[Japanese medicines studied by Hepburn, an American missionary, in 1860s].

In the last days of the Tokugawa shogunate when Japan was opened its door to trade, J.C. Hepburn came to Japan, and he contributed to the modernization of Japan as a missionary, a doctor and an instructor. His great academic achievement was publishing a Japanese-English and English-Japanese dictionary. The dictionary contained words which were generally used in those days. It is said that it was way of living and culture in Japan as seen by an American. Words about medicine show the conditions of the business of medicine in those days. Until 1889 a pharmacist was not professionalized yet, nor was the separation of dispensary from medical practice effective. As most of doctors were Chinese herb doctors, there was not the word "MD." As Chinese doctors were the leading ones, lots of words about herb were mentioned. We can find only a few words about chemicals. The common people used patent medicine such as Daranisuke, Mankintan. Less than 100 years, sorts of medicine in Japan have changed to those of medicine in Western countries.

[The essays of Keizou Tanba and Ougai Mori about separation of dispensary from medical practice].

Keizou Tanba wrote an article on separation of dispensary in Chugai-iji-shinpo in June, the year 23 of Meji Era. It was said that separation of dispensary had to be carried out and it brought many advantages. First, a pharmacist can examine the prescription and correct it if there are any mistakes. He will be able to give an advice to a doctor that the side effects will perhaps occur due to preparation of medicine. Secondly, a pharmacist can superintend whether unnecessary medicine is prescribed or not. He will check surplus prescription in such a case that a patients will be cured by observing dietary or exercise. According to Tanba's opinion, if we will get people to know what separation of dispensary is, it is easy to be carried out. Ougai Mori wrote an essay separation of dispensary in the same period. His opinion was opposite, basically. It caused to impose a burden on a patient. As the number of pharmacist were not enough, it was impossible to carry it out. A conflict of opinion also remains among people related to medicine at present. The circumstances on separation of dispensary have not been changing during one hundred years.

[A case of sarcoidosis with middle lobe atelectasis].

We report a case of sarcoidosis complicated by middle lobe atelectasis. A chest radiograph and a chest CT film on admission revealed middle lobe atelectasis and mediastinal lymphadenopathy. Bronchofiberscopic findings showed slit-like stenosis and reddish, edematous mucosal change of the middle lobe orifice. On admission, the serum angiotensin-converting enzyme (ACE) level was within the normal range. However, the ACE level had increased beyond the normal range by 2 months later, histological examination of a specimen from a transbronchial lung biopsy (TBLB) of the middle lobe revealed noncaseating epithelioid cell granulomas. We diagnosed sarcoidosis because of the histological findings from the TBLB and serum ACE elevation. Treatment with predonisolone resulted in remarkable alleviation of middle lobe atelectasis and mediastinal lymphadenopathy. Cases of sarcoidosis with middle lobe atelectasis are rare in Japan.

[Analysis of DNA contents in hematological malignant cells using whole blood lysis method].

The experiments have been undertaken whether DNA contents could be measured using whole blood lysis method by FACScan. Cell population in the phases of G1, S and G2 + M were well analyzed, when we used 3 x 10(6) cells lysed with 0.1% Triton X-100 in 1 ml of phosphate buffered saline, staining with 30 micrograms/ml of propidium iodide (PI) within 30 min after staining with PI. We have further developed cell cycle analysis for cells bearing lineage specific antigens recognized with FITC-conjugated monoclonal antibodies using two color analysis. When we fixed cells with 50% ice-cold ethanol after staining cells with FITC-conjugated antibodies, positive population ratio in these cells have been unchanged before and after fixing for CD3, CD4, CD5, CD8. CD10, CD19, CD14, CD33, and HLA-DR, but CD7 positive cells were markedly decreased after fixing. Using this method, CD41 positive leukemia cells have 3.4% in S phase and 6.8% in G2 + M phase, while CD41 negative cells have 1.8% in S phase and 2.0% in G2 + M phase in a patient with AML: M7, resulting leukemia cells were rich in S phase and G2 + M phase. The similar results were obtained in patients with AML:M2 using CD33 antibodies. During the clinical course, the changes of the blast numbers were well-correlated with changes of S-phase proportion in the patient with AML:M2. Among 47 patients with hematological malignancies in our hospital tested here, only 2 cases with 4.3% of total patients showed to have aneuploidy in malignant cells. One is a patient with non-Hodgkin lymphoma, the other is myelodysplastic syndrome.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of pleural tumor with hypoglycemia].

