[Breast carcinoma metastasis to perineal neurofibroma].

A 51-year-old woman as admitted to our department because of multiple palpable tumors that were noted on the left breast, the perineum, and the neck, shoulders, arms, and legs. The breast tumor showed ulceration on the surface. The perineal tumor was 30 x 15 cm in size and had group slowly for 17 years. A chest X-ray revealed a bilateral coin lesion. She was diagnosed as having a left breast cancer with a skin lung metastasis and a perineal benign tumor. A simple mastectomy and a resection of the perineal tumor were performed. A postoperative examination revealed an adenocarcinoma of the breast and a fibroneurinoma of the perineum with a metastasis from the breast cancer.

[Two cases of superior mesenteric vein thrombosis; the utility of CT scan].

Two cases of superior mesenteric vein thrombosis were examined with computed tomography. Prior to the operation the CT appearances of the disease were, as follows: focal thickening of the bowel wall, gas and fluid collection in the dilated bowel, mesenteric leaf, a little ascites and dilated superior mesenteric vein with central filling defect. We conclude that CT scan is very useful for an early diagnosis of the superior mesenteric vein thrombosis.

[Evaluation of reservoir inserted for the prevention of liver metastasis of colorectal cancer].

The subjects were 22 patients who had undergone intra-arterial chemotherapy using reservoirs for prevention of postoperative liver metastasis during the period from January 1986 to September 1989, 3 with non-resected liver metastasis, 1 with resected liver metastasis with intra-arterial chemotherapy, and patients with systemic chemotherapy. None of the 7 patients with macroscopic stage II disease, who had undergone intra-arterial therapy for the prevention of liver metastasis survived (maximum duration, 25 months), had liver metastasis. Two of 8 patients with stage III disease died, and liver metastasis was observed. Two of 7 patients with stage IV disease died, one of whom them had liver metastasis. Four of 12 patients with stage II disease treated by systemic chemotherapy died, and 2 of them had liver metastasis. Four of 18 patients with stage III disease treated by systemic chemotherapy died, and one of them had liver metastasis. One of 11 patients with stage IV disease systemic chemotherapy died, and liver metastasis was noted in another. None of the patients who had undergone intra-arterial chemotherapy for the prevention of liver metastasis had liver metastasis within a period of one year. At present there is no difference in survival period between the group given intra-arterial chemotherapy for the prevention of liver metastasis and the group given systemic chemotherapy.

[Application of a stent splint to protect intraoral organs from radiation injury in a 97-year-old patient with multiple oral cancers who survived to over 100 years old].

Radiation therapy had been used with increasing frequency in recent years in the management of oral cancers of advanced ages. In those cases we have to take good care to maintain the oral health of patients undergoing cancerocidal dose of radiation therapy. Using splints, as a tissue displacer, during radiation, we could treat a 99-year-old female patient without serious radiation sequelae, successfully she survived over 100 year-old. As she visited us at 97 year-old, the primary lesions located on the left upper lip, nose, upper and lower gums were diagnosed as multiple verrucous carcinoma histologically. Seventeen months after the first radiotherapy to the lip, nose and upper jaw, we planned again radiotherapy to the recurrent tumor of the lower gum. In order to eliminate and minimize side effects of the second irradiation for the contiguous intraoral organs, we devised a splint to exclude the tongue and upper gum apart from a radiation field. The splint, as tissue displacer, was made of heat-cured acrylic resin and divided into two pieces which were formed like full denture without artificial teeth. They were applied to the upper and lower jaws. The lower one had a large wing to exclude the tongue from irradiation field. After setting of the splint, she had been clenched slightly with an aid of chin cap. Then we could finish successfully the radiotherapy with 10 MV X-ray 40 Gy as scheduled without serious troubles.

Improved separation of alpha chains of collagen type I, type III, and type V by noninterrupted electrophoresis using thioglycolic acid as a negatively charged reducer.

Improved separation of alpha chains of collagen type I (alpha 1 [I]2 alpha 2[I]), type III(alpha 1[III]3), and type V (alpha 1[V]alpha 3[V])was achieved by noninterrupted sodium dodecyl sulfate-polyacrylamide gel electrophoresis with a negatively charged reducer, thioglycolic acid. The thioglycolic acid, added to the running buffer of the cathodic reservoir, in the middle of electrophoresis quickly migrated in the gel anode, reducing interchain disulfide linkages in collagen type III and dissociating it into its alpha chain monomer, alpha 1[III], without an interruption of electrophoresis. The alpha chain, alpha 1[III], migrated more slowly than the alpha 1 [I] and alpha 2[I] chains of collagen type I, resulting in an excellent separation of alpha 1[III] from alpha 1[I]. The mobility of alpha 1[III] could be controlled by varying the time of thioglycolic acid addition to the running buffer. This enabled us not only to separate alpha 1[III] from alpha 1[I] and alpha 1[V], but also to precisely quantitate these alpha chains, even at low protein loading of mixed samples.

