This study investigated the effect of age on adaptive behavior and academic skill in autistic and mentally retarded children. Subjects were 47 autistic and 128 mentally retarded children from a special school. Cross-sectional comparisons were made between junior and senior groups using ratings obtained from teachers on adaptive behaviors and academic skills. We found that the levels of toilet training, eating skills, participation in group activities, and self-control in the autistic children improved significantly with age. The skills of number concepts in the autistic children also improved with age. However, these adaptive and academic levels were in general significantly lower than those of the mentally retarded children. The levels of initiative did not improve significantly in either the autistic or the mentally retarded children, and they were significantly lower in the autistic children. The implications of these findings in the context of our previous study on the changes of communication and maladaptive behaviors with age in the autistic children are discussed.
A very unusual case of normotensive primary aldosteronism is described. A 25 year old woman first noticed numbness of both hands followed by a typical tetany attack. The blood pressure was within normal ranges. The results of other physical examinations were negative except for the findings of Trousseau's sign. Hypokalemi, increased potassium clearance, disturbed urine concentration and increased circulating plasma volume were noticeable. Diagnosis was established by (1) increased levels of plasma aldosterone, (2) low plasma renin activity, (3) normal adrenocortical function and (4) typical aldosterone-producing adenoma. After removal of the adenoma, the abnormalities subsided. The lack of hypertension in primary aldosteronism is a rare condition. Several possible causes of normotension must be considered, such as the early phase of primary aldosteronism, essential hypotension associated with primary aldosteronism and lack of concomitant secretion of other mineralocorticoids from the adenoma, but these were all negligible. As blood pressure response to the administration of angiotensin II was lower than is typical for primary aldosteronism, and the patient transiently suffered from frequent attacks of blackouts soon after the operation, the cause of normotension was thought to be due to the existence of a hypotensive mechanism which counteracts the increase in blood pressure in primary aldosteronism.

A statistical study on the comparison of the comprehension skill levels between the group with and the group without each maladaptive behavior of nine items was done for autistic and mentally retarded children. Results indicated that the group of the autistic children with hyperactivity or withdrawal had slightly, not significantly, lower comprehension skill levels than the group of those without each of these maladaptive behaviors. On the other hand, the correlation between the prevalence of hyperactivity or withdrawal and lower comprehension skill levels was seen more clearly, with statistically significant differences, rather among the mentally retarded children.
Forty-seven autistic and 128 mentally retarded children, ages 6 to 14, from a special school were assessed in terms of nine maladaptive behaviors and speech skill levels. The results indicated that the group of the mentally retarded children with withdrawal had significantly lower speech skill levels than the group of those without withdrawal, and the group of the autistic children with self-injury had significantly lower speech skill levels than the group of those without self-injury.

The cross-sectional method was used to compare junior and senior groups, and ratings on communication skills and maladaptive behaviors were obtained from teachers. We found that the skills of comprehension and conversation in autistic children improved significantly with age, and speech improved somewhat. In spite of this improvement in communication skills, maladaptive behaviors in the autistic children other than hyperactivity did not change significantly with age. Withdrawal improved significantly with age in the mentally retarded children but not in the autistic children.
An operative technique consisting of a diagonal epigastric incision extending to the left chest wall and severing the inner side of diaphragm adjacent to the pericardium to the hiatus is introduced. This technique was successfully used in 16 cases and the present surgical approach facilitates frontal visualization of the lower esophagus without disruption of the circulatory system. Furthermore, a sufficiently large operative field is obtained and radical dissection of cardia and upper gastric region or combined resection of infiltrated adjacent organs is facilitated. Postoperative recovery of pulmonary function in terms of PaO2 and PaCO2 was similar to that of patients who had received an upper median incision. We found no postoperative complications peculiar to the operative technique introduced here in any of the 16 patients.
M-mode echocardiography on a fifty-eight year old female with Uhl's anomaly showed several characteristic findings, which were considered to be useful in differentiating Uhl's anomaly from Ebstein's. Those findings were normal tricuspid diastolic closing velocity, the relatively early tricuspid valve opening, easy visualization of the tricuspid valve in usual position and the unusual mid-diastolic pulmonic valve full opening. The premature opening of the pulmonic valve correlated with the right atrial and ventricular mid-diastolic pressure exceeding that of the pulmonary artery.

One hundred and twenty-eight mentally retarded children, ages 6 to 14, from a special school were assessed by teachers in terms of nine maladaptive behaviors. The results indicated that the prevalence of some maladaptive behaviors changed as a function of IQ and age, and that there were splits of such changes as IQ-dependent and as age-dependent by maladaptive behavior.

The reproductive activity of Japanese mothers of mongoloid children decreases significantly after the birth of such a child, in comparison with mothers of normal children matched on the basis of maternal age. This decline of reproduction is observed more clearly among younger mothers whose fertility is thought still high.