A 72-year-old woman was admitted to the hospital because of hypoglycemia. Her glucose level on admission was 40 mg/dl. A giant intrathoracic mass was demonstrated on X-ray, Computerized tomographic scan of the cerebrum showed no abnormalities. Serum insulin and C-peptide levels were within normal limits. The mass (weight 2.3 kg) and the right lower lobe were excised by thoracotomy and the hypoglycemia ceased. Pathological examination revealed the mass to be a localized fibrous tumor of the pleura.

Teratogenic evaluation of di-n-butyl phthalate in rats.

Pregnant rats were given di-n-butyl phthalate (DBP) by gastric intubation at a dose of 0, 0.5, 0.63, 0.75 or 1.0 g/kg on days 7-15 of pregnancy. A significant decrease in the maternal body weight gain after treatment with DBP was found at a dose of 0.63 g/kg and above. Maternal death and complete resorption of implanted embryos in all the surviving dams were observed in the 1.0 g/kg group. Significantly increased incidence of postimplantation loss and decreased fetal weight were detected at doses of 0.63 and 0.75 g/kg. The incidence of fetuses with malformations was higher in the 0.63 and 0.75 g/kg groups than in the control group, and the difference was significant in the 0.75 g/kg group. Cleft palate were predominantly observed.

[A case report of miliary tuberculosis with tubercular meningitis diagnosed and followed by polymerase chain reaction method].

We report here a case of miliary tuberculosis with tubercular meningitis in which the polymerase chain reaction (PCR) amplification method was useful for its rapid diagnosis and follow up. A 70 year old male was hospitalized for further examination and treatment of diffuse small nodular shadows on the chest X-ray. After receiving antimicrobial therapy shadows still remained and he gradually lost visual acuity. He had no meningeal signs, and no remarkable finding on cranial CT. Cerebrospinal fluid examination showed increased cell number with predominantly lymphocytes. Cranial MRI (Gd DTPA) showed lateral ventricular ependymitis. Pulmonary tuberculosis and secondary tubercular meningitis were suspected, but we failed to detect microorganisms from the cerebrospinal fluid, gastric juice, sputum, and urine by the conventional method. However, by the polymerase chain reaction amplification method specific DNA fragments of Mycobacterium tuberculosis complex was detected from the cerebrospinal fluid, gastric juice, bronchoalveolar lavage fluid and serum. The final diagnosis of miliary tuberculosis with tubercular meningitis was established. We administered antitubercular drugs and observed the clinical course. He recovered and the polymerase chain reaction showed negative consequences in all samples. The judgement of PCR and the clinical course were compatible and parallel with the clinical course.

Axial length of eyes with rhegmatogenous retinal detachment.

We conducted a prospective study of 226 eyes with nontraumatic rhegmatogenous retinal detachment; we examined patient characteristics as well as changes in axial length and corneal curvature induced by scleral buckling surgery. Eyes with a round hole in lattice degeneration were characterized by axial length that was longer than that of eyes with retinal tears with and without lattice degeneration. Sex and age also correlated significantly with axial length. Surgically induced changes in axial length depended upon the procedures performed, but the retinal detachment surgery primarily caused a change in the refractive status.

Acute lymphoblastic leukemia associated with a t(1;19)(q23;p13) in an adult.

A cytogenetic study of a 38-year-old patient with acute lymphoblastic leukemia (ALL) revealed a t(1;19)(q23;p13), which is a characteristic translocation of childhood ALL. The leukemic cells were positive for the CD10, CD19, HLA-DR, TdT and cytoplasmic mu-chain. Both of the immunoglobulin heavy chain gene loci were rearranged and the RNA-based polymerase chain reaction demonstrated the E2A/PBX1 fusion transcript which is the result of the t(1;19). This finding suggests that the t(1;19) is implicated not only in childhood ALL but also in adult ALL patients.

