We studied left atrial thrombi (LAT) by both echocardiography and computed tomography (CT) and compared the features of the 2 methods. A total of 15 patients with mitral stenosis complicated by atrial fibrillation were selected as the subjects. LAT were noted on the M-mode echocardiograms in 2 patients including a questionably positive one, on the two-dimensional echocardiograms in 5, and on the CT scans in 6 of 15. The history of thromboembolism was rather frequent and was found in 7 of 15 patients. However, LAT was found in only 3 of these on the CT scans. A shaggy or fuzzy pattern on the M-mode echocardiogram cannot be regarded as representing thrombi, while a laminar pattern undoubtedly represented thrombi. Two-dimensional echocardiography has considerably contributed to the improved detection rate of LAT. For the characteristic properties of ultrasound beams, however, it was impossible to investigate the entire left atrium. The detection of the thrombi in the appendage was especially difficult. However, computed tomography, permitting transverse cross-sectional tomography, was capable of sectioning the heart even in the presence of air and bones. The measurement of CT values was suggestive of the properties of the substance or substances involved, and also allowed the presumption as to whether the thrombus has been fibrosed. Furthermore, it was possible to estimate more accurately as well as 3-dimensionally the location, shape and dimensions of the thrombi by the reconstruction of the heart according to the CT values. It was concluded that echocardiography and computed tomography are the mutual aid to further improvement in the detection rate of left atrial thrombi.

The histogenesis of pelvic endometriosis can not be sufficiently accounted for by a single theory. Thus it may be necessary to pressure plural theories for the explanation of their histogenesis. On collected 253 cases of endometriosis, some clinical and pathological studies were carried out and classified to adenomyosis (A) 37%, ovarian and pelvic endometriosis (O.P) 17%, and others (A+O.P) 45%. Age of the group O.P adenomyosis (A) 37%, ovarian and pelvic endometriosis (O.P) 17%, and others (A+O.P) 45%. Age of the group O.P tended to be younger, as compared with other groups. Pathological study revealed that in group (A+O.P), the lesion tended to be younger, as compared with other groups. Pathological study revealed that in group (A+O.P), the lesion in uterine wall was extensive and deep proliferation in many cases, and it was frequently more at least than group A. Especially, it is necessary to distinguish from each other two different cases which showed endometriosis in the ovary and pelvic peritoneum, that is, to distinguish them by the presence or absence of adenomyosis. In the case of group O.P complicated by A, it is thought highly probable that discrete dissemination may take place from the foci of adenomyosis. In the case of group O.P there may be sufficient room for the application of the Sampson's theory or metaplastic theories. In view of conspicuous trait in the age, however, the possibility of lymphogenous dissemination of endometriosis may be valuable.

A case of red cell adenosine deaminase (ADA) overproduction associated with hereditary hemolytic anemia is reported here. This appears to be the second report. Proband is a 38-year-old Japanese male who had hemoglobin, 15.8 g/100 ml; reticulocyte count, 4.5%; serum indirect bilirubin, 4.9 mg/100 ml; 51Cr-labeled red cell half-life, 12 days; red cells showed moderate stomatocytosis. His red cell ADA activity showed 40-fold increase while that of the mother showed 4-fold increase. The mother was hematologically normal. The father had a normal enzyme activity. The proband and the mother showed slightly high serum uric acid levels. The proband's red cell showed: ATP, 628 nmoles/ml (normal, 1,010--1,550); adenine nucleotide pool, 46% of the normal mean; 2,3-diphosphoglycerate content, 3,782 nmoles/ml (normal 4,170--5,300); increased oxygen affinity of hemoglobin, P50 of intact erythrocytes being 21.8 mmHg (normal, 24.1--26.1). Red cell glycolytic intermediates in the proband were low in general, and the rate of lactate production was low. Kinetic studies using crude hemolysate revealed a normal Km for adenosine, normal electrophoretic mobility but slightly abnormal pH curve and slightly low utilization of 2-deoxyadenosine. The ADA activity of lymphocytes was nearly normal.
Biochemical properties and antibiotic susceptibilities of 168 clinical isolates of Klebsiella pneumoniae were tested. On the basis of the indole reaction, 30 isolates (18%) were indole-positive and 138 isolates (82%) were indole-negative. A significant difference in antibiotic susceptibility was found in each of the two groups of isolates. Of the indole-negative isolates, 82.6% were susceptible to cefazolin at 6.25 microgram/ml or lower, while the indole-positive isolates varied in susceptibility to cefazolin. All of the indole-positive isolates were susceptible to chloramphenicol and tetracycline, but the indole-negative isolates varied in their susceptibility to these antibiotics. Whereas there was no relationship between indole production and susceptibility to cephalothin among the K. pneumoniae isolates studied, it appeared that there was a correlation between the indole reaction and susceptibility to cefazolin, chloramphenicol and tetracycline.
