[Thyroid cancer with amyloid stroma. Results of a familial study].

We have recently studied a kindred in which there have been 2 proven and 2 probable cases of medullary thyroid carcinoma without pheochromocytoma, hyperparathyroidism or Cushing's disease. Four other members suffered from intestinal occlusion and death occurred in three of them. The family has been traced through 4 generations (80 members) and 47 members could be examined; circulating calcitonin and carcinoembryonic antigen levels were measured. This study leads to two conclusions: 1) Medullary thyroid carcinoma is transmitted as an autosomal dominant trait with a high degree of penetrance. 2) Carcinoembryonic antigen is a valuable tumour marker particularly if no calcitonin radioimmunoassay is avialable for the diagnosis of M.C.T.

[Continuous recording of plasma glucose level after glucose tolerance and 3 different breakfast. Search for a normal post-prandial plasma glucose (author's transl)].

We have studied glucose tolerance test (GTT) and glucose after 3 different breakfast (50, 100, 150 g CHO), in order to define a postprandial glucose level for the detection and control of diabetes mellitus. If the breakfast is sufficient enough (100 or 150 g CHO) the response after breakfast is similar to GTT. The post meal glucose is < 1,5 g/L mean + 2 SD) at 35 mn and < 0,97 g/L at 108 mn. Glucose level over these limits set to search a diabetes. Nevertheless this study is restricted by two points: the subjects are young and the load must be sufficient enough (over 100 g CHO).

[Effects of chronic administration of lithium on thyroxine (T4), triiodothyronine (T3), reverse T3 (rT3) free thyroxine ratio (FTR) and thyreostimuline (TSH) in thyrotoxicosis (author's transl)].

Fourteen hyperthyroid subjects were studied between 3 and 141 days. The reduction of thyrotoxicosis is rarely obtained (2 cases), mainly because on the one hand, correct plasma lithium level (0.7-1.2 mEq/L) is rarely obtained (3 cases only), on the other hand because a high dose of lithium is required, perhaps owing to high clearance of lithium in thyrotoxicosis. The indications of lithium use in thyrotoxicosis are reviewed.

[Thyroiditis : diagnostic and therapeutic features (excluding Graves disease and myxedema) : 13 case-reports (author's transl)].

The incidence of De Quervain's thyroiditis is probably underestimated, since a radioiodine uptake study is not performed in all cases. An isolated cold nodule can reveal Hashimoto's thyroiditis. In other cases, the diagnosis is suggested by otalgia and positive tests for antimicrosomial antibodies. Surgery is often necessary because of the possible association with cancer ; the procedure should be extensive since spontaneously the outcome is often thyroid insufficiency.

[HLA DRW3 and Graves' disease (author's transl)].

HLA A, B, C antigens were studied in 86 caucasians with Graves' disease, using the lymphocytotoxicity microtechnique. Results were compared to those obtained in 336 healthy controls. HLA DR antigens were studied by the technique of microlymphocytotoxicity after long incubation and results compared to those of 100 healthy controls. There is an increased incidence of DRW3: 51.6% as compared to 20% in controls with a high degree of statistical significance pc less than 0,0003. There is also an increased incidence of B8: 44,19% in Graves' disease against 22,47% in controls pc less than 0,001. These results agree with the conclusions of two recent studies where the incidence of DW3 analyzed by a lymphocytic mixed culture was greater than incidence of the B8 antigen.

[Measurement of seminal fluid 5 alpha dihydrotestosterone levels during investigations of sterility (author's transl)].

Two androgens (testosterone, 5 alpha dihydrotestosterone) were measured by radioimmunoassay in the seminal liquid. A statistic comparison of the results has been applied to samples taken from sterile men and classed chiefly according to sperm count. Different groups were explored: normospermia; oligospermia; various azoospermias. No significant differences in testosterone were found between the groups. Levels of DHT are lower in oligospermia, and much more in azoospermia. The signification of the results is discussed.

[Familial surveys in screening for thyroid cancers with amyloid stroma. Report of a personal survey carried out on 80 members of the same family].

