Intracytoplasmic lumina in breast carcinoma: a helpful histopathologic feature.

Eelectron microscopy has disclosed the presence of intracytoplasmic lumina within breast cancer cells. These structures can be recognized with the light microscope by their sharp, round outlines and thick walls. Their identification in large numbers may provide additional support for the breast origin of a metastatic tumor. Three illustrative cases in which demonstration of intracytoplasmic lumina was diagnostically helpful are presented.

Electron microscopy in the diagnosis of head and neck tumors.

Six patients with malignant head and neck tumors are shown to have required electron microscopy for accurate diagnosis. In all of these tumors, there were ultrastructural features of cytodifferentiation that were not discernible by light microscopy, such as neurosecretory granules, desmosomes, cytoplasmic processes, tonofibrils, and myofilaments. Electron microscopy is helpful in the differential diagnosis of tumors in general, but its effectiveness is particularly apparent in small-cell "undifferentiated" tumors such as neuroblastoma, rhabdomyosarcoma, Ewing's sarcoma, undifferentiated squamous-cell carcinoma of the lymphoepithelioma type, and malignant lymphoma. It has also been helpful in the identification of amelanotic melanoma and spindle-cell carcinoma.

Myxoid liposarcoma: study of ten cases.

The ultrastructure of 10 myxoid liposarcomas from 9 patients is presented. Features closely resembling those of developing fetal adipose tissue were seen in all tumors. THe various cell types identified ranged from undifferentiated cells to typical multivacuolated and univacuolated lipoblasts. In addition, a secretory type mesenchymal cell with distended cisternae of rough endoplasmic reticulum was found in every case as well as intermediate cells with secretory product and lipid accumulation. relative proportions of these types varied from case to case. A prominent feature in all cases was the particular spatial relationship of the abundant capillaries to the various cell types. The undifferentiated cells had perivascular processes with a tendency to envelop the primitive capillaries. These cells progressively enlarged and acquired features of the differentiated cell types as they separated from the lamina. Mitotic activity, when detectable, was prominent in the capillary wall and its vicinity. Lipoblasts resembling those of brown adipose tissue frequently coexisted in the same tumor with others analogous to those of white adipose tissue. Our findings suggest that tumor cells in liposarcoma originate from a dividing population of perivascular mesenchymal cells, in a manner akin to that of developing white adipose tissue.

Epithelioid schwannoma: diagnosis by electron microscopy.

The clinical, gross, and light microscopic features of two epithelioid tumors were not typical of either benign or malignant schwannoma. Both tumors displayed long, enveloping cytoplasmic processes that were outlined by layers of discrete basal lamina. Long-spaced collagen was found in one case. These well-developed electron microscopic features are those that are commonly encountered in benign Schwann cell tumors. Although one patient died postoperatively, the other is alive and well 12 months after surgery. The extent to which ultrastructural data can have an impact on the evaluation of Schwann cell tumors is unknown but requires further study.

Lennert's lymphoma with giant multivesicular lysosomal bodies optically visible.

A case of a 43-year-old male patient with lymphoepithelioid lymphoma is discussed. During the 10-year follow-up of this patient, different biopsy specimens were obtained. Immunohistochemical studies with a complete paraffin panel determined a T-cell nature of the specimens. Electron microscopy showed a peculiar finding in the cytoplasms of the tumoral cells that correlated with optical inclusions observed in optical imprints; these imprints corresponded to lysosomic multivesicular bodies, whose significance is discussed.

Improved apparatus for spinal fluid cytomorphology.

The design of an apparatus for spinal fluid cytomorphology is described. It represents an improvement over previous devices using the enhanced sedimentation principle and has given reliable clinical results over a three year period.
