Cerebral atrophy in Parkinson's disease--represented in CT.

To clarify the importance of brain atrophy in relation to the symptoms of Parkinson's disease, 173 patients were examined by computed tomography (CT). In 51.4% of the CT findings, brain atrophy was considered to be pathological. Statistically significant relations of age and sex were found with regard to the extent and localization of brain atrophy. Cortical atrophy also showed a significant dependence on duration of disease. Linear measurements at the lateral ventricles and the third ventricle lead us to assume that brain atrophy in Parkinson's patients is more prevalent than in normal patients within the scope of age involution.

The course of brain atrophy in Parkinson's disease.

In 110 parkinsonian patients (53 men, 57 woman) aged 38--81 years, computer-tomographic follow-up investigations were done to assess the development of brain atrophy. The control examinations were done after an average of 28 months. At that time an increase in brain atrophic changes of different localization could be observed in 23% of the patients. In addition, it could be demonstrated that the increase in pathologic CT findings is to be observed especially in patients with higher age, a more marked impairment in psycho-organic capacity, more pronounced handicaps in the fine-motorial performances at the beginning of the study. From the neuroradiological point of view, patients with more marked pathologic CT findings upon the first examination, be these ventricular enlargement and/or cortical atrophy, more often showed a progression of brain atrophy.

[Reintervention after choledocho-duodenostomy].

Shrinkage of the anastomosis and blind loop syndrome with impacted stones or sludge in the retroduodenal common bile duct caused by papillary stenosis are the main reasons for an ineffective choledochoduodenostomy. The clinical picture is that of an ascending cholangitis. 30 choledochoduodenostomy repairs were performed in our hospital. The operative treatment is described in detail. In a follow-up of 30 patients there was only one case with a persisting chronic cholangitis.

[Therapy of so-called shock lung. Experimental animal studies of the lung in shock].

1. Under hemorrhagic-traumatic shock conditions the depression of the flow and the pressure in pulmonary arteria and of the pressure in the left atrium was significant. The elevation of the pulmonary vessel resistance was significant, the pulmonary circulation time was longer. The depression of the oxygen partial pressure in arterial and venous blood was significant. 2. In comparison to single re-transfusion of the taken blood, the re-transfusion and application of 6-Methylprednisolone (Urbason; 30 mg/kg) could raise the pulmonary pressure and the flow in the pulmonary arteria and the oxygen pressure in the arterial and venous blood over their basic level. 3. After re-transfusion and simultaneous addition of phenoxybenzamine a renewing shock took place caused by a relative hypovolemia. 4. The hemodynamic circulation in the pulmonary and peripherial vessels was restored by hemodilution. While a single infusion of Haemaccel intensified the hypoxemia in comparison to shock conditions, the infusion of albumin (4%) caused no change in measured values. The application of Urbason (30 mg/kg) combined with Haemaccel-infusions raised the arterial and venous oxygen pressure significantly, in comparison to the hypotonic-phase they exceeded the basic values.

[Prenatal diagnosis of trisomy-18-fetus of two women with chromosomal aberrations (author's transl)].

In two pregnant women a prenatal diagnosis was performed because of their advanced age. In both cases trisomy 18 was found. The mothers had chromosomal variations, not known until then. In one woman a structural aberrant chromosome No. 9 was found, the other woman had a gonosomal mosaic. In connection with the advanced maternal age these chromosomal findings possibly induced the trisomies in the children.

Production of interferon in the murine mixed lymphocyte culture. II. Interferon production is a T cell-dependent function, independent of proliferation.

Interferon production occurs after two days of culture in murine mixed lymphocyte cultures (MLC). This was demonstrated in various combinations of mouse spleen cells differing at the major histocompatibility (H-2) locus. Interferon production could be demonstrated in one-way MLC when F1/parent combinations were used and in reactions in which one partner was treated by puromycin. After treatment of both cell populations with mitomycin C, interferon production occurred in the absence of lymphoproliferation. Interferon production in response to alloantigen did not occur in spleen cell cultures of nude mice and in cultures treated by anti-theta antiserum plus complement indicating that interferon production is a T cell-dependent function.

Cerebral atrophy and long-term response to levodopa in Parkinson's disease.