Thermal influence on intracellular calcium concentration in vestibular hair cells isolated from the guinea pig. A preliminary report.

Caloric responses to intracellular calcium homeostasis of vestibular hair cells isolated from the guinea pig were examined. An increase in intracellular free calcium ion concentration ([Ca2+]i) in the isolated cells was observed by direct heat stimulation. The cells, dissociated enzymatically or mechanically, were stimulated by perfusion of solution, in the temperature range of 2.2-5.2 degrees C. The [Ca2+]i was analysed using a digital imaging microscope and the Ca2+ sensitive dye, Fura-2. There was an increase in [Ca2+]i in both Types I and II vestibular hair cells with this heat stimulation. The direct thermal influence on vestibular sensory cells is discussed in relation to mechanisms involved in caloric nystagmus.

[The essay on separation of dispensary from medical practice written by Yukichi Fukuzawa].

In November 1891, the union of pharmacists submitted the petitions letter on revision of the Law No. 10 issued on March 15, 1889, because by this law physicians acquired the right to dispense medicine rather than to give prescriptions to their patients. Yukichi Fukuzawa criticized the above mentioned problems and he contributed essays to the famous newspaper, Jiji-shin-pou in december 1891. Principles of his essays were as follows: 1) separation of dispensary from medical practice widely done in western countries were important from scientific viewpoint; 2) however, both physicians and patients had no concept on total medical fee which was consistent with consultation fee and charge for medicines. Thus, patients in Japan have been receiving medicines from physicians customarily over several hundred years; 3)introduction of new system on separation of dispensary from medical practice would cause troubles, because additional payment at the pharmacy should be required other than payment for physicians; 4)he was aware that opportunities to commit sick persons to doctor's care would be decreased especially in case of poor persons. Compared the public opinion to the separation of dispensary from medical practice of present day, there was found no difference on the attitude of Japanese people in these hundred years.

Protective effect of a selective endothelin receptor antagonist, BQ-123, in ischemic acute renal failure in rats.

To elucidate the pathophysiological role of endogenous endothelin (ET), we examined the effects of the newly synthesized ETA receptor-selective antagonist, BQ-123, on ischemic acute renal failure induced by bilateral clamping of renal artery and vein followed by reperfusion in rats. BQ-123, when given by i.v. infusion of 0.5 mg/kg per min for 2.5 h during the pre- and post-ischemic period, was found to prevent the decrease in creatinine clearance and increases in blood urea nitrogen, plasma creatinine and the fractional excretion of sodium. Morphological observation also showed an effect of BQ-123, i.e. prevention of proximal tubular (S3 segment) necrosis. At 2 h after the start of reperfusion, the ET-1 content in the kidney increased to its maximal level. At this time, the Ca2+ content in the mitochondrial fraction of the renal cortex increased, with a concomitant increase in blood urea nitrogen. However, these increases were limited by treatment with BQ-123. Thus, BQ-123 was effective to both prevent mitochondrial Ca2+ accumulation in the early phase of ischemic acute renal failure and protect proximal tubular cells from post-ischemic degeneration. We conclude that ET may be at least partially involved in the pathogenesis of tubular cell injury in this acute renal failure model.

Some considerations on the biology of pancreatic serous cystadenoma.

Five cases of pancreatic serous cystadenoma were examined pathologically, and their nuclear DNA ploidy patterns were determined. Four were unifocal tumors, and one was a multifocal tumor. The four unifocal tumors were typical serous cystadenomas. However, the multifocal tumor exhibited an increased N/C ratio, irregular nuclear margins, various-sized nuclei, coarse nuclear chromatin, and neural invasion. All tumor cells were stained with antiCA19-9 but none with antiCEA. In the antiCA19-9 staining, the four unifocal tumors and the tumors of the pancreatic tail in the multifocal case were positive only on the apical membrane, whereas the tumor cells of the pancreatic head in the multifocal case were positive within the whole cytosol. The unifocal tumors were diploid with a DNA Index (DI) of 1.0 and proliferation indices (PI) from 4.9 to 20.9% with a mean of 14.4%. In the multifocal case, the tumor in the pancreatic head was aneuploid (DI = 1.9) and had a PI of 27.8%. The multifocal sites in the pancreatic body were aneuploid (DI = 1.9) with a PI of 22.4%. We suggest that the biological property of serous cystadenoma should be revisited.