We have recently studied a large family of 80 persons in which 47 were examined. The evaluation included history, blood pressure determination, palpation of the thyroid gland and determination of serum carcino-embryonic antigen (C.E.A.) and calcitonin (C.T.). Two members of the kindred had a proven M.C.T. without pheochromocytoma, hyperparathyroidism or Cushing's disease and two others a probable M.C.T. Four members suffered from intestinal occlusion and death occurred in three of them. Our conclusions are: 1) In this family traced through 4 generations: it appears that M.C.T. is transmitted as an autosomal dominant trait with a high degree of penetrance; 2) Our cases associated with those reported in the literature in the past few years point to the existence of a rare but distinct syndrome characterised by the association of M.C.T. and congenital megacolon with hyperplasia of the myenteric plexus; 3) As far as we know, this is the first indication of C.E.A. coupled with elevated calcitonin among several individuals of the same family. We confirm here the conclusions of previous studies: "C.E.A. is a valuable tumour marker which can be used for the detection of M.C.T., particularly if no calcitonin radio-immuno assay is available.

Increase in thiobarbituric acid-reactive substances and vascular complications in type 2 diabetes mellitus.

Lipid peroxidation was assessed by measuring the concentrations of thiobarbituric acid-reactive substances in plasma from 204 Type 2 diabetic patients, relative to 107 controls. The concentrations obtained in diabetic patients (3.08 +/- 0.37 mumol/l) were significantly higher than in controls (2.80 +/- 0.34 mumol/l) (p < 0.0001). Values were also significantly increased in patients with macroangiopathy and/or microangiopathy (3.17 +/- 0.41 mumol/l), relative to patients void of vascular complications (2.92 +/- 0.34 mumol/l) (p < 0.001). Elevated concentrations were independent of the type of vascular complication and their possible associations. In patients without vascular affection, thiobarbituric acid-reactive substances were in significantly higher concentrations in hypertensive (3.07 +/- 0.36 mumol/l) than in normotensive (2.87 +/- 0.29 mumol/l) (p < 0.01) patients. There was a correlation between these values and those of total cholesterol (r = 0.46, p < 0.0001) and triglyceride (r = 0.45, p < 0.0001). Statistical analysis by multivariate logistical regression revealed that among the independent factors (TBARS, APO A1, hypertension, age), thiobarbituric acid-reactive substances constituted the parameter most strongly linked to the existence of vascular complications. This study has evidenced a lipid peroxidation disorder in non insulin- dependent diabetes mellitus, more marked in patients with vascular affection. Thiobarbituric acid-reactive substances appear to be an independent marker of vascular complications in Type 2 diabetes.

Predictive value of age-related acute insulin response to glucose in subjects at risk for type 1 diabetes: results of a 6-year follow-up study from west-France.

The acute insulin response to i.v. glucose (AIRG) was evaluated in 344 first-degree relatives of patients with Type 1 diabetes. In 318 relatives aged 3 to 48 years without islet cell antibody and insulin autoantibody, correlations (p < 0.0006) were found between age and fasting insulinaemia, fasting glycaemia, or AIRG, with a peak during puberty. Assuming that these relatives without islet cells and insulin auto-antibodies have a low risk of developing Type 1 diabetes, we provided a "standard age-related chart" for AIRG with a "low" AIRG defined as a value below the 1st percentile for each pubertal stage. Using these cut-off points, predictive characteristics of a low AIRG for progression towards diabetes within 6 years were analysed. Four relatives developed diabetes and one displayed impaired oral glucose tolerance. Four out of these 5 subjects had islet cell and insulin auto-antibodies, but the other one was negative for these markers. Three of these 5 subjects had low AIRG at entry (30, 24 and 1 months before diabetes, respectively). The two others displayed a steady progressive decline (p < 0.02) of age-related during the follow-up before impaired oral glucose tolerance and diabetes appeared (rate of decline: 15 microU/ml/year). Thus, independently of the presence of islet cell antibodies, the predictive value of a low age-related AIRG during the follow-up is greater than the single low AIRG at entry.(ABSTRACT TRUNCATED AT 250 WORDS)
