Streptococcus mutans in saliva: intraindividual variations and relation to the number of colonized sites.

Analysis of the salivary levels of Streptococcus mutans may contribute to the evaluation of the caries risk. It is therefore important to identify factors that might influence the outcome of such analyses. This investigation consisted of three parts. The first aim was to evaluate the short-term variation of S. mutans concentration in saliva. Furthermore, we estimated the effect of discontinued oral hygiene on salivary S. mutans. We also analyzed the relation between salivary levels of S. mutans and the number of colonized approximal and occlusal sites. Tongue samples were also included in the comparison. Systematic short-term variation could not be demonstrated. A 95% confidence interval for an observed S. mutans value was obtained by multiplying and dividing the observed value by a factor of 5 (for S. mutans samples less than 10(6) or 2.3 (S. mutans greater than 10(6). One week of discontinued oral hygiene did not significantly change the level of S. mutans in saliva. Saliva samples correlated significantly with tongue samples and with the number of colonized approximal sites. The results of these studies confirm the stability of the S. mutans colonization level.

Sleep of shiftworkers within the arctic circle.

The sleep of shiftworkers in the Arctic has been investigated using electroencephalography. The subjects were studied four times a year over a 2-year period. There was a trend toward less restful sleep during the autumn and winter months, but otherwise sleep at various times of the day was similar to that of individuals elsewhere. This would suggest that sleep disturbance in polar shiftworkers can be managed in a similar way to that of shiftworkers in temperate regions.

Serum beta 2-microglobulin in patients with non-Hodgkin's lymphoma.

Eighty-one patients with non-Hodgkin's lymphoma had serum beta 2-microglobulin (beta 2M) estimated at presentation. A significant association was found between beta 2M levels and stage of disease (P less than 0.001), presence of hepatomegaly (P less than 0.001) and bone marrow involvement (P less than 0.05). No association was found between the level of beta 2M and histological group, presence of splenomegaly, lymph-node masses greater than 5 cm in diameter, lymphocyte count or the presence of systemic B symptoms. Pretreatment levels of beta 2M did not help predict response to treatment and achievement of a complete remission, the length of remission obtained or survival.

VAC (vincristine, adriamycin, cyclophosphamide) chemotherapy for metastatic carcinoma from an unknown primary site.

Twenty patients presenting with metastatic carcinoma from an unknown primary site were studied. All patients were treated with a triple chemotherapy regimen of vincristine, adriamycin and cyclophosphamide repeated at three-week intervals. The response rate was 50%, and the four patients achieving complete response are alive and disease-free at 13, 16, 36 and 39 months. Toxicity was minimal and the majority of patients' performance status improved with the chemotherapy. VAC chemotherapy is indicated for patients with metastases, particularly of soft tissues originating from a carcinoma from an unknown primary site.

Prognostic significance of pretreatment serum beta 2-microglobulin levels in multiple myeloma.

Pretreatment serum beta 2m levels were estimated in 65 patients with multiple myeloma. After correction for changes in renal function, the level of beta 2m attributable to multiple myeloma (beta 2m-m) no longer correlated with the serum creatinine, but showed a strong correlation with the total body myeloma cell mass (P = 0.002). There was an inverse correlation between beta 2m-m and the Karnofsky performance status of patients at presentation (P = 0.001). Patients with a pretreatment beta 2m-m level of less than the median value of 2.94 mg/l survived significantly longer than those with a raised level (P = 0.0008).

Intermediate dose single agent cyclophosphamide chemotherapy of advanced breast cancer.

Twenty-one evaluable patients with metastatic breast cancer received three pulses of intravenous cyclophosphamide, escalating from 1.5 g/m2 to 2.5 g/m2 in the second and third courses. There were eight partial remissions (34.7%), but most were of short duration. All responding patients had soft tissue or nodal disease, but additional sites of response were bone (three cases), liver and lung (one case each). Marked leucopaenia (median WBC 0.7 X 10(9)/L) occurred 10-12 days after the higher doses (2.5 g/m2) and was associated with nine episodes of severe infection in five patients. Marked vomiting and anorexia led to significant weight loss (mean 4.5 kg) in half the patients, and alopecia was universal. In metastatic breast cancer cyclophosphamide seems to have a shallow dose response curve and high intermittent dosage seems to have no advantage over chronic daily administration.

Asbestos disease in maintenance workers of the chemical industry.

In several large groups of workers employed in chemical plants, chest x-ray abnormalities (small irregular opacities and/or pleural changes) of the type known to be induced by asbestos were found in a proportion of those examined. A cross-sectional study of maintenance workers in a large chemical plant was undertaken to evaluate the prevalence of asbestosis; 185 workers were examined. Radiologic evidence of parenchymal interstitial fibrosis was found in 24% of those examined; in 10% of workers, parenchymal fibrosis was the only abnormality. Pleural fibrosis and/or calcification was found in the absence of parenchymal fibrosis in 14% of cases; in another 14% of workers, both parenchymal and pleural abnormalities were detected. The prevalence was significantly higher in those employed 20 or more years. Pleural abnormalities were more prevalent than were parenchymal changes. The increased risk of lung cancer and mesothelioma remains to be studied.