In 92 parkinsonian patients (42 men, 50 women) aged from 37-79 years (mean 62.8) the impact of cerebral atrophy as assessed by computed tomography on the course of the clinical symptomatology under levodopa during a period of 1 to 5 years was investigated. It could be shown that patients suffering from medium to severe degrees of atrophy-independent of its special location--have a less favorable response than those with normal CT findings. The data do not allow a definite decision whether cortical artrophy or ventricular enlargement are of major importance. At any rate, the width of the third ventricle seems to have no influence on the course of the clinical symptoms. After 3 years of levodopa treatment a dissociation between cerebral atrophy and therapeutic effectiveness can be observed.

[Studies of gastric circulation in the dog, II. Ligation of main arteries (author's transl)].

Following ligation of a single gastric artery total and regional gastric blood flows are measured using radioactive microspheres of 8 mu size. There is no significant influence on total gastric blood flow. Within the depending anatomical region no significant reduction of total wall blood flow can be determined. However, blood flow to the antrum mucosa is reduced to approximately 40% after ligation of any gastric artery. Thus, it can be concluded that ligation or embolisation of a single gastric artery in diffuse gastric bleeding does not necessarily reduce or terminate the bleeding.

[Studies of gastric circulation in the dog. III. Remaining blood flow in the dissected stomach (author's transl)].

After mobilisation of the stomach for high anastomosis between esophagus and stomach, gastric blood flow is maintained by only one gastric artery. Total and regional gastric blood flows are measured in 13 dogs using the microspheres method after ligation of three main gastric arteries. In any part of the stomach blood flow does not decline below 40% of the initial blood flow. Blood flow to mucosa of corpus and fundus is hardly reduced even after this drastic reduction of total gastric blood flow. Blood flow in the muscle layer of the fundus is reduced to about 38% if supplied only by the right gastroepiploic artery. Different operative procedures of replacements of the esophagus are discussed.

[Gastric blood flow in animals. I. Normal blood flow (author's transl)].

Total and regional gastric blood flow were measured in 15 dogs under control conditions (mean AOBP 100 mm Hg) using microspheres labelled with different isotopes (125J, 141Ce, 85Str, 46Sc) of 8 mu size. The mucosa in fundus and corpus showed significantly higher blood flow compared to the antrum. There was no difference in the muscular layer for either part of the stomach. It is concluded from these data that differences in regional blood flow in the unstimulated stomach of the dog are related to different energy-demands within the stomach.

The significance of cerebral atrophy for the symptomatology of Parkinson's disease.

Hitherto published results on the impact of brain atrophy on the neurological and psychopathological sympion of this problem without risk of complications. We investigated 173 parkinsonian patients (89 men, 84 women) aged 37--83 years. Besides CT in all patients a standardized neurological and psychopathological investigation was carried out, including psychological tests, intelligence, personality structure, fine motor performance and visual reaction times. The investigations revealed pronounced correlations between CT-findings and more severe clinical symptomatology and a corresponding impairment in daily activities in the levodopa-treated and particularly in the untreated group, above all when there was a combination of cortical atrophy and ventricular enlargement. The same was true for fine motor performance and simple and complex reaction times. These functions were more impaired in patients with brain atrophy, irrespective of its localisation, whether cortical atrophy or ventricular enlargement. In contrast, no statistically significant relationship between the different parameters of brain atrophy and intelligence could be found. Considering cortical atrophy as a specific sign in Parkinson's disease -- our results are in favour of this assumption -- and ventricular enlargement more related to increasing age, parkinsonism is influenced by extranigral lesions inherent in the disease and dependent on increasing age.

Fibromatosis of the upper limb.

Two cases of fibromatosis of the upper limb are presented, neither of which could be placed in a specific category as described in the literature. Both patients were black, and both had a history of previous trauma to the region, which has been regarded as an initiating factor. Emphasis is placed on conservative treatment in order to maintain hand function as long as possible.

Shunt metastases in posterior fossa tumors.

Permanent (CSF-drainages for treatment of hydrocephalus in posterior fossa tumors have been used in the last few years, increasingly. The rare incidence of extracranial metastases and it's higher frequency in the same period of time has been remarkable. An additional case of metastasis via ventriculoperitoneal shunt is presented. Such deleterious complications of malignant CNS-tumors should be avoided, as progressively improving results with combined radio-chemotherapy have established a more encouraging prognosis.