Effects of pre-motion electromyographic silent period on dynamic force exertion during a rapid ballistic movement in man.

The effects of pre-motion silent period (PSP) on dynamic force exertion were studied in ten healthy subjects performing ballistic elbow extensions. The experiments were designed to evaluate the significance of mean differences between the averaged dynamic force curves of two groups: PSP-presence groups and PSP-absence groups. The presence of PSP was judged quantitatively and automatically by means of a newly developed method using statistical analysis. The results indicated that there were two effects of PSP on dynamic force exertion: one was a reducing effect, observed prior to the movement; the other was a reinforcing effect, observed in the first part of the ballistic movement. The duration of the reinforcement was significantly correlated with the duration of the reducing effect of PSP. The findings suggested that the reinforcement of dynamic force may be produced by the pre-stretch of agonistic muscles caused by prior force reduction due to PSP occurrence. The fact that PSP plays an important role in dynamic force exertion suggests that PSP may be incorporated in the central motor control system designed to interrupt the background activity, to stretch the agonist and to reinforce the dynamic force.

Cerebral endothelial regeneration following experimental brain injury. Variation in the regeneration process according to the severity of injury.

It is still unknown when and in which area endothelial regeneration occurs after brain injury, and to what extent such changes depend on the severity of the injury. We have, therefore, studied bromodeoxyuridine (BrdU) uptake by regenerating endothelial cells in two different groups of rats given cold lesions using immunohistochemistry employing anti-BrdU monoclonal antibody, anti-factor VIII-related antigen antibody and anti-glial fibrillary acidic protein antibody. The earliest evidence for the presence of BrdU-positive endothelial cells (BrdU + end) was observed at 2 days after injury, the injured endothelial cells regenerating from the edge toward the center of the lesion in both groups. We considered that edema fluid could act as an important factor, since at 2 days post-injury BrdU + end were not in contact with macrophages and were always found in Evans blue-stained areas. Study of endothelial cell kinetics also confirmed that the repair of injured endothelial cells was intimately involved in the reconstruction of the blood-brain barrier, since the time of disappearance of BrdU + end coincided with the disappearance of Evans blue-stained areas. The difference in the process of endothelial regeneration was first apparent on the 3rd day, there being no difference at 2 days.

Infrasellar craniopharyngioma.

A rare case of infrasellar craniopharyngioma mainly occupying the paranasal sinuses, the infratemporal fossa and the skull base is presented. The patient had been treated by an otolaryngologist as a case of mucocele of the sphenoid and ethmoid sinuses because the biopsy specimens obtained at endonasal surgery failed to confirm the true diagnosis. Computed tomography (CT) showed multi-lobulated low density cysts in the paranasal sinuses, infratemporal fossa and skull base, and the cyst wall was enhanced. Magnetic resonance Imaging (MRI) revealed the cysts to have high intensity in both T1- and T2-weighted images, and excellent three-dimensional visualization was obtained. Characteristic motor-oil-like fluid was aspirated during transnasal surgery and the diagnosis of craniopharyngioma was confirmed by histological examination.

Cloning of the gene for cell-surface protein antigen A from Streptococcus sobrinus (serotype d).

A gene library of Strep. sobrinus B13N (serotype d) chromosomal DNA was constructed in Escherichia coli, with the bacteriophage vector lambda L47.1. A recombinant phage, lambda MDSM49, containing a 15.5 kb DNA insert, directed the expression of a 210 kDa antigenic protein. The recombinant 210 kDa protein was shown by Western blot analysis to be identical with cell-surface protein antigen A (spaA) from a serotype g strain. However, the restriction patterns of a subclone plasmid, pMD51, from lambda MDSM49 differed from those of serotype g strain. The cell-surface protein antigen I/II from serotype c Streptococcus mutans is a potential immunogen for vaccination against dental caries and corresponds to the spaA from serotype d and g strains. A recombinant clone, pDM51, will be a useful tool for serological and molecular biological studies. The recombinant spaA provides useful material for assessment of its diagnostic and immunogenic potential.

DNA homology of surface protein antigen A gene in mutans streptococci.

