[Gynaecological use of prolactin inhibitor bromocriptin (author's transl)].

Bromocriptinee has proved to be effective in treatment of patients for hyperprolactinaemic diseases. The authors suggest that in gynaecology it should be used, first of all, in treating patients for hyperprolactinaemic sterility and galactorrhoea and, in extraordinary cases, for postpartum inhibition of lactation. Bromocriptin should be prescribed only at endocrinological institutions with facilities for follow-up control by serum prolactin assay.

Soft tissue attachment of a filamentous carbon-absorbable polymer tendon and ligament replacement.

Soft tissue structures can be replaced by a filamentous carbon-polylactic acid polymer tissue scaffold. A successful replacement requires a secure bond between the synthetic material and living soft tissue. This attachment has been investigated in a rabbit model. In 23 male, white New Zealand rabbits, the proximal third of the Achilles tendon of the hind leg was resected. Seventeen of these rabbits received composite tendon implants; six received no implants and served as shams. In all cases, the contralateral limb served as a unoperated control. Biological fixation of the synthetic material to the tendinous and myotendinous tissues of the rabbit gastrocnemius system was achieved. This occurred by ingrowth of soft tissue into the 7 mu carbon fiber network. This bond developed rapidly and was mechanically secure under physiologic loading conditions. The anastomoses' strengths were tested in tension over a 12-week period and compared to the breaking strengths of their contralateral, unoperated gastrocnemius systems and the sham systems. After four weeks, the systems with implants had strengths equivalent to those of the unoperated systems. Sham systems were significantly weaker throughout the test period. Histologically, the ingrowth process was associated with little inflammatory response. The ingrown tissue was found to be highly cellular with collagen oriented longitudinally to the tendon. After 12 weeks, the volume of collagenous tissue making up the regrown tendon section appeared nearly normal. Secure biological fixation of the carbon-polylactic acid scaffold to soft tissues may allow it application in repair or replacement of tendon or ligament deficits.

The role of bioengineering in orthopedics.

Orthopedic bioengineering is an example of professionals in distinct disciplines applying their skills to problems that require an interdisciplinary approach. Two current research projects, the development of replacement ligaments and tendons and of an absorbable fracture fixation device, illustrate this integrated approach.

[Hyperprolactinaemic endocrinopathy in gynaecological practice (author's transl)].

Prolactin and, to some extent, the gonadotrophins were determined in the serum of 50 patients with amenorrhoea (oligomenorrhoea) and or galactorrhoea. -- Hyperprolactinaemia was safely recorded from 32 cases. That relatively high number of patients with hyperprolactinaemia was attributed to retrograde examination of sterility patients who had been resistant to therapy. -- In the presence of galactorrhoea prolactin levels were increased in nine of 16 patients, whereas the same levels were increased in only three of 18 patients with postpill amenorrhoea. -- Gonadotrophin levels usually dropped in concomitance with marked hyperprolactinaemia, but they failed to deviate from normal with moderate prolactin increase. -- The above results are likely to underscore the importance of prolactin to the diagnosis of gynaecological endocrinopathies.

[Anovulation syndrome in nulligravidae following intake of hormonal contraceptives (author's transl)].

Evaluated in this paper is clinical experience recorded from 70 young women, aged between 14 and 27 years, with post-pill syndrome. The aetiology of post-pill amenorrhoea was found to be closely related to lability of cycle prior to pill-taking. Nearly 60 per cent of all patients had used Non-Ovlon. 10 patients had used Sequenz-Ovosiston. The prognosis of iatrogenic normogonadotrophic ovarian insufficiency was found to depend on the basal oestrogen level, reaction of corpus endometrium, and serum prolactin. Pregnancy was successfully induced in 53 per cent of all patients who wanted to have a child. The aboration rate accounted for 7.4 per cent. Clinical examination of the newborns did not give any clue as to accumulation of congenital malformations. Roughly 40 per cent of all patients remained to be anovulatory, in spite of intensive treatment.

Mechanical analysis of Kirschner wire fixation in a phalangeal model.

Transverse divisions of the mid-shaft of freshly frozen pig metacarpals were fixed with Kirschner wires of two sizes and using four different configurations. Compared to the usual cross-pin fixation using wires of 0.889 mm. four wires (0.712 mm) eccentrically placed and with their ends hooked provided a 300% improvement in bending rigidity and 170% in bending movement.

Determining skin thickness with pulsed ultra sound.

A high frequency, high resolution ultrasonic echo technique is presented for determining the thickness of human skin. An experimental series is described in which the accuracy of this new technique is demonstrated by comparison with a radiological method of proven accuracy. The "ultrasonic biometric ruler" is shown to provide an accurate, simple, noninvasive method for measuring full-thickness human skin. In addition to the determination of skin thickness, it is demonstrated that the underlying subcutaneous fat and muscle can also be noninvasively "explored" with the possibility of identifying a variety of skin and underlying tissue lesions.

Children and smoking.

An experimental educational programme to reduce childhood cigarette smoking in the Hunter health region (NSW) is described. It is an example of the efforts currently being made to tackle childhood cigarette smoking as an increasing problem, particularly among teenage girls. The key factors influencing children to begin smoking are the role of the family, and pressures from the peer group. Preventive strategies need to be implemented in childhood before the smoking habit becomes firmly established in an individual's lifestyle.

Sphygmomanometry in the pediatric patient.

The indirect measurement of blood pressure in the pediatric population is often difficult or impossible to perform and when values are obtained, they are often unreliable or inaccurate. An accurate measurement of arterial systolic pressure in the upper extremity is obtainable with a properly designed and sized occluding cuff and an ultrasonic device to detect the first Korotoff sound. For the detection of the first Korotkoff sound ultrasonic Doppler devices are superior to the stethoscope. When wider occluding cuffs than usual are applied to the upper extremity, the error of measurement is minimized. A method for accurately measuring end diastolic pressure has yet to be described despite assertions to the contrary.

[Behavior of serum immunoglobulin E in female genital cancer].

More seldom allergic patients suffer from cancer than healthy ones. The level of serum-IgE of these patients is higher whereas IgE of cancerous patients is often lower. 16 patients with gynaecologic cancer and without allergic anamnesis were examined and the results were compared with references in literature. A possible importance of IgE for the physical prevention against tumor is being discussed.

Prostaglandins and primary dysmenorrhoea.

In 100 patients a total of 171 PG determinations in the menstrual blood was performed under standardized conditions. The primary PGE1 and PGF2alpha were determined by RIA. The evaluation of the analysis of the sexual hormones resulted in a group with a biphasic cycle without dysmenorrhoea and a group with a biphasic cycle with dysmenorrhoea. Highly significant differences (p less than 0.01) were found in the PGF2alpha concentration in menstrual blood between a group of healthy women and one of patients with dysmenorrhoea. In dysmenorrhoea the ratio of the PGs investigated is shifted in favour of PGF2alpha. Indomethacin can decrease the PGF2alpha levels in dysmenorrhoea up to 15% (p less than 0.01). All treated patients reported that their conditions had improved. Five out of seven women were completely without any complaints.
