[A Technic of re-using a thrombosed hepatic artery in hepatic intra-arterial chemotherapy].

Nowadays the palliative treatment of hepatic metastasis very often uses the hepatic arterial chemotherapy in subcutaneous injection cavity. Unfortunately in about ten per cent of the cases, the thrombosis of hepatic artery limits the lasting quality of use of the catheters which last about twelve months (more or less four). As mentioned in our observation, if the patient "answers" the chemotherapy, we propose reusing this thrombosized artery by means of an original technical artifice. In this observation, in fact, we grafted a small segment of submesenteric vein on the hepatic disobstructed artery. We think that this artifice could also be used in case of anatomic variations of the hepatic artery like trifurcation.

[Congenital diverticulum of the bile ducts. Apropos of a case and review of the literature].

The authors report a case of Congenital Diverticulum of the common bile duct treated by surgery. Literature reviews are giving detailed reports on this rare affection which falls within the scope of Congenital Diverticulum of the common bile duct, it has only a small share in this report. The comments relate the anatomic, pathogenic, diagnostic and evolutive data. The treatment appears to be possible in any case: surgical exeresis of the diverticulum.

[Lymph node-hepatosplenic hemangioma in an adult with consumption coagulopathy and fatal cardiac insufficiency].

Hemangioendothelioma is seldom seen in adults. Its severe evolution is due, not so much to the exceptional transformation into hemangiosarcoma, but mainly to haemorrhagic complications by rupture or consumption coagulopathy and to severe cardiac insuffficiency secondary to arteriovenous shunts. The case reported here concerns a 64 year-old woman presenting pain in the left hypochondrium and splenomegaly. A splenectomy was performed and the histological findings were compatible with the diagnosis of hepato-splenoganglionic hemangioendothelioma. The evolution was unfavorable. The patient died a few months later in a picture of haemorrhagic syndrome and cardiac insufficiency. Histological findings on autopsy specimens indicated a cavernous hemangioma. The treatment of these diffuse hemangiomas is a difficult one. Hepatic artery ligation has been advocated in certain desperate situations. Nevertheless, because of a collateral circulation, recurrences are frequent.

[Free tumors of the peritoneum. Review of the literature apropos of a case].

Free bodies in the peritoneum are observed infrequently, particularly large solid tumors as in the present report. Of slightly more frequent occurrence are detached epiploic appendices or cystic tumors. Symptomatology varies greatly according to whether organs are compressed or irritated by the free body. Diagnosis is always preoperative, and the postoperative course simple because the tumor is delivered without dissection or ligature. Prognosis is good since these tumors are benign. Histopathology demonstrates origin and histologic nature of free body, and operation for a "chronic uterine fibroma" in the present case revealed a large free tumor. Operative findings (absence of right appendage) and histology suggested a diagnosis more likely to be of ovarian fibroma with a ruptured pedicle from chronic torsion. Despite the advanced age of the patient (96 years) no postoperative complications were reported.

[Localized intestinal lymphangiectases presenting as peritonitis].

The authors report a case of localised intestinal lymphangiectases in a segment of jejunum with the unusual presentation of an acute perforation. The anatomical findings did not provide a satisfactory aetiopathogenic explanation, although the most likely hypothesis is a past history of abdominal irradiation. The mechanism of the perforation also remains unclear and the presence of microscopic lesions in the wall of the remaining small intestine raises the problem of the long-term prognosis.

[Intraperitoneal dirofilariasis].

Discovery of Dirofilaria (Nochtiella) repens during a pancreatectomy in the adipose tissue of the pancreas. Brief description of the worm and transverse sections which show longitudinal ridges of the cuticule (photo no 4). Clinical history, pathological anatomy study of liver; mesenteric infarction and patient's death two weeks after the operation. Parasitosis known in the South of France near at hand littoral pools but any visceral topographical diagnosis had never been signaled in France.

[Sexual disorders following abdominoperineal resection of the rectum].

The authors analyse the results of a survey of 450 patients who had undergone abdominoperineal resection. 142 replies were exploitable. After recalling the analytical criteria, they specify the incidence of sexual dysfunctions by sex and causal disease. The comparison of the results with those of other series leads to certain conclusions which will be better appreciated after a brief recapitulation of the pelvic nerve supply and the physiology of the sex function. The frequency of these disorders and their effect on the psychological and affective balance of the patient should prompt surgeons to question their patients preoperatively about their sexual lives, and to give preference, for this and other reasons, to anterior resection whenever this is possible and carcinologically sound.

[A case of barium-peritoneum with neurological involvement. Importance of barium determination in biological fluids].

Barium in the peritoneum following a barium enema led to a severe and irreversible chronic barium poisoning, although from the abdominal and gastro-intestinal point of view, there were virtually no sequelae. Barium assay on the cerebro-spinal fluid of this patient revealed excessively high levels, which provides the laboratory evidence of the direct action of barium on the central nervous system. Barium assays on biological fluids after barium-peritoneum could be used to assess the degree of barium poisoning and may have prognostic value.