1. A recombinant plasmid, pYA724, containing an 8.45 kb DNA fragment encoding surface protein antigen A (spaA) from Streptococcus sobrinus 6715 was used to examine the DNA homology of the spaA gene with chromosomal DNA of various mutans streptococci strains. 2. Restriction endonuclease BamHI-digested pYA724 DNA was radio-labeled by nick-translation, and a DNA-DNA hybridization experiment was carried out. pYA724 DNA hybridized with chromosomal DNA of serotypes a, c, d, e, f and g strains, but not with b by dot DNA-hybridization and Southern blot DNA hybridization. 3. Chromosomal DNAs were isolated from several serotype c Streptococcus mutans strains, digested with BamHI, and analyzed by Southern blot DNA hybridization. pYA724 DNA hybridized with different sizes and numbers of BamHI-digested DNA fragments of the chromosomal DNAs. 4. These data indicated that all mutans streptococci strains except serotype b have DNA homologous with the spaA gene, although within the same serotype strain the spaA gene has a diversity of arrangement within the chromosome.

Extradural nasal and orbital extension of malignant glioma. Case report.

A rare case of extradural nasal and orbital extension of malignant glioma is presented. The development of malignant changes was observed during 10 years. The mode of transdural extension was via the olfactory nerve, where it was directly destructive in all four cases that have been reported to date. The biological malignancy of transdurally extending glioma is also described.

Two circulatory routes within the human corpus cavernosum penis: a scanning electron microscopic study of corrosion casts.

The microvascular architecture of the human corpus cavernosum penis was studied by scanning electron microscopy of vascular corrosion casts. The corpus cavernosum was supplied by the penile deep artery. It gave off branches to become either arteries distributed within the corpus cavernosum or those directly supplying the corpus spongiosum urethrae. The former arteries further divided into small arteries which fell into two categories: 1) arteries breaking up into capillaries, and 2) arteries draining directly into the cavernous sinuses. The capillaries were collected into venular networks just beneath the tunica albuginea (the subalbugineal venular plexus), while the cavernous sinuses were collected into venules at the periphery of the corpus cavernosum. These postcavernous venules also received venules from the subalbugineal venular plexus, and left the corpus cavernosum. Thus, two circulatory routes are evident within the corpus cavernosum. These findings suggested that the penile erectile cycle is controlled by hemodynamic changes between these two routes within the corpus cavernosum.

Quantitative analysis of outflow pathway of corpora cavernosa by pressure flow technique.

Using a pressure flow technique, quantitative analysis of the physiological characteristics of the outflow pathway of the corpus cavernosum was carried out in 19 male dogs weighing 7.5 to 23.0 kg. Pressure flow curves were made on dogs whose pelvic nerve was stimulated electrically and on dogs left unstimulated. When a cyclical change in saline perfusion rate was applied without nerve stimulation, the variable of the intracorporeal pressure showed a large hysteretic loop, indicating that the resistance of the outflow canals to flow was altered by the distension of the sinusoidal space. In dogs whose pelvic nerve was stimulated, the pressure flow curves shifted to the left side in comparison with the outward phase of the pressure flow curve of animals without pelvic nerve stimulation, and this curve piled on the returning phase. No hysteretic relation was observed between the outward and returning phase of the pressure flow curve with pelvic nerve stimulation, but in the detailed analysis, in which the % flow rate was used instead of actual flow rate of saline perfusion, a small hysteretic loop based on the difference of the elasticity of the outlet canals was found. The distension of the corpora cavernosa and the pelvic nerve electrostimulation probably act as the triggers of the same occlusive mechanism in the outflow pathway. The percentage decrease in the blood flow in the outflow canal of the corpus cavernosum induced by the distension of the sinusoidal space or by the pelvic nerve electrostimulation was 69.6 +/- 14.4% (mean +/- SD).

Preparation and characterization of antibodies against 3,2'-dimethyl-4-aminobiphenyl-modified DNA.

