Clinical significance of ultrasonography, computed tomography, and biochemical tests in the rapid diagnosis of gallstone-related pancreatitis: a prospective study.

Real-time ultrasonography (US), computed tomography (CT), and biochemical tests were prospectively performed to detect gallstones in 88 consecutive patients immediately after the onset of an attack of acute pancreatitis. The sensitivity of biochemical tests was 84.6% when the patients had three or more positives of five parameters [including serum bilirubin, alkaline phosphatase (AP), gamma-glutamyl transpeptidase (GGT), alanine transaminase (ALT), and alanine transaminase-aspartate transaminase (ALT-AST) ratio]. The sensitivity, specificity, and accuracy were 71.8, 98.0, and 86.4% for US, and 52.9%, 100%, and 79.5% for CT. The sensitivity, specificity, and accuracy were improved to 82.1, 100, and 93.2% by the combination of US and CT, and 94.9, 100, and 97.7% by the combination of US and biochemical tests. Adding CT to the combination of US and biochemical tests resulted in only a slight improvement in sensitivity and accuracy. In conclusion, a combination of US and biochemical tests can provide the best noninvasive method in rapidly detecting gallstones as an etiological factor in acute pancreatitis. Computed tomography is not cost-effective. A positive result of biochemical tests despite a negative finding in US calls for an intensive search for gallstones by further investigation with endoscopic retrograde cholangiography or repeated US examinations.

High-resolution real-time ultrasound in Peyronie's disease.

Sonography is not considered to be a radiation hazard; it is noninvasive and can be widely used for examining external genitalia. We examined six cases of Peyronie's disease of the penis by high-resolution real-time ultrasound. The sonographic findings include a thick echogenic plaque with echogenicity similar to or higher than the tunica albuginea; a calcified plaque in thickened tunica albuginea; and are occasionally associated with calcification in the corpora cavernosa. Sonography may clearly localize the abnormal thickened area of collagen deposition and calcification. Repeated sonography after intracavernosal papaverine injection may further confirm the correlation of the plaque and the abnormally curved erectile penis. Doppler ultrasound may be used for further evaluation of the vascular condition if impotence coexists.

Congenital short bowel.

Congenital short bowel with malrotation or nonrotation but without atresia is a very rare anomaly. Two male siblings having the same condition are reported here and in addition the other four cases previously cited in the English literature are reviewed.

CT of gastritis cystica polyposa.

Gastritis cystica polyposa (GCP) is an uncommon lesion that usually occurs at the gastroenterostomy site, although occasionally it can be found in an unoperated stomach. We present a rare case of GCP which presented as a large polypoid mass in the gastric fundus and contained cystic components demonstrable on preoperative computed tomography.
