Plasma catecholamines in arterial and capillary blood.

Adrenaline and noradrenaline concentrations in arterial and simultaneously collected "arterialised" capillary plasma (vasodilated ear-lobe) obtained from unmedicated resting subjects (n = 9) were measured radio-enzymatically and found to be indistinguishable (adrenaline: 112 +/- 42 ng/l versus 109 +/- 52 ng/l; noradrenaline: 378 +/- 174 ng/l versus 410 +/- 219 ng/l; mean +/- SD; paired t-test: P greater than 0.10). The fitted regression lines did not differ significantly from the line of identity (slope = 1). The correlation coefficient was 0.969 for adrenaline and 0.945 for noradrenaline. Adrenaline and noradrenaline concentrations in plasma from freely flowing capillary blood (non-hyperaemic fingertip) were very similar to those in arterial plasma (adrenaline: 101 +/- 89 ng/l versus 90 +/- 72 ng/l;noradrenaline: 399 +/- 240 ng/l versus 395 +/- 240 ng/l; n = 13 each; paired t-test: P greater than 0.10). The correlation coefficient was 0.981 and 0.917 for adrenaline and noradrenaline, respectively. Adrenaline concentrations in capillary plasma (vasodilated earlobe), however, were significantly higher than those in venous (forearm) plasma (100 +/- 51 ng/l versus 61 +/- 23 ng/l; n = 15; P less than 0.01; capillary-venous difference = 35 +/- 18%) while noradrenaline concentrations did not differ significantly. We conclude that capillary plasma resembles arterial plasma in its catecholamine content and we show that arterial catecholamine levels may be determined in capillary, especially "arterialised" specimens, eliminating the need for arterial blood sampling.

[Diflunisal compared with naproxen in chronic polyarthritis. Double-blind study].

Diflunisal, a new analgesic/anti-inflammatory monosubstance, has been compared with naproxen in a double-blind study in patients with rheumatoid arthritis. In doses of 1 g diflunisal daily and 750 mg naproxen daily both therapies resulted in significant improvements in subjective and objective parameters measured - day and night pain, morning stiffness, Ritchie index, grip strength, and doctor and patient assessment of overall effect. Side effects were uncommon and of little significance in the diflunisal-treated patients, whereas two patients in the naproxen group were dropped from the study because of clinically significant drug-related side effects. It is concluded that diflunisal should be a very useful addition to the therapies currently available.

[Proximal selectivive Vagotomie. Intra-operative Vollständigkeitskintrolle].

In the treatment of peptic gastroduodenal ulcers the modified vagomotor electrotest according to Burge has yielded remarkably good results as an intraoperative test for the completeness of proximal selective vagotomy (PSV). The test is as useful for the surgeon in training as for the expert in vagotomy. Moreover the clinical value of the test is demonstrated. If, according to this test, vagotomy is complete, an extreme reduction of gastric acid and a very low rate of relapsing ulcers are seen in our patients. We cannot recommend performing the PSV without this test.

Effect of sodium nitroprusside on myocardial performance and venous tone.

The action of sodium nitroprusside upon the mechanical performance of the isolated, working rat heart and on isolated bull veins has been studied. Cumulative concentrations of 1--1000 ng sodium nitroprusside per ml had no effect on left ventricular pressure, end-diastolic left ventricular pressure, dp/dtmax, cardiac output and calculated wall stiffness of isolated rat hearts with constant rate, preload and afterload. Isolated strips of bull veins contracted with noradrenaline showed a dose dependent relaxation by sodium nitroprusside in concentrations of 5--1000 ng per ml. This study shows that sodium nitroprusside has no direct effect on myocardial contractility and causes venous relaxation.
