Antibody to glycolipid in a patient with IgM paraproteinemia and polyradiculoneuropathy.

Serum from a patient with IgM paraproteinemia and polyradiculoneuropathy, diagnosed as malignant lymphoma reacted specifically with a lipid component in a crude ganglioside fraction isolated from human peripheral nerve, but did not react with myelin proteins including myelin-associated glycoprotein (MAG). The reactive glycolipid migrated between GM1 and GD1a gangliosides on TLC, and there was no distinct band corresponding to this glycolipid on examination with resorcinol reagent. Antibody to glycolipid in this case may play a role in the pathogenesis of neuropathy.

Ultrastructural visualization of carbohydrates in oxytalan fibers in monkey periodontal ligaments.

Fullmer's oxytalan fibers appear to be special connective tissue fibers belonging to elastic system fibers. We have ultrastructurally examined carbohydrates in oxytalan fibers in monkey periodontal ligaments after glutaraldehyde fixation and ethylenediaminetetraacetic acid (EDTA) decalcification using: Thiéry's periodic acid-thiocarbohydrazide-silver proteinate (PA-TCH-SP) method for thin-section staining of vicinal glycol-containing complex carbohydrates, and the concanavalin A-ferritin (Con A-ferritin) and Con A-horseradish peroxidase (Con-A-HRP) en bloc staining methods specific for alpha-D-mannosyl and alpha-D-glucosyl groups. PA-TCH-SP stained collagen fibrils weakly to moderately and stained oxytalan fibers moderately. Con A-ferritin and Con A-HRP stained collagen fibrils weakly or moderately and stained oxytalan fibers intensely within the superficial region of specimen blocks. The penetration of staining reagents was improved by prior saponin treatment and/or chondroitinase ABC digestion. Thus, these studies demonstrate that PA-TCH-SP and Con A staining of carbohydrates is very useful in identifying oxytalan fibers at the ultrastructural level and that more carbohydrate components are present in oxytalan fibers than in collagen fibrils.

The therapeutic effects of orally administered 5'-deoxy-5-fluorouridine, 1-(2-tetrahydrofuryl)-5-fluorouracil and 5-fluorouracil on experimental murine tumors.

The antitumor effects of three 5-fluorouracil-related compounds, 5'-deoxy-5-fluorouridine (5'-DFUR), 1-(2-tetrahydrofuryl)-5-fluorouracil (FT-207) and 5-fluorouracil (5-FU) itself, on several experimental murine tumors were compared after oral administration. 5'-DFUR showed strong antitumor activity against the solid type of four kinds of tumors tested with a wide range of effective doses, and also showed moderate antitumor activity against three kinds of solid tumors with a narrow range of effective doses. 5'-DFUR was effective against a few kinds of ascites tumors. In general, the antitumor activity of FT-207 was not very strong, with narrow ranges of effective doses under the present conditions. 5-FU showed strong toxicity but at lower doses its antitumor effectiveness was almost the same as that of FT-207. When the doses were divided into three and the divided dose was given orally three times a day for five consecutive days to mice bearing L1210 leukemia, this modality (with any of the three drugs) enhanced the ILS of the mice by two to three times in the case of the ascites type but not the solid type of L1210. The chemotherapeutic index in oral treatment of the solid type of tumors was higher for 5'-DFUR than for FT-207 or 5-FU. The minimum lethal doses in oral administration for five consecutive days were about 3, 1.5 and 0.5 mmol/kg/day for 5'-DFUR, FT-207 and 5-FU, respectively. In conclusion, 5'-DFUR appeared to have stronger antitumor activity and less toxicity than FT-207 and 5-FU, and it is therefore expected to be clinically useful.

Studies of human tear proteins: 4. Analysis by crossed immunoelectrophoresis of tears in various diseases.

