Studies on the in vitro development of rat peritoneal mast cells.

Mast-cell-depleted rat peritoneal exudate cells have been shown to differentiate into pure mast cell cultures when placed in a special medium containing 15% horse serum and 20% L-cell supernatants. In the present communication, the changes that occur within the cells during culture are examined by several parameters. Prior to culture, the cells resemble mononuclear phagocytes morphologically. During culture, their total cell volume and cytoplasmic mass increase, and they develop metachromatic, alcian blue and later on safranin-positive cytoplasmic granules. Proliferation of the cells, as shown by 3H-thymidine incorporation, is optimal between days 3 to 11, and is decreased by changing the composition of the medium or by adding high concentrations of histamine (greater than 10-6M) and heparin (greater than 50 millimicron/ml). By cytochemical staining and by analysis of subfractionated cellular components, eosinophil chemotactic factor (ECF), histamine, beta-glucuronidase, alpha-naphthyl acetate esterase and myeloperoxidase are demonstrated, mostly within the granules. The findings suggest a close morphological and functional relationship between peritoneal mast cell precursors and mononuclear phagocytes.

Urticaria pigmentosa: clinical picture and response to oral disodium cromoglycate.

Fifty-six children and adults with urticaria pigmentosa were examined and their symptoms evaluated. Thirteen patients with cutaneous and systemic symptoms consented to participate in a blind trial and were treated for 1 month with placebo and for another month with oral disodium cromoglycate (DSCG). More than 66% of the patients noted relief of pruritus and gastrointestinal symptoms with DSCG. Reduction of wealing was less prominent. Patients have been continued on 400-800 mg of DSCG daily for up to 15 months with continuous improvement of their symptoms and no adverse effects.

[The diagnostic and therapeutic value of second look operation in patients with testicular germinal cell tumors (author's transl)].

We report on 32 patients with testicular tumor who--after earlier retroperitoneal lymph node dissection--underwent a second-look operation. Whereas in 16 patients vital tumor tissue was found in the retroperitoneum 9 patients had only degenerative changes with fibrosis and necrosis. In 7 patients the retroperitoneum showed no evidence of disease. It is our conclusion that an extensive pre-operative diagnostic workup, including tumor-markers, ultrasound and CT in patients treated with chemo- and radiotherapy cannot determine whether viable tumor tissue is present in the retroperitoneum. In 13 patients with non-seminomatous germinal cell tumors where viable tumor tissue was found at second-look operation 61,5% had normal tumor-markers. Those patients in whom at second-look operation no viable tumor tissue was found benefit from that approach because of the possibility of terminating aggressive chemotherapy. On the other hand patients with vital tumor tissue had a poor prognosis.

Rhabdomyosarcomas: chemotherapy and limited supplementary treatment program to avoid mutilation.

A treatment program was set up to minimize mutilation in children with rhabdomyosarcomas (RMS) in the otorhinolaryngeal region, the urogenital tract, and other regions where surgery and radiotherapy (RT) could be mutilating. The primary treatment was chemotherapy with vincristine, dactinomycin, and cyclophosphamide, and each patient was assessed individually to determine whether supplementary treatment was needed. The initial results were so promising that a controlled clinical trial was undertaken by the International Society of Paediatric Oncology to compare the effects of chemotherapy alone versus chemotherapy and RT on the original tumor volume. At present, the place of irradiation in the treatment of RMS remains uncertain. It is unavoidable only in those patients who are likely to develop intracranial extension, because chemotherapeutic agents do not penetrate the blood-brain barrier.

[Analysis of vesico ureteral reflux by functional scintigraphy (author's transl)].

With a large view scintillation camera and gamma 11/34 DEC-System it is possible to detect a reflux out of the bladder, if the volume is 0.2 ml or larger. Description of the function of upper urinary tract is given. Time of reflux beginning a bladder volume combined with measurement of intravesical and intraabdominal pressure and more details of the reflux dynamic are demonstrated. Background activity and absorption of body soft tissue should be corrected by phantom measurement. The results of investigations of 26 patients are shown in tables and by examples of morphologic and functional analyses.

[Surgical procedure for salmonellosis of the urogenital tract (author's transl)].

A 45-year-old man with multiple abscesses caused by Salmonella typhi murium in the left kidney is presented. The patient had bilateral polycystic renal degeneration and terminal renal insufficiency. The illness manifested itself clinically as a case of septicaemia which did not respond to antibiotic treatment, and pain in the left flank. There was concomitant haematuria. The patient was treated with dicumarol for thrombosis of the Chimino shunt. The symptoms receded following nephrectomy. The patient is to be considered cured in the absence of positive findings in the urine, blood, sputum, gastric juices or faeces.

Palmar flexion creases in childhood neoplasia.

About 90% of Caucasian individuals in the general population may observe two transverse palmar flexion creases when their fingers are slightly bent. A small minority may find in one hand a single transverse crease or the usual two creases, of which one seems to cut across the palm to the ulnar margin. Those unusual creases are called simian- or Sydney-creases, respectively. Normal healthy people hardly ever observe them in both hands. We observed those unusual creases, however, in more than 50% of children suffering from different type of malignant neoplasia, quite often in both hands. The difference between patients and controls of similar ages is highly significant. Among patient with childhood acute lymphocytic leukemia the variant creases were mostly of the Sydney type. They most frequently occurred in those patients in whom the disease had become manifest at an early age. Since fathers as well as mothers of the patients showed significantly higher frequencies of unusual creases, the phenomenon seems to be a familial one. A most singular effect is the striking incidence of those creases in younger siblings of the patients. With regard to crease variance our data are essentially similar for lymphoproliferative disorders (ALL and NHML) and embryonic malignant tumors. This might be the first indication of a common host factor in patients with ALL or embryonic tumors occurring in early childhood. We have postulated that this factor may be a regulatory one associated with cellular growth and differentiation in early fetal palmar pads as well as with cell-mediated immune response to early pediatric tumors. The observation of palmar flexion creases may prove to be rewarding in future studies of cellular defense mechanisms in young patients with neoplasia.

Ultrastructure of isolated human mast cells during histamine release induced by ionophore A 23187.

In the presence of calcium, the ionophore A 23187 (10(-7) TO 10(-6) M) causes a dose-dependent histamine release from isolated human mast cells. The accompanying degranulation process is characterized by a formation of channels of fused mast cell granules and by an exocytotic extrusion of altered granule material. Simultaneously, large numbers of newly formed 70 A filaments occur. These filaments probably have a key function in secreting human mast cells.

[Diffuse cutaneous mastocytosis in childhood].

We report the case of a 15-month-old child suffering from diffuse cutaneous mastocytosis characterized by blisters and widespread skin involvement but without systemic manifestations. Skin biopsies were examined by electron microscopy. We observed various alterations in the morphology of mast cells and mast cell granules.