Fistuloscopy for the management of postoperative intra-abdominal abscesses.

A new technique of fistuloscopy for evaluation and treatment of postoperative intra-abdominal abscesses is described which was used in 8 patients in whom drainage of abscesses following surgery was insufficient. A cholangiofiberscope was introduced into the fistula canal produced by the postoperative drainage. Fistuloscopy was combined with fistulography in all cases to delineate the extent of the inflammatory process. The guidewire was then placed under visual control through the endoscope followed by re-introduction of the drainage tube until a position most effective for pus drainage was achieved. Following this procedure, fever resolved in all 8 patients. Fistuloscopy is recommended to achieve sufficient drainage of intra-abdominal abscesses in those cases where percutaneous ultrasound- or CT-guided drainage fails to lead to significant clinical improvement.

Immunoelectron microscopy of influenza A virus neuraminidase glycoprotein topography.

Using immunoelectron microscopy, the distribution of influenza A virus neuraminidase (NA) glycoproteins was examined, after performing immunoreactions to virions on the grid. With polyclonal antibody, the immunolabels of the glycoproteins were found to be homogeneously distributed, whereas with monoclonal antibody they were found to be distributed in clusters. After destruction of haemagglutinin (HA) but not of NA activity with a high concentration of trypsin, the remaining visible spikes were evenly distributed. This finding was consistent with the absence of immunolabelling with anti-HA antibody, and the homogeneous pattern of immunolabels with anti-NA polyclonal antibody, but not with the clustered labelling with the anti-NA monoclonal antibody. Thus, the immunolabelling image with anti-NA polyclonal antibody was considered to reflect the true one.

[Isolation of MRSA from inpatients, staff and environment in the hospital].

The state of MRSA contamination of inpatients, hospital staff, and the hospital environment was evaluated. Nasal, pharyngeal, and digital samples from 182 patients admitted in September and October, 1990, and 288 hospital staff members and sputum, urine, and feces of the inpatients were cultured. Environmental contamination was examined in samples collected from the air by air sampling and from the floor by the wiping method. The MIC and the coagulase type of the MRSA obtained were determined, and their relationships were evaluated. MRSA was detected in 9.5% of nasal samples, 7% of pharyngeal samples, 10% of sputum samples, 0% of urine samples, and 2.6% of fecal samples from the 182 inpatients. It was detected in 4% of nasal samples, 0.7% of pharyngeal samples, and 1% of digital samples from the 288 hospital staff members. From the environment, MRSA was detected from hospital rooms of the surgery and neurosurgery wards, the nursing room and corridors of the obstetrics and gynecology ward, and the recovery room of the urology ward. The coagulase type of the MRSA obtained was the primarily type II regardless or whether the samples were obtained from the subjects or the environment. Concerning the drug sensitivity, many MRSA strains were highly resistant to DMPPC and FOM, but the sensitivity to RFP was 0.1 microgram/ml or less in all strains except for one highly resistant strain (200 micrograms/ml or above).(ABSTRACT TRUNCATED AT 250 WORDS)

Acidity in the endolymphatic sac fluid of guinea pigs.

The pH value and the DC potential in the endolymphatic sac (ES) were measured using H(+)-selective double-barreled electrodes and the intra- and extradural approach, the pH value in the ES fluid (6.65 +/- 0.12, n = 7) was markedly lower than that in the cerebrospinal fluid (7.39 +/- 0.07, n = 7). Acidity in the ES fluid, under normal (nontreated) conditions may be closely associated with metabolism in the epithelium of the ES.
