameter during expansion than during retraction at corresponding pressures. The pressure strain elastic modulus (Ep) and stiffness (beta) were at rest [Mean Arterial Pressure (MAP), 81 mmHg] 0.70 10(5) N/m2 and 6.0, respectively. During isometric exercise (MAP, 122 mmHg), Ep increased significantly by 91% and stiffness (beta) nonsignificantly by 27%. The variability of the compliance determinations was 5% when the ultrasonic system was combined with intra-arterial blood pressure measurements and less than 7% when combined with auscultatory blood pressure measurements. It is concluded that the phase-locked, echo-tracking system fulfills clinical requirements for routine measurements of vascular compliance.

Diameter and compliance in the male human abdominal aorta: influence of age and aortic aneurysm.

In this study changes in the diameter and compliance of the distal abdominal aorta (76 healthy Caucasian males, 5-71 years old) were determined non-invasively and related to age by means of an ultrasound phase-locked echo-tracking system. The diameter of the aorta increased not only in the period between 5 and 25 years of age, but also by about 30% between the ages of 25 and 71. The pressure diameter curves at 25, 51 and 70 years were non-linear with flattening between 90 and 110 mmHg, and the slope of the curves declined with age. Thus, the pressure strain elastic modulus (Ep) and stiffness (beta) increased (i.e. compliance decreased) in an exponential manner according to age. A group of 37 males with aneurysmal widening of the distal abdominal aorta had a significant increase in Ep when compared to an age-matched control group. Furthermore, the ranges for both Ep and beta were much larger in the aneurysm group than in the control group, indicating diversity in the pathogenesis of the disease. The non-invasive ultrasonic method of phase-locked echo-tracking is an appropriate method for studying compliance in major arteries under a variety of pathophysiological conditions.

Increasing prevalence of abdominal aortic aneurysms. A necropsy study.

OBJECTIVE: To analyse the prevalence of abdominal aortic aneurysms in the city of Malmö. DESIGN: Retrospective demographic study. SETTING: Malmö General Hospital. MATERIAL: Reports of all 45,838 necropsies done at the Department of Pathology, Malmö General Hospital 1958-1986. RESULTS: Abdominal aortic aneurysms were found in 4,300/100,000 men and 2,100/100,000 women. The mean annual age-standardised increase of aortic aneurysmal disease was 4.7% among men and 3.0% among women. The prevalence among men increased rapidly after the age of 55 and reached a peak of 5.9% at the age of 80; that among women increased after the age of 70 and reached a peak of 4.5% above the age of 90 years. CONCLUSION: The prevalence of aortic aneurysmal diseases has increased during the last three decades, and is age and sex-dependent.

A population based screening of abdominal aortic aneurysms (AAA).

The prevalence of abdominal aortic aneurysm (AAA) in a defined population of elderly men and its correlation to some risk factors were studied in the population cohort "men born in 1914 from Malmö". A total of 499 were invited to attend and 375 (75%) did so. The aorta could be visualised with ultrasound in 364 patients, 39 (10.7%) of whom had aneurysmal changes. The presence of an AAA was related to the findings at a general health examination undertaken 5 years previously. Tobacco and alcohol consumption, impaired lung function and a history of angina pectoris were related to the presence of an AAA. No relationship was found between an AAA and hypertension, hyperlipidaemia or hyperglycaemia. A decreased tissue elasticity as a common denominator for the lung function impairment and development of AAA is discussed.

Relationships between perceptual defence reactions and subjective tendencies in the phenomenal representation of facial displays of affect.

The present study examines if non-veridical reports of a stimulus-motif presented below the threshold for correct recognition are related to subjective tendencies in the ordinary perception of thermatically related stimuli. Sixty-four subjects were tested with the standard picture of Kragh's Defence Mechanism Test (DMT) which reflects a theme of aggression and threat. Stimulus-inadequate report of the picture-motif were related to the subject's phenomenal representation of video-recorded facial displays of anger, fear, sadness, and joy. The results demonstrate a correspondence between the individual's perceptual distortions, eliminations, or additions to the threatening subliminal stimulus and his or her phenomenal representation of the supraliminal affective stimuli.

Ultrasonographic screening of the abdominal aorta among siblings of patients with abdominal aortic aneurysms.

To evaluate the prevalence of abdominal aortic dilatations among asymptomatic brothers and sisters of patients with abdominal aortic aneurysms (AAAs), an ultrasonographic screening study was performed. One hundred and two siblings of patients operated on for AAAs at two Swedish hospitals were invited to attend, and 87 of them (35 men and 52 women) from 32 different families, accepted the invitation. Their median age was 63 years (range 39-82 years). Aortic dilatation was diagnosed in ten of the brothers (29 per cent) and three of the sisters (6 per cent). In ten cases (eight men and two women) there was a localized dilatation caudal to the coeliac axis, and in three a general dilatation of the abdominal aorta with the diameter at the coeliac axis greater than 29 mm. None of the aortic dilatations had been known before this study was performed. The conclusion is that the prevalence of asymptomatic aortic dilatations among brothers of patients with AAAs seems to be high and that this group should be selected for further screening studies.

Ultrasound screening of the abdominal aorta in patients with intermittent claudication.

The presence of abdominal aortic dilatations and aneurysms (AAA) among 372 patients (302 men and 70 women) who originally presented with intermittent claudication was studied. The cohort was analysed in two ways, first retrospectively from the date of diagnosis of intermittent claudication until 1st of August 1985 (mean follow-up time being 70 months), second those alive at that date and who had not been operated on were offered ultrasound screening of their aorta. Retrospectively nine male patients were found to have had an operation for an aneurysm, one because of a rupture. Of the 110 patients who died and had not had surgery, 88 (73 men, 15 women) were autopsied and AAA was found at autopsy in six males and one female. Two males died of rupture. Ultrasonographic screening of the abdominal aorta was performed in 183 patients (147 men and 36 women) and dilatation was found in 25 patients (24 men, 1 woman). In the male part of the total material (n = 257) there were 39 patients (15%) with aneurysm or dilatation. Male patients with AAA or dilated aortas were significantly heavier, with a lower ankle arm index and higher serum cholesterol values than patients without AAA or aortic dilatation.

Solitary aneurysms of the iliac arterial system: an estimate of their frequency of occurrence.

Solitary iliac artery aneurysms are rare, and most reports have been presented as case reports. By combining autopsy records and operating records, a total of 13 cases were found: during a 15-year period (1971 to 1985), 42,010 of the inhabitants of Malmö died (population 230,000) and 35,265 (including 9014 forensic autopsies) underwent autopsy (84%). Solitary iliac artery aneurysms were found in seven (0.03%) of the 26,251 patients who underwent autopsy at the hospital; six of those had been asymptomatic and one was ruptured. Among the 9014 persons who underwent forensic medical autopsy, there were two with ruptured solitary iliac artery aneurysms. Four patients had clinically detected solitary iliac artery aneurysms, three of which were ruptured. All patients underwent surgery, and two of the three patients with ruptured solitary iliac artery aneurysms left the hospital well. The rupture rate of iliac aneurysm among those found at autopsy was one of seven (14%) and among those clinically detected three of four.