Polyclonal antibodies for 3,2'-dimethyl-4-aminobiphenyl (DMAB)-DNA adducts were obtained from the sera of rabbits immunized with N-OH-DMAB-modified DNA. Using the enzyme linked immunosorbent assay (ELISA), these antibodies were shown to recognize DNA modified by N-OH-DMAB and N-hydroxy-4-aminobiphenyl, but not unmodified DNA, 2-acetylaminofluorene- or 4-nitroquinoline-1-oxide-modified DNA, free aminobiphenyl or DMAB derivatives. Using competitive ELISA with DMAB-modified denatured DNA, a 50% inhibition of antibody antigen binding was caused by 7 fmol of DMAB adducts applied to each assay well as an inhibitor. Native DNA bearing adducts was associated with 50% inhibition in the range of 22-90 fmol/asay well, depending on the levels of modification. The adducts recognized by the antibody were shown to be stable against treatment of heat or alkali denaturation. Optimal conditions for the detection of adducts in DNA from the rat organs exposed to DMAB were established by means of competitive ELISA, using alkaline-denatured DNA as an inhibitor. Thus, as little as 5 fmol adducts in 1 microgram of DNA could be detected. Indirect immunofluorescence staining indicated that anti-DMAB-DNA antibodies bound specifically to nuclear components of rat fibroblast 3Y1 cells treated with N-OH-DMAB. The intensity of the fluorescence was proportional to the dose of carcinogen administered. The antibodies should be helpful for use in studies on the formation of adducts and their removal in cells and tissues after DMAB administration.

Functional neuromuscular stimulation system using an implantable hydroxyapatite connector and a microprocessor-based portable stimulator.

For developing functional neuromuscular stimulation (FNS) orthoses, percutaneous implants made of sintered hydroxyapatite (HAp) and a microprocessor-based portable stimulator were devised, and related basic studies were carried out. First, a button-shaped HAp was percutaneously implanted up to 23 months in the skin of dogs, and 30 months in human subjects. The HAp was closely contacted with the skin tissue. Then, button-shaped HAp electrical connectors were percutaneously implanted in the skin of rabbits. Sufficient contractions of the calf muscle were elicited by electrical stimuli delivered to intramuscular electrodes through the connector. These results indicated that the HAp electrical connector could be clinically utilized as an implantable skin interface for FNS. The microprocessor-based stimulator capable of processing control command signals and delivering 16-channels of electrical stimuli was developed. Usefulness of the stimulator was demonstrated by applying it to flexor and extensor muscles of the thumb in a C5 quadriplegic patient.

Experimental study on the adsorption of excess heparin with anion exchange resin fiber.

Anion exchange resin fiber (Ionex) was used as a heparin adsorbent. Ionex has the adsorption capacity of 70 mg/g (weight by desiccation) for heparin, and was used in an attempt to remove the heparin from blood-perfused artificial organs, before the blood was transfused back into the patients. In the ex vivo study, the 5 systemically heparinized dogs (500 U/kg) were treated with a 35-40 g column of Ionex, by direct hemoperfusion (DHP). The concentration of heparin was significantly reduced, within 15 to 60 minutes, using the Ionex. This suggested the possibility of removing excess heparin from the living body. In in vitro, the relationship between the amount of heparin by Ionex and the blood-flow volume were evaluated. Whole blood taken from dogs was added to 5 U/ml of heparin. This was then introduced into a small column containing 0.9 g Ionex, at blood-flow rates of 0.8, 1.6, and 3.2 ml/min. A good adsorption capacity was shown at the blood-flow rate of 0.8 ml/min. One possible explanation for this is that the heparin dispersed in the blood cells gradually was passed on into the plasma and, so, did not have time to be adsorbed at the faster flow rates.

Antigenic relationship between streptococcal preparation OK-432 and tumors and its effect on immunotherapy with OK-432 in patients with superficial bladder tumor.

The cross-antigenicity between streptococcus, Su strain (OK-432), and rat bladder cancer BC-47 was studied by the immunocytochemistry. Both anti-Su (OK-432) and anti-BC-47 reacted with both homo- and heterogenous antigens. The specificity of the PAP reactions using these sera was confirmed by absorption study of the first antibodies. But, OK-432 did not share with normal rat tissues. Furthermore, the presence of common antigens between OK-432 and animal tumors where OK-432 was effective was indicated by the PAP studies, but OK-432 did not share with tumors where OK-432 was ineffective. By the PAP studies using antigroup A streptococcus-specific C-polysaccharide and antigroup-specific sera, it was suggested that the determinant of the cross-antigenicity was group A-specific C-polysaccharide. The common antigens between OK-432 and human urogenital cancers were also identified by the PAP study. Then, local immunotherapy with OK-432 was carried out in 38 patients with superficial bladder tumor and the tumors were eliminated in 23.7%. In 77.8% of the complete response and in 10.3% of the no change, the tumor tissues demonstrated antigens common between OK-432 and tissues. From these results, it was concluded that common antigens which OK-432 shared with tumors could favorably augment host defense against human bladder tumors, the same as animal tumors.