Tears were collected from patients with familial amyloidotic polyneuropathy (FAP), orbital tumors, sarcoidosis, trigeminal and facial nerve palsy and corneal ulcer. These tears were analyzed by crossed immunoelectrophoresis. Two cases of FAP with deficiency of lacrimation and keratoconjunctivitis sicca showed normal tear protein patterns. In these cases, the deficiency of lacrimation may have been due chiefly to disturbance of the peripheral parasympathetic nerve innervating the lacrimal gland. Five other patients with FAP showed from slight to severe decrease in the tear-specific proteins. One of two cases with lacrimal gland tumor showed a decrease in the tear-specific proteins before removal of the tumor. After the surgery two patients showed significant decrease or disappearance of the tear-specific proteins. Three cases with other orbital tumors revealed no alterations in production of tear proteins. The case of sarcoidosis with keratoconjunctivitis sicca showed a decrease in the tear-specific proteins. Lacrimal gland involvement in sarcoidosis was confirmed by the tear protein analysis. The patient with facial and trigeminal nerve palsy showed a decrease in the tear-specific proteins. This was thought to be due to atrophy of the main lacrimal gland caused by functional disturbance of the lacrimal nerve. The analysis of tear proteins is useful in the diagnosis of lacrimal gland tumor disturbing the gland function and also of other diseases involving the lacrimal gland.

[An autopsy case of primary splenic hemangiosarcoma].

A 28-year-old woman was admitted six months before her death because of abdominal pain, anemia and hepatosplenomegaly. Clinical diagnosis by biopsy examination was primary hepatic angiosarcoma. The autopsy revealed about 700 ml of bloody ascites. The spleen weighed 510 g, and was completely replaced by dark grayish-red tumors. The enlarged liver was 6,560 g and revealed multiple tumor nodules. Microscopically, the tumor cells formed irregular vascular spaces. Factor VIII-related antigens were stained in the tumor cells. Zonula adherens was observed also by electron microscopic observation.

Detection of purine nucleoside phosphorylase in paraffin sections by the immunoperoxidase method.

A method is described for immunohistochemical demonstration of purine nucleoside phosphorylase (PNP: EC 2.4.2.1) in paraffin sections from routine surgical histology specimens. A peroxidase-antiperoxidase (PAP) method was employed, using specific rabbit antiserum against human PNP, which was purified from postmature human erythrocytes. In human lymph nodes, intensive staining for PNP was observed in the vast majority of small lymphocytes in paracortical areas, in many small lymphocytes in medullary cords, and in a few small-to medium-sized lymphocytes in germinal centers. Small lymphocytes in the primary follicles and those in the mantle zones of secondary follicles were negative for PNP staining. Tingible body macrophages, lymphatic sinus cells, and most of the large cells in germinal centers did not stain with anti-human PNP (hPNP) antibody. Endothelial cells of small vessels in the cortex and plasma cells did not show any constant pattern of PNP staining intensity. Histochemistry revealed that the distribution pattern of PNP activity was quite similar to that demonstrated on paraffin sections by the PAP method.

Familial amyloidotic polyneuropathy: ocular manifestations with clinicopathological observation.

Clinical findings of ocular involvement in two patients with familial amyloidotic polyneuropathy are described. Both cases revealed irregular pupillary margin, white membranous material on the pupillary border and on the lens surface. Open-angle glaucoma was found in one case, and ocular hypertension in the other. Histopathological examination of the tissues obtained during trabeculectomy in Case 1 revealed a large amount of amyloid substance around the vessels of the conjunctiva and the iris and in the endothelial meshwork of the chamber angle. Electron microscopic observation revealed that amyloid fibrils had formed in the basement membrane of the endothelial cells of the blood vessels in the conjunctiva and the iris. However, no amyloid fibrils were observed in the endothelial cells of the trabecular meshwork. Crossed immunoelectrophoresis of the aqueous humor of Case 1 showed a high-protein content, especially of alpha 1-lipoprotein and ceruloplasmin. The findings described above suggest that glaucoma or ocular hypertension in association with familial amyloidotic polyneuropathy is a result of the accumulation of amyloid substance in the endothelial meshwork.
